Cluster B Personality Disorder Is Independently Associated With Opioid Abuse at Two and Five Years After Primary Total Knee Arthroplasty.

BACKGROUND: Psychiatric comorbidities are associated with worse results after total knee arthroplasty (TKA), but outcomes in patients who have personality disorders (PD) remain poorly defined. This study evaluated complications and opioid-related outcomes after primary TKA in patients who have PD. METHODS: Patients who underwent primary TKA from 2010 to 2022 were identified in a national all-payer claims database. Patients who had PD were identified by diagnosis codes and classified by the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition cluster. Because they carried a gre

The impact of the COVID-19 pandemic on patients with personality disorders.

Three years after the outbreak of COVID-19 pandemic, numerous epidemiological studies confirm its significant psychological impact. Recent m … 5 Most studies on the impact of the pandemic on patients with personality pathology have focused on borderline personality disorder (BPD).6 Social distancing measures during the pandemic and increased sense of loneliness are serious aggravating factors for patients with BPD and may cause anxieties of abandonment and rejection, social withdrawal, and feelings of emptiness. Inclusion of coexisting morbidity in a TBSA% and age based model for the prediction of mortality after burns does not increase its predictive power.

INTRODUCTION: Several models for predicting mortality have been developed for patients with burns, and the most commonly used are based on age and total body surface area (TBSA%). They often show good predictive precision as depicted by high values for area under the receiver operating characteristic curves (AUC). However the effect of coexisting morbidity on such prediction models has not to our knowledge been thoroughly examined. We hypothesised that adding it to a previously published model (based on age, TBSA%, full thickness burns, gender, and need for mechanical ventilation) would furthe

The Grey Zone Project: Risk-Based Classification of ABCD1 Variants in X-Linked Adrenoleukodystrophy.

Newborn screening (NBS) for X-linked adrenoleukodystrophy (ALD) enables early identification of boys at risk for adrenal insufficiency (AI) and cerebral ALD (CALD). However, NBS frequently identifies ABCD1 variants of uncertain significance (VUS), which are associated with only borderline-elevated C26:0-lysophosphatidylcholine (LPC(26:0)) levels. Traditional American College of Medical Genetics and Genomics (ACMG) pathogenicity classification does not account for age-dependent penetrance or the broader phenotypic spectrum, complicating risk assessment and clinical management. Through the Grey

Suicide among youth and young adults, 15 through 24 years of age. A report of 392 cases from Paris, 1989-1996.

The aims of our study were (1) to examine the socio-demographic, clinical characteristics, autopsy and toxicological findings in 392 youth suicides in Paris, between 1989 through 1996, and (2) to analyze the psychodynamic determinants leading up to the onset of the suicide. During the eight-year study period 392 suicides involving young people were investigated at the Institute of Forensic Medicine of Paris. Two hundred and sixty victims (66%) were males. The mean age was 22 years in both sexes. Fifteen percent of the victims were below 20 years. Ninety-two percent of the subjects were single.

Defining competencies for language-discordant care in migrant and refugee mental health: an international modified e-Delphi study.

BACKGROUND: Migrants and refugees experience a higher prevalence of mental disorders than host populations, and language discordance undermines care from help-seeking through diagnosis, adherence and outcomes. The WHO Refugee and Migrant Health: Global Competency Standards for Health Workers set broad behavioural expectations for individual health workers but do not operationalise language-discordant care, leaving clinicians and administrators without measurable standards. OBJECTIVE: To develop an international, consensus-based competency framework for healthcare professionals working in lang

Interactions of factors in self-injuries among enrolled students: a network approach.

PURPOSE: Suicidal and non-suicidal self-injuries are types of self-directed violence that can become complex health issues. This study assessed how and to what degree the factors of self-injuries are interrelated among enrolled students. METHODS: A total of 1481 students were recruited from college and middle or secondary schools, and 1465 (98.92%) subjects comprised the final sample. Mixed graphical models were used to establish network structures. Also explore their shortest paths and conduct a regression analysis. RESULTS: Of the 1465 students, we observed intersections that connected the

Dense-map genome scan for dyslexia supports loci at 4q13, 16p12, 17q22; suggests novel locus at 7q36.

Analysis of genetic linkage to dyslexia was performed using 133,165 array-based SNPs genotyped in 718 persons from 101 dyslexia-affected families. Results showed five linkage peaks with lod scores >2.3 (4q13.1, 7q36.1-q36.2, 7q36.3, 16p12.1, and 17q22). Of these five regions, three have been previously implicated in dyslexia (4q13.1, 16p12.1, and 17q22), three have been implicated in attention-deficit hyperactivity disorder (ADHD, which highly co-occurs with dyslexia; 4q13.1, 7q36.3, 16p12.1) and four have been implicated in autism (a condition characterized by language deficits; 7q36.1-q36.2,

Adolescent suicide.

In the introduction to this report our committee, with its focus on adolescent development, expressed its concern that adolescent suicidal behavior represented a grave crisis in the adolescent, a crisis not only in the development of the adolescent but one that endangers the existence of the adolescent. The possibility of a fatal outcome is abhorrent to us as physicians and psychiatrists, as it is to all those entrusted with the care and development of our fellow human beings. Consequently, we explored the ways in which developmental and other forces lead to adolescent suicide and the measures

Prevalence and 10-Year Stability of Personality Disorders From Adolescence to Young Adulthood in a High-Risk Sample.

BACKGROUND: With the implementation of the 11th edition of the International Classification of Diseases (ICD-11) in early 2022, there will be a radical change in the framework and process for diagnosing personality disorders (PDs), indicating a transition from the categorical to the dimensional model. Despite increasing evidence that PDs are not as stable as previously assumed, the long-term stability of PDs remains under major debate. The aim of the current paper was to investigate the categorical and dimensional mean-level and rank-order stability of PDs from adolescence into young adulthood

Experiences in close relationships, interpersonal trust and OXTR expression in individuals with childhood maltreatment.

BACKGROUND: Social cognition is essential for forming and maintaining intimate relationships. Individuals with a history of childhood maltreatment (CM) often show impairments in interpersonal functioning, including trust and romantic attachment. The oxytocin system has been implicated in social behavior, yet the role of oxytocin receptor (OXTR) gene expression in CM-related social functioning remains insufficiently understood. This study examined the impact of CM on social cognition in a large web-based sample and explored the association between CM and OXTR gene expression in a well-character

Psychometric properties of the Spanish structured interview of personality organization revised (STIPO-R) in a clinical and community sample.

BACKGROUND: The diagnosis of personality disorders (PD) remains challenging in clinical practice. The Diagnostic and Statistical Manual of Mental Disorders reviewed (DSM-5-TR) proposes an alternative diagnostic hybrid model (AMPD), that emphasizes dimensional assessment of personality pathology, particularly severity of impairment in personality functioning (Criterion A), a construct conceptually derived from Kernberg’s model of personality organization (PO) but operationalized in a highly simplified form. However, few structured diagnostic interviews aligned with this framework are available

Help-seeking patterns for non-suicidal self-injury across borderline symptom profiles in a large sample of university students.

BACKGROUND: Non-suicidal self-injury (NSSI) is common in young adults and may represent a marker of current or emerging borderline pathology. However, rates of help-seeking for NSSI are low, potentially delaying recognition of clinically relevant borderline symptoms. To our knowledge, research on NSSI help-seeking has not examined whether help-seeking differs according to underlying borderline symptom profiles. This study therefore examined whether patterns of NSSI help-seeking differ across borderline symptom profiles in a community sample of university students. METHODS: In this cross-secti

Elevated Negative Affectivity and Psychoticism in individuals with active Nonsuicidal Self-Injury (NSSI) compared with individuals with previous or no NSSI.

BACKGROUND: Non-suicidal self-injury (NSSI) is a transdiagnostic symptom associated with substantial clinical burden and increased risk of suicidal behavior. Although NSSI occurs across a wide range of psychiatric disorders, research has consistently linked it to maladaptive personality traits. However, most studies have compared individuals with NSSI to healthy controls, limiting our understanding of which personality traits are specifically associated with NSSI. The inclusion of clinical control groups may therefore help clarify which personality traits are associated with NSSI rather than w

Decoding personality functioning: the impact of DSM-5 pathological traits mediated by emotion dysregulation and metacognition.

BACKGROUND: Personality disorders (PDs) have long been studied in adulthood, with relatively limited attention to their developmental course during adolescence. Recent research, however, underscores the importance of early identification and intervention in youth, as personality pathology can be reliably detected in this period and is associated with significant functional impairments. Grounded in the DSM-5 Alternative Model for Personality Disorders (AMPD), the present study explores potential intrapsychic mechanisms linking maladaptive trait domains to global personality functioning in adole

Psychosocial functioning in personality disorders.

The present paper takes a broad perspective on the psychosocial functioning in adult patients with personality disorders. We start with a working definition, then we report on psychosocial functioning in personality disorders from both categorical and dimensional perspectives of personality disorder. We add a section on assessment tools which may be used in this context. We then address the question of how problematic psychosocial functioning may be addressed in psychotherapy and other treatment contexts, when it comes to supporting the person's move towards sustained recovery. We add a lived

Intra- and interpersonal emotion regulation are altered in individuals with childhood maltreatment: cross-sectional associations and effects on daily life mood.

BACKGROUND: Childhood maltreatment (CM) is a potent predictor of lifelong emotional and psychological difficulties. We investigated how CM affects intra- and interpersonal emotion regulation (ER) processes and explored the impact of these ER difficulties on daily life mood. METHODS: We explored the CM-ER association in two studies. Data and code are available at https://osf.io/cbkyj/ . Study 1 tested pre-registered hypotheses ( https://osf.io/2kt35 ) on the association between CM and self-reported ER difficulties in a web-based sample (N = 553). Study 2 used ecological momentary assessment da

Sex differences in personality dysfunction in help-seeking adolescents.

INTRODUCTION: Understanding sex differences is crucial for improving diagnosis and treatment for personality disorders (PDs). This study aimed to investigate sex differences in personality dysfunction as per Criterion A of the DSM-5 Alternative Model of Personality Disorders in help-seeking adolescents. METHODS: The sample comprised 706 adolescent patients (mean age = 15.4 years; 80.88% females). Personality dysfunction was assessed using the Semi-Structured Interview for Personality Functioning DSM-5 (STiP 5.1). RESULTS: Females showed significantly higher overall personality dysfunction (C
