Brain aspergilloma in an immunocompetent individual: A case report.

BACKGROUND: Aspergillosis is caused by fungi from the Aspergillus species, mainly Aspergillus fumigatus. Patients with aspergillosis typically have an underlying immunosuppression, and it is rare within the central nervous system (CNS) in an immunocompetent host. The mortality rate is as high as 90% if untreated, and the diagnosis is usually delayed due to nonspecific clinical presentation. This study described a case of CNS aspergillosis in an immunocompetent patient, through which we sought to discuss the current knowledge regarding brain Aspergillus focusing in the immunocompetent host. Exploring Balloon Sinuplasty Adverse Events With the Innovative OpenFDA Database.

BACKGROUND: Balloon sinuplasty (BSP) has become a common, minimally invasive treatment for chronic sinusitis. Like any surgical procedure, there are definite risks when using it as a stand-alone procedure or in conjunction with other revision procedures. OBJECTIVE: To explore the adverse events of BSP using the openFDA cloud with regard to malfunction and injury following surgery. METHODS: Between January 2015 and December 2018, the Food and Drug Administration's (FDA) openFDA database was queried for adverse event use. Reports were further categorized by procedure location, injury type, dev

Adenoid Cystic Carcinoma of the Cavernous Sinus - Otolaryngological Sequelae of Therapy: Case Report.

A 60-year-old woman, otherwise in a good health condition, was first admitted to the hospital with a year-long tingling sensations of the right half of the face, which gradually turned into severe neuralgia corresponding to maxillary (V2) and mandibular (V3) branches of the right trigeminal nerve. MRI scans of the head revealed an unusual hyperplastic or inflammatory changes along the brain base, cavernous sinus extending toward the pterygopalatine fossa, and lateral pterygoid on the right side. Meningioma was suspected and neuralgia was treated conservatively. About 2 years later, due to seve

Comparison of anatomical abnormalities in patients with limited and diffuse chronic rhinosinusitis.

BACKGROUND: The role of anatomical abnormalities in the pathogenesis of chronic rhinosinusitis (CRS) remains unclear. We hypothesized that anatomical abnormalities causing obstruction of mucus drainage would be more prevalent in patients with limited CRS than in patients with pansinusitis, who are more likely to have a generalized mucosal abnormality. METHODS: The computed tomography (CT) scans of patients with limited sinusitis (maxillary sinus and/or the ostiomeatal unit), patients with diffuse bilateral disease, and asymptomatic controls were studied. Lund-Mackay scores were calculated and

Acute neurological deficits in a young adult with cystic fibrosis.

An abrupt onset of a neurological deficit is a rare occurrence in patients with cystic fibrosis (CF). As many CF patients have indwelling intravenous catheters, one of the complications may be deep venous thrombosis. Cerebral thromboembolism through an intracardiac shunt should be considered in CF patients who develop unexplained acute neurological deficits. We report on the case of a 19-year-old CF patient with insulin-dependent diabetes mellitus who was on oral contraceptives and had a Port-A-Cath(R) in place. The patient developed an acute neurological deficit after pulmonary function testi

Burst pressure of different suturing techniques for simulated oroantral communication closure: an ex vivo porcine skin study.

PURPOSE: Oroantral communications (OACs) most commonly occur following tooth extraction in the posterior maxilla and require prompt surgical closure to prevent chronic oroantral fistula and maxillary sinusitis. Although numerous flap techniques have been described, evidence regarding the optimal suturing technique for OAC closure remains limited. This study aimed to compare the burst pressure resistance of different suturing techniques using an ex vivo porcine skin model. METHODS: An experimental biomechanical study was performed using unfixed porcine donor skin. Standardized 1-cm and 2-cm in…

Persistent Unilateral Nerve Paresthesia Following Suspected Viral Infection.

The infraorbital nerve, a terminal branch of the maxillary division (V2) of the trigeminal nerve, supplies sensation to the upper lip, lower eyelid, and midface. Neuropathy in this distribution typically presents as paresthesia with or without anesthesia, dysesthesia, and/or allodynia. While trauma, dental pathology, sinusitis, and certain neurological conditions are possible causes, emerging literature has identified post-viral neuropathies, including cranial mononeuropathies, as potential sequelae of COVID-19. This case highlights a rare presentation of presumed post-viral infraorbital neuro

Orbital apex syndrome in a man with rhino-ocular cerebral mucormycosis: a case report.

BACKGROUND: Orbital apex syndrome is a symptom complex of visual loss and ophthalmoplegia resulting from a disease involving the orbital apex. It can be caused by inflammation, infection, and malignancies. Mucormycosis is an infection caused by filamentous saprophytes of the order Mucorales. It is ubiquitous, and the infection can occur from ingesting contaminated food, inhaling spores, or injecting the disrupted skin or wounds. It is mainly a disease of the immunocompromised, affecting patients with poorly controlled diabetes, organ transplant recipients, and patients with hematological malig

Bacterial Brain Abscess and Life-Threatening Intracranial Hypertension Requiring Emergent Decompressive Craniectomy After SARS-CoV-2 Infection in a Healthy Adolescent.

Acute coronavirus 2 (SARS-CoV-2) infection usually results in mild symptoms, but secondary infections after SARS-CoV-2 infection can occur, particularly with comorbid conditions. We present the clinical course of a healthy adolescent with a brain abscess and life-threatening intracranial hypertension requiring emergent decompressive craniectomy after a SARS-CoV-2 infection. A 13-year-old healthy immunized male presented with invasive frontal, ethmoid, and maxillary sinusitis and symptoms of lethargy, nausea, headache, and photophobia due to a frontal brain abscess diagnosed three weeks after s…

A Pilot Study Assessing the Incidence of Chronic Sinusitis Following Le Fort I Osteotomy in Maxillofacial Surgery.

Orthognathic surgery utilizing a Le Fort I osteotomy is performed regularly by oral surgeons to correct midface and dental occlusal abnormalities, yet little has been written discussing the impact these operations may have on sinonasal function. The objective of this study was to assess the incidence of objective sinonasal inflammation and subjective sinonasal symptoms following the use of Le Fort I osteotomies for maxillary advancement surgery.Thirty-eight subjects who previously underwent Le Fort I osteotomies for purposes of elective orthognathic surgery were enrolled retrospectively to ass

[Endoscopic surgery of chronic sinusitis].

Between 1984 and 2000 the author has performed endoscopic sinus surgery (ESS) in 1072 patients with chronic sinusitis resistant to medical therapy. In 201 patient (19%) the operation was limited to the maxillary sinus and in 871 patients (81%) multiple cavities were involved. Ethmoidectomy was performed for 749 patients (70%) of which polyposis was found in 315 (29%), bilateral in (87%). History of previous sinus surgery was found in 399 patients (37%). This retrospective study with a long-term follow-up (average 8.6 years) analyzes the objective and subjective result in 212 patients after ESS
