Indication, Technique and Long-term Results after Shoulder Arthrodesis Performed with Plate Fixation.

AIM: Arthrodesis of the shoulder joint is a radical event, so it is difficult to explain its significance to patients. It has been the last resort in hopeless cases. Knowledge of long-term results and evaluation of advantages and disadvantages seems to be helpful in this regard. METHODS: Eleven patients with mean age of 45 (31 - 58) years were operated between 2000 and 2013. All patients could be included in the investigation (FU mean 8.5 [3 - 16] years; 8 male and 3 female; right 9, left 2). Patients had the following indications: persistent instability 5 (2 with epilepsy), rotator cuff rupt

Subacromial decompression surgery for rotator cuff disease.

BACKGROUND: Surgery for rotator cuff disease is usually used after non-operative interventions have failed, although our Cochrane Review, first published in 2007, found that there was uncertain clinical benefit following subacromial decompression surgery. OBJECTIVES: To synthesise the available evidence of the benefits and harms of subacromial decompression surgery compared with placebo, no intervention or non-surgical interventions in people with rotator cuff disease (excluding full thickness rotator cuff tears). SEARCH METHODS: We searched CENTRAL, MEDLINE, Embase, Clinicaltrials.gov and W

How patients sleep after rotator cuff repair: a prospective analysis of pain, position, and recovery.

BACKGROUND: Sleep disturbance is common among patients undergoing rotator cuff repair (RCR), yet the perioperative course of pain-related sleep disruption and the influence of behavioral factors are not well defined. Patients frequently ask perioperative sleep-related questions that lack evidence-based answers. This study aimed to characterize changes in sleep quality and sleeping patterns before and after RCR. METHODS: Adults undergoing primary elective RCR at a single academic center (November 2021-October 2024) were prospectively enrolled. Surveys were completed preoperatively and at 2, 6,

Liposomal Bupivacaine + Bupivacaine Versus Bupivacaine Interscalene Nerve Block Effect on Pain After Rotator Cuff Repair: A Randomized Control Trial.

BACKGROUND: Interscalene nerve blocks (ISB) are commonly used for perioperative pain control after shoulder arthroscopy. Bupivacaine has historically been the primary anesthetic used in ISBs. In 2018, the U.S. Food and Drug Administration approved liposomal bupivacaine for ISBs. This study aims to identify the effect of ISBs using bupivacaine alone (B) versus liposomal bupivacaine + bupivacaine (LBB) on postoperative pain control after arthroscopic rotator cuff repair (RCR). METHODS: A single-center, prospective, double-blinded, randomized, control trial following Consolidated Standards of Re

The use of cold therapy unit in the postoperative period influences pain and narcotic use following arthroscopic rotator cuff repair.

BACKGROUND: Opioid overdose is a leading cause of injury-related death in the United States, prompting calls for reform in surgical prescription practices. Cold therapy units (CTUs) have been widely accepted as an adjunct to pharmacologic therapy for pain management, but these devices are not consistently reimbursed by insurance companies and have not been extensively studied in arthroscopic shoulder surgery. The purpose of our study is to assess the impact of a standardized CTU protocol on postoperative pain and opioid consumption following arthroscopic rotator cuff repair (ARCR), along with

Clinical outcomes of fully arthroscopic latissimus dorsi and teres major transfer for the treatment of posterior-superior irreparable rotator cuff tears.

BACKGROUND: Posterior-superior irreparable rotator cuff tears (PSIRCTs) are a significant challenge in managing active individuals without arthritis traditional latissimus dorsi (LD) transfer is prone to retear due to the tendon's thinness and yields suboptimal outcomes with subscapularis tears. This study evaluates the clinical outcomes of fully arthroscopic combined LD and teres major transfer in patients with PSIRCTs without arthritis. MATERIALS AND METHODS: This retrospective study included patients who underwent fully arthroscopic LD teres major (LDTM) transfer for PSIRCTs from January 2

Bleeding disorder increases the risks of complications following arthroscopic rotator cuff repair.

BACKGROUND: It is imperative to determine patients' risk factors prior to arthroscopic rotator cuff repair (ARCR), so that the physician and patient are both aware of the possible postoperative complications. However, the impact of bleeding disorders on a patient's short-term postoperative outcome has not yet been analyzed. METHODS: A national database was queried for patients undergoing ARCR from 2006 to 2018. Two patient cohorts were defined: patients with a bleeding disorder and patients without a bleeding disorder. In this analysis, outcomes including postoperative complications, hospital

Improved long-term functional outcome after a latissimus dorsi transfer with or without subscapularis muscle lengthening or release.

A brachial plexus birth injury (BPBI) can lead to a limited shoulder function, especially abduction and external rotation. One of the treatment options to restore those shoulder functions is a latissimus dorsi transfer (LDT). The aim of this study is to analyze long-term functional outcome after a single LDT and compare these results with LDT combined with subscapularis muscle lengthening (SSL) or subscapularis muscle release (SSR). This cohort study included 39 patients (≤12 years old) with one-sided BPBI. All patients had an inter- nal rotation- and adduction contracture without glenohumeral

Discrimination between pain and contracture in limited passive motion patients with rotator cuff tear: A STROBE-compliant cross-sectional study.

Either pain or contracture may limit shoulder passive range of motion (PROM) in patients with rotator cuff disease, and an appropriate treatment may be determined according to its cause. If there is no change in PROM under general anesthesia, contracture, rather than pain, may be the underlying condition. Our goal was to devise a physical examination that would help discriminate between pain and contracture in limited PROM patients with rotator cuff tear. This is a STROBE-compliant cross-sectional study. Patients with rotator cuff tears (N = 28) were scheduled for arthroscopic repair. The main

Biceps tenodesis versus tenotomy in isolated LHB lesions: a prospective randomized clinical trial.

INTRODUCTION: Currently there exists no clear evidence concerning the surgical treatment of LHB lesions with either tenotomy or tenodesis. The aim of the study is therefore to evaluate elbow flexion and forearm supination force as well as the biceps muscle distalization according to both techniques in isolated LHB lesions. METHODS: Consecutive patients aged 40-70 years with shoulder arthroscopies for isolated SLAP or biceps pulley lesions were prospectively randomized to arthroscopic suprapectoral intraosseous LHB tenodesis or tenotomy. Postoperative Mobilization After Superior Rotator Cuff Repair: Sling Versus No Sling: A Randomized Prospective Study.

BACKGROUND: Patients are commonly advised to wear a sling for 4 to 6 weeks after rotator cuff repair despite negative effects of early immobilization and benefits of motion rehabilitation. The aim of this study was to compare clinical and radiographic outcomes up to 6 months following rotator cuff repair with and without postoperative sling immobilization. METHODS: We randomized 80 patients scheduled for arthroscopic repair of a small or medium superior rotator cuff tear into sling and no-sling groups (40 patients each). Minimum Five-year Outcomes and Clinical Survivorship for Arthroscopic Transosseous-equivalent Double-row Rotator Cuff Repair.

INTRODUCTION: Despite the widespread use of arthroscopic double-row transosseous-equivalent (TOE) rotator cuff repair (RCR) techniques, midterm outcome data are limited. The purpose of this article was to assess midterm clinical outcomes of patients following arthroscopic TOE RCR using either a knotless tape bridge (TB) repair or knotted suture bridge (SB) repair technique. We hypothesized that there would be significant improvements in patient-reported outcomes with TOE RCR that would be durable over time. We also hypothesized that the knotless TB technique would yield equivalent clinical res
