Developmental hip dysplasia treated by total hip arthroplasty using a cementless Wagner cone stem in young adult patients with a small physique.

BACKGROUND: Developmental hip dysplasia (DDH) may lead to severe acetabular and femoral abnormalities that can render total hip arthroplasty (THA) challenging, especially in DDH patients with a small physique. Most conventional cemented or cementless femoral components are often difficult to implant in the narrow femoral canal and require slight version correction during surgery. The aim of this study was to present the mid-term results of THA in the treatment of DDH patients with a small physique using a cementless Wagner cone prosthesis (Zimmer®, US). METHODS: Between January 2006 and March

Displaced humeral shaft fractures: Assessment of fracture union and complications following dual plate fixation using an anterior approach.

The humeral bone is subject to torsional forces. In case of displaced shaft fractures, internal fixation remains the standard of care. This retrospective two-center study assessed the fracture union rate and complications after dual 3.5 mm locking compression plate (LCP) fixation using an anterolateral approach. Over a 9-year period, 38 patients underwent surgery in two centers. They had a mean age of 53.7 years (15-97, ± standard deviation (SD) 26) and there were three open fracture cases (7.9 %). The dominant side was affected in 21 cases (55.3 %) and there were 11 polytrauma patients (29 %)

Long-Term Functional Outcomes and Complications of Intra-Articular (AO type B, C) Distal Humerus Fractures in Adults: A Retrospective Review.

Introduction Treatment of intra-articular fractures of the distal humerus is challenging due to their complexity, comminution, and associated complications. The evolution of surgical approaches and the design of elbow-specific implants over the last decades have failed to improve clinical and radiological outcomes. Studies are sparse regarding the long-term influence of surgical treatment of these types of fractures in the upper limb function. The purpose of the current study was to retrospectively review the long-term functional outcome and complications of all intraarticular (AO type B, C) d

Clinical and radiological outcome of Mason-Johnston types III and IV radial head fractures treated by an on-table reconstruction.

BACKGROUND: Only few methods treating comminuted radial head fractures have been established providing sufficient joint reconstruction, restoring radial length and enabling early joint mobilization. When an anatomical reconstruction using open reduction and internal fixation is not possible, radial head resection or primary arthroplasty is often conducted. An "Ex situ/on-table" reconstruction is widely disregarded but can be an option. The purpose of this study was to evaluate the functional and radiological outcome of comminuted radial head fractures treated with an "on-table" reconstruction

Hybrid coracoclavicular and acromioclavicular reconstruction in chronic acromioclavicular joint dislocations yields good functional and radiographic results.

PURPOSE: Optimal treatment of chronic unstable acromioclavicular (AC) joint dislocations (stage 3-5 according the Rockwood classification) is still debated. Anatomic coracoclavicular (CC) reconstruction is a reliable option in terms of two-dimensional radiographic reduction, clinical outcomes, and return to sports, but there remain concerns regarding anterior-posterior stability of the AC joint with CC ligament reconstruction alone. The aim of the present study was to describe the mid-term results of a new hybrid technique with CC and AC ligament reconstruction for chronic AC joint dislocation

Humeral capitellum fracture combined with humeral lateral column injury: A novel classification system and treatment algorithm.

BACKGROUND: Shear humeral capitellum fracture (CF) treatment can be complicated by comminution of the distal lateral humeral column (LHC). Although treatment with a 3.5 mm posterolateral distal humerus plate with support (PDHPWS) has been proposed, its indications have not yet been outlined. The purpose of this study was to describe a classification system for this fracture pattern and provide a therapeutic algorithm to avoid complications associated with PDHPWS fixation. METHODS: Thirty-four patients who underwent surgical treatment for CF with LHC comminution were enrolled. The humeral capi

Reduction and stabilization of radial neck fractures by intramedullary pinning: a technique not only for children.

BACKGROUND: Isolated radial neck fractures occur only in rare cases. The majority of cases are non-displaced or minimally displaced and can be treated conservatively. Conservative treatment, however, might result in secondary displacement and/or malunion. On the other hand, open reduction and internal fixation (ORIF) as standard surgical approach in adults is associated with non-union, implant-related complications and reduced range of motion. For isolated radial neck fractures with an intact radial head, the procedure of centromedullary pinning--as widely used in the treatment of paediatric r

Early follow-up outcomes after treatment of degenerative disc disease with the discover cervical disc prosthesis.

BACKGROUND CONTEXT: Although anterior cervical decompression and fusion has been used successfully in the treatment of symptomatic radiculopathy and/or myelopathy, biomechanical studies have suggested possible deleterious effects of cervical fusion on adjacent level kinematics. The emergence of cervical arthroplasty as an alternative to arthrodesis may restore a more normal spinal movement and reduces a kinematic strain on adjacent segments. To date, with existing prostheses, the risk adjacent segment disease has not been proven to be reduced using artificial disc replacement. PURPOSE: The st

Paternal GNAS mutations lead to severe intrauterine growth retardation (IUGR) and provide evidence for a role of XLαs in fetal development.

CONTEXT: Heterozygous GNAS inactivating mutations cause pseudohypoparathyroidism type Ia (PHP-Ia) when maternally inherited and pseudopseudohypoparathyroidism (PPHP)/progressive osseous heteroplasia (POH) when paternally inherited. Recent studies have suggested that mutations on the paternal, but not the maternal, GNAS allele could be associated with intrauterine growth retardation (IUGR) and thus small size for gestational age. OBJECTIVES: The aim of the study was to confirm and expand these findings in a large number of patients presenting with either PHP-Ia or PPHP/POH. PATIENTS AND METHO

Dislocation of the elbow with fractures of the coronoid process and radial head.

INTRODUCTION: The aim of the study was to evaluate the factors relevant to prognosis after operative treatment of an elbow dislocation/fracture involving the coronoid process and the radial head. In 30-50% of cases, elbow dislocations are accompanied by concomitant bony injuries. Here, the ulnar coronoid process and the radial head are particularly crucial to the stability of the elbow joint. MATERIALS AND METHODS: In a retrospective study, 27 out of 37 patients who were treated surgically in our clinic between 1990 and 1999 for elbow dislocation with involvement of the coronoid process and t
