Anorexia, serum zinc, and immunologic response in small cell lung cancer patients receiving chemotherapy and prophylactic cranial radiotherapy.

Anorexia is a major clinical problem for patients with certain types of cancer. The specific mechanisms that result in this spontaneous decline in food intake remain unknown. In noncancer populations, zinc has been shown to play a role in maintaining normal appetite, taste acuity, and immunocompetence. One purpose of this prospective, longitudinal study of cachexia in ten males with small cell lung carcinoma was to determine if anorexia (caloric intake), perceived taste changes, zinc intake, and impaired cellular immunity were associated with serum zinc concentrations. The average daily calori

Health screening for Gurkha families in UK by chest X-ray, Heaf and blood testing: is it worthwhile?

This paper reports the chest X-rays, Heaf tests and haematology findings on a population of Gurkha wives and their children who arrived in the UK to accompany their husbands on an army base in 1997-8. The conclusions from this study are that there is no benefit from performing chest X-rays, but that screening for rubella and iron deficiency anaemia should continue.

Cell-mediated immunity in canine marrow graft recipients given cyclophosphamide.

The cellular immune status of 48 canine chimeras was studied between 15 days and 3 years after a lethal dose of cyclophosphamide (CY) and an allogeneic marrow graft. Marrow graft recipients were given no further immuno-suppressive treatment. Recovery of total white blood cell counts was complete by day 20 after grafting. Peripheral blood lymphocyte counts did not reach the normal range until day 100. Short-term chimeras (less than 100 days postgrafting) showed marked impairment of cell-mediated immunity as indicated by the inability to respond adequately to nonspecific mitogenic (PHA, ConA, PW

Yield of a contact investigation among children living with drug-resistant TB patients.

<title>BACKGROUND</title>Children are at risk of TB, especially if they are household contacts of someone newly diagnosed. Research is needed on outcome of paediatric household contacts of drug-resistant TB (DR-TB) index patients, where routine follow-up is usually offered instead of preventive treatment. We describe the yield of a contact investigation protocol implemented among children ages 0-14 years living with DR-TB patients.
<title>METHODS</title>A single centre prospective cohort study was conducted to follow-up paediatric household contacts of newly diagnosed (within la

Orbital tuberculosis: Clinicopathological correlation and diagnosis using PCR in formalin-fixed tissues.

This article describes the clinicopathological correlation and challenges encountered in the diagnosis of orbital tuberculosis (TB). Retrospective, interventional case series. A chart review of six patients who underwent biopsy for an orbital mass between January 2012 and December 2013 was performed. Institutional review board approval was obtained for the study. Clinical, radiological, and laboratory findings were documented, and response to antitubercular therapy (ATT) was noted. Age at presentation ranged from 18 to 64 years and duration of complaints varied from 2 weeks to 2 months. Pain,

Latent Tuberculosis Infection and Associated Factors among Health Care Workers in Kigali, Rwanda.

INTRODUCTION: Data are limited regarding tuberculosis (TB) and latent TB infection prevalence in Rwandan health facilities. METHODS: We conducted a cross-sectional survey among healthcare workers (HCWs) in Kigali during 2010. We purposively selected the public referral hospital, both district hospitals, and randomly selected 7 of 17 health centers. School workers (SWs) from the nearest willing public schools served as a local reference group. We tested for latent TB infection (LTBI) using tuberculin skin testing (TST) and asked about past TB disease. We assessed risk of LTBI and past history

High prevalence of tuberculosis infection in HIV-1 exposed Kenyan infants.

BACKGROUND: Infants born to HIV-1 infected mothers may have increased risk for tuberculosis (TB), but the prevalence of TB infection in this population is undefined. In contrast to tuberculin skin tests that are confounded by recent bacille Calmette-Guérin (BCG) vaccination, TB interferon gamma release assays (IGRAs) do not cross-react with BCG and enable detection of TB infection in infancy. METHODS: In a nested observational cohort of HIV-1 infected Kenyan mothers and their infants, we conducted T-SPOT.TB assays on cryopreserved peripheral blood mononuclear cells from 6-month-old infants wi

Contribution of interferon-γ release assays (IGRAs) to the diagnosis of latent tuberculosis infection after renal transplantation.

BACKGROUND: Renal transplant recipients (RTRs), as all immunosuppressed patients, are at increased risk of reactivating latent tuberculosis infection (LTBI). Detecting LTBI in this population is therefore important to prevent active TB. The tuberculin skin test (TST) has a poor sensitivity in this setting. METHODS: The aim of this prospective study was to compare the diagnostic performance of the TST and two interferon-γ release assays (IGRAs): T-SPOT.TB (Oxford Immunotec, Oxford, UK) and QuantiFERON Gold In-Tube (QGIT; Cellestis, Australia), performed simultaneously, for the detection of pat

Management of tuberculosis and latent tuberculosis infection in human immunodeficiency virus-infected persons.

The syndemic of human immunodeficiency virus (HIV)/tuberculosis (TB) co-infection has grown as a result of the considerable sociogeographic overlaps between the two epidemics. The situation is particularly worrisome in countries with high or intermediate TB burden against the background of a variable HIV epidemic state. Early diagnosis of TB disease in an HIV-infected person is paramount but suffers from lack of sensitive and specific diagnostic tools. Enhanced symptom screening is currently advocated, and the wide application of affordable molecular diagnostics is urgently needed. Treatment o

Successful expression and purification of DPPD using a codon optimized synthetic gene.

DPPD (Rv0061) is a difficult to express protein of Mycobacterium tuberculosis that elicits strong and specific delayed type hypersensitivity reactions in humans infected with M. tuberculosis. Therefore e DPPD is a molecule that can improve the specificity of the tuberculin skin test, which is widely used as an aid for the diagnosis of tuberculosis. However, a pitfall of our initial studies was that the DPPD molecule used to perform the skin tests was engineered as fusion molecule with another Mycobacterium protein. This approach was used because no expression of DPPD could be achieved either a

Clinical manifestation and outcome of tuberculosis in children with acquired immunodeficiency syndrome.

BACKGROUND: Atypical clinical manifestations and rapid progression of tuberculous disease (TB) are well-recognized in adults with the acquired immunodeficiency syndrome (AIDS). There are few reports of children with AIDS and TB. We report the manifestations, clinical course and outcome of 12 pediatric patients with AIDS and TB. METHODS: The charts of all children admitted to our institution, from 1989 through 1994, with the diagnoses of AIDS and culture-proved TB were reviewed. RESULTS: Twelve children between the ages of 2 months and 13 years fit the criteria. The mean time between the diag

[A case of local reactivation of tuberculous peritonitis with suspected gastric submucosal tumors after laparotomy].

After complaining of right hypochondrial pain, a 36-year old man was diagnosed as having a gastric submucosal tumor at a nearby hospital and admitted to Chiba Cancer Center Hospital. On admission, a hard and immobile mass was palpable on the right upper quadrant. Laboratory findings were within normal limits. A chest X-ray revealed bilateral pleural adhesions and streaking shadows in the bilateral lung area. These findings were assumed to be due to a prior tuberculous infection. Upper gastrointestinal series and endoscopic examination showed a protruding lesion on the stomach body. An abdomina

The phenotypic distribution and functional profile of tuberculin-specific CD4 T-cells characterizes different stages of TB infection.

BACKGROUND: Recent publications have suggested that altered proportions of functional CD4 T-cell subsets correlate with active pulmonary TB. Also, CD27-expression on tuberculin-activated IFN-γ(+) CD4 T-cells is known to differ significantly between patients with active pulmonary TB and healthy TB-unexposed BCG vaccinees. Here, we explore links between CD4 T-cell phenotype, multiple functional subsets, and control of TB. METHODS: We examined ex-vivo overnight tuberculin activated CD4 T-cells in regards to CD27-expression and the activation markers, CD154 upregulation, IFN-γ, TNF-α, IL-2, and d
