Case Report: Uremic encephalopathy in acute kidney injury caused by abdominal sepsis.

Uremic encephalopathy encompasses a wide range of central nervous system disorders resulting from acute or chronic kidney damage. The pathogenesis of this syndrome is multifactorial and includes the accumulation of uremic toxins, hormonal imbalance, electrolyte and acid-base imbalance, blood-brain barrier dysfunction, and systemic inflammation. There are no specific clinical or laboratory markers for uremic encephalopathy, and the diagnosis is often made retrospectively, based on the dynamics of neurological status. The diagnostic process is further complicated by the presence of comorbid and

Association of Body Mass Index With In-Hospital Complications After Abdominal Firearm Injury.

BackgroundBMI is available early in trauma care, but its association with complications after abdominal firearm injury remains unclear. We evaluated the association between BMI and trauma outcomes in adults with these injuries.MethodsWe analyzed the 2022 American College of Surgeons Trauma Quality Improvement Program (ACS-TQIP) database. Adults with abdominal firearm injury, abdominal AIS ≥2, and BMI 18.50-44.99 kg/m2 were stratified by BMI class: normal (18.50-24.99), overweight (25.00-29.99), class I (30.00-34.99), class II (35.00-39.99), and class III obesity (40.00-44.99); sex-interaction

An unexpected finding of cardiac injury penetrating the diaphragm in a suspected abdominal trauma case.

We report a rare case of penetrating abdominal trauma involving right ventricular injury. An 81-year-old man presented with a stab wound in the upper abdomen and hemodynamic instability. Hemorrhagic shock was initially suspected to be due to intra-abdominal organ damage. Emergency laparotomy revealed no evidence of intra-abdominal organ injury; however, subsequent thoracotomy and sternotomy identified cardiac tamponade from right ventricular injury caused by a penetrating object that traversed the diaphragm. Prompt surgical intervention enabled successful resuscitation. This case highlights th

Isolated Duodenal Injury in Blunt Abdominal Trauma in Children: A Series of Five Cases.

Blunt abdominal trauma is one of the leading causes of trauma-related deaths in children. The commonly affected organs include the spleen, liver and kidneys, followed by uncommon ones such as the pancreas and intestine. Isolated duodenal injury is very rare and, at times, poses a diagnostic challenge due to the paucity of clinical symptoms and signs. Hence, delayed presentation is not uncommon. In this write-up, five such cases are presented with emphasis on their clinical presentation, diagnosis, management and outcome.

From penetrating abdominal injury to enterocutaneous fistula, a deadly outcome: A case report.

Enterocutaneous fistula is a dreaded complication by most surgeons especially after emergency abdominal surgery. It can also occur spontaneously from an underlying disease. The pathology is demanding both mentally and physically and causes medical and nursing problems for the affected individual. In this case report we present a timeline of a young 4-year-old boy who sustained penetrating abdominal-perineal injury from a fall and later presented with peritonitis. His condition progressed to complicate into enterocutaneous fistula and succumbed unfortunately due to multifactorial reasons. This

Successful surgical management of a combined abdominal and thoracic penetrating injury: a case report.

Penetrating rebar injuries are extremely rare occurrences, but they are very life-threatening, particularly when involving the thoracic and abdominal cavities. The surgical approach to these traumatic injuries depends upon the length and diameter of the rebar as well as the path of penetration into the abdominal and thoracic regions. Due to the highly uncommon occurrence of penetrating rebar injuries, there is very limited information and studies pertaining to this topic in the literature. In this case report, we present a 43-year-old male patient sustaining a rebar penetrating injury, with th

Ultrasonography in thoracic and abdominal stab wound injury: results from the FETTHA study.

BACKGROUND: While the role of Extended Focused Assessment with Sonography in Trauma (eFAST) is well defined in the management of severe blunt trauma, its performance in injuries caused by stab wounds has been poorly assessed. METHODS: Prospective single centre study which included all patients with stab wounds to the thorax or abdomen between December 2016 and December 2018. All patients underwent initial investigation with both eFAST and CT scan, except in cases of haemodynamic or respiratory instability, and in cases with a positive diagnosis by eFAST in which case surgery without CT scan w

High-Velocity Penetrating Abdominal Injury Secondary to a Motorized Wire Brush in the Workplace.

Penetrating injuries are one of the most common types of workplace accidents. The majority of these injury patterns are due to non-missile type, low-velocity sharp objects. We present an uncommon presentation of subacute abdominal pain secondary to non-missile type, high-velocity workplace injury. It is important to keep a wide differential in mind when evaluating patients with lingering abdominal pain without an obvious cause. Additionally, collecting the patient's employment history, occupational exposures, and job tasks are important when a workplace injury is suspected. We present the case

Preventive effect of sanguinarine on intestinal injury in mice exposed to whole abdominal irradiation.

Intestinal injury is one of the major side effects that are induced by medical radiation exposure, and has limited effective therapies. In this study, we investigated the beneficial effects of sanguinarine (SAN) on intestinal injury induced by ionizing radiation (IR) both in vitro and in vivo. Mice were exposed to whole abdominal irradiation (WAI) to mimic clinical scenarios. SAN was injected intraperitoneally to mitigate IR-induced injury. Histological examination was performed to assess the tissue injuries of the spleen and small intestine. A small intestinal epithelial cell line-6 (IEC-6) w

Resveratrol against lung injury in an ischemia/reperfusion model of abdominal aortic rupture.

BACKGROUND: The aim of this study was to examine the effects on the lungs of ischemia/reperfusion injury in a ruptured abdominal aortic aneurysm model in rats and to investigate the potential protective effects of resveratrol. METHODS: Thirty-two male Sprague-Dawley rats were randomly divided into four groups: control, ischemia/reperfusion, sham (ischemia/ reperfusion + solvent/dimethyl sulfoxide), and ischemia/reperfusion + resveratrol. In the groups subjected to ischemia/reperfusion, following 60-min shock to the abdominal aorta, vascular clamps were attached from the levels of the infraren

CT features of blunt abdominal aortic injury: an infrequent but life-threatening event.

Blunt injury to the abdominal aorta is a rare but potentially fatal event. Despite being much less common than thoracic aorta involvement, it carries high morbidity and mortality. Computed tomography (CT) has become the standard method for evaluating the trauma patient and can provide a rapid accurate diagnosis of aortic injury. Depending on the magnitude of the traumatic forces, aortic injuries may be present in blunt abdominal trauma and appear as a spectrum of disease ranging from small intraluminal defects to full-thickness circumferential ruptures. We classify lesions into four groups as

Gut Microbiota Was Involved in the Process of Liver Injury During Intra-Abdominal Hypertension.

Background: Intestinal damage caused by intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) can lead to the ectopic gut microbiota, which can contribute to liver injury via portal veins. Therefore, it is speculated that gut microbiota disorder caused by IAH/ACS may result in liver injury. The relationship between gut microbiota and IAH/ACS-related liver injury was investigated in this study. Methods: A model of IAH was established in rats, and 16S rRNA sequencing was analyzed for gut microbiota in the feces of rats. The elimination of gut microbiota was completed by ant

Challenges in management of an isolated intrapancreatic Common Bile Duct injury in an abdominal trauma.

INTRODUCTION: Biliary tract injuries are rare following abdominal trauma. If detected late, outcome is less favourable. It adds to morbidity if there is involvement of head of pancreas or duodenal wall. CASE REPORT: We present a case of an adult male with sharp and blunt trauma over the right side of the abdomen with omentum protruding out. Exploratory laparotomy revealed non expanding paraduodenal hematoma without evidence of solid or hollow viscous injury. Post-operative day 2 drain showed bilious content. Contrast Enhanced CT scan ruled out the solid or hollow viscous injury. Magnetic Reso

Isolated gallbladder injury from blunt abdominal trauma: A rare co-incidence.

Gallbladder injury resulting from blunt abdominal trauma is a rare entity and generally associated with other intra-abdominal injuries. Incidence of isolated gallbladder injury has not been reported yet. The most common mechanism of injury reported is road traffic accident. Diagnosis is usually made on imaging as clinical presentation may vary from no symptoms to peritonitis due to extravasation of bile in the abdominal cavity. Cholecystectomy is the treatment of choice and minimally invasive approach can be considered in haemodynamically stable patients.
