Measurement of the nasal mucosa is a challenging task. There are many different methods; each with advantages and disadvantages. In the last decade two new methods have been used extensively: acoustic rhinometry and rhinostereometry. Many studies with rhinostereometry have shown interesting results. However, there have been doubts about this method, since only a few investigators have used it, and it has never been compared with other methods. On the other hand, the acoustic rhinometer has been compared with many other methods. In this study, we compare the results of measurements with acousti

The authors describe a cardiac acoustic mapping system designed to acquire, analyse, and display the amplitude distribution of a phonocardiogram (PCG) recorded from 22 sites on the thorax. A new PCG envelope detection approach, implemented by analogue circuits, enables simultaneous sampling of PCG envelopes from the 22 sites at a rate of 250 Hz/channel. A calibration procedure removes channel-to-channel gain variations, and a coherent averaging of each PCG envelope over several cardiac cycles improves the signal-to-noise ratio. Time congruent samples from the 22 averaged PCG envelopes are then

Clinical pathways are tools for cost containment and quality improvement in the managed care environment. This report describes our experience with developing and implementing a clinical pathway for acoustic neuroma surgery. Intensive care unit bed days and inpatient hospitalization days for cerebellopontine angle surgery significantly decreased during the months that the pathway was developed, and the incidence of complications was unchanged after adoption of the algorithm. We conclude that with joint neurootology and neurosurgery leadership clinical pathways can be a safe, cost-effective, an

The growth rate of acoustic neuromas is very variable: some tumours grow rapidly, some do not grow and some even get smaller. When making treatment decisions, it may be important to have an idea of the growth rate of the individual tumour, and this is only possible when there are comparable examinations. We performed both CT and MRI on 15 patients. Two radiologists estimated the size of their acoustic neuromas. There was a significant difference between the two examiners' calculations of tumour volumes on CT and between the first examiner's CT and MRI volume calculations. No difference was fou

The purpose of this investigation was to study the effects of translabyrinthine acoustic neuroma surgery on tinnitus in a consecutive sample of patients operated on between 1988 and 1994 in Uppsala (Sweden). A postal questionnaire was returned by 141 patients, yielding a 90% response rate without reminder. The results showed that tinnitus was experienced by 70% of the patients before surgery and 60% after surgery. In general, low degrees of tinnitus distress were found, which was confirmed by the questionnaire results. Ratings of tinnitus distress after surgery, using the Klockhoff and Lindblo

Ideally, clinicians recommend diagnostic tests when the patient's risk of disease is sufficient to justify putting numerous similar patients through the morbidity required to diagnose disease in one patient. In the case of acoustic tumor diagnosis, there are few published data available to the clinician to help assess risk in an individual patient. The purpose of this study was to obtain information by an opinion poll of a group of experts. We used the Delphi method to poll clinicians trained at the House Ear Clinic. We asked these experts 20 questions related to acoustic tumor diagnosis. Some

To avoid misdiagnosis and mistherapy of acoustic neuroma, 104 cases of acoustic neuroma treated between 1986 and 1995 were retrospectively reviewed. Among them, 23 patients (24 ears, 23%) presented with sudden hearing loss as the starting symptoms. The clinical and audiological analysis demonstrated that 54.2% of the ears showed hearing loss over 71 dB HL, and all ears had deteriorated ABRs. -SP/AP was measured in nine ears, seven showed -SP/AP greater than 0.4. Acoustic reflex was negative in all 11 ears tested, and 94.4% of the ears showed abnormal ENG. The rate of positive CT scan was 88.8%

OBJECTIVE: Ongoing controversy regarding the optimal treatment of acoustic neuromas in the elderly population has prompted us to examine the our experience in order to arrive at a treatment algorithm.
STUDY DESIGN: Retrospective case review.
SETTING: Tertiary referral center.
PATIENTS: The records of 48 elderly patients ranging from age 70 to 90 years with acoustic tumors were reviewed.
INTERVENTIONS: In 34 cases, tumor size was followed through serial imaging for a mean 28.5 months (range 5-108 months). Eight of these patients subsequently required surgery for significant tumor growth. An

The purpose of this study was to explore the acoustic discrimination and graded severity of three clinical voice types. Listeners classified 102 samples of dysphonic vowels /a/ and /i/ on the basis of voice types: breathy, hoarse, and strained. The vowels were analyzed acoustically with two measures of perturbation and 2 measures of spectral noise. Discriminant analysis showed that apriori, acoustic classifications of voice type were made with 92% accuracy using four acoustic parameters: (a) cepstral peak prominence (CPP), (b) jitter standard deviation (SD-J), (c) fundamental frequency (F0), a

In 1952, Wildervanck described the first case of what he called the Cervico-Oculo-Acoustic (COA) syndrome, which comprises Klippel-Fetl anomalad (congenitally fused cervical vertebrae), congenital deafness and Duane's retraction syndrome (abducen's paralysis and retraction on adduction). Since the first case was reported, there have been more reports describing this triad, either completely or incompletely. A further case of this syndrome with CSF leakage is reported here and exploratory tympanotomy was performed. Abnormalities of both inner and middle ear were found. In addition, MRI and CT s

The purpose of this study was to establish the frequency and pattern of depressive disorders after surgery for acoustic neuroma, and to look for associations. Twenty seven patients with acoustic neuroma underwent thorough psychiatric assessment before surgery and at three and 12 months after surgery. Three patients had a depressive disorder in the preoperative assessment. Of the remaining 24 patients, nine (38%) had a depressive disorder at the three month check up. Deterioration of hearing was the only postoperative detriment associated with a depressive disorder (P = 0·024). All nine patient

Two experiments are described concerning voice characteristics of male-to-female transsexuals. In the speech production experiment isolated words and longer utterances were realized in a male and female mode by subjects. Speech samples were analyzed as to a number of acoustic parameters and compared intraindividually. It appeared that, in spite of anatomical constraints, subjects were able to realize a number of vocal characteristics that are known to add to a feminine voice quality in the female speaking mode. In the second experiment, consisting of a perceptual task, male and female versions

900 acoustic neurinomas were removed by the suboccipital approach at Nordstadt Neurosurgical Department from 1978 to 1992 by the same surgeon (M. S.). While 247 patients were deaf on the involved side before surgery, there were 653 patients ears with some preoperative hearing. Preservation of the cochlear nerve was always attempted, and the overall-rate of hearing preservation was 38% (249 of 653), regardless of pre- and postoperative quality of hearing or of tumour sizes. In small tumour sizes below 3 cm of diameter preservation rate was 51%, in large tumours above 3 cm of diameter it was 22%

We have been able to culture acoustic neuroma (AN) tissues by taking small tumor pieces during surgery from the cisternal or, preferably, metal part of the tumor where macroscopic tissue degeneration is not prominent. The AN pieces were transported in culture tubes to our cell laboratory where they were sliced and transferred to culture medium. The AN tissue demands a crude culture suspension containing laminin. We have been successful in culturing 15 successive ANs. The cells have different morphological features with characteristics of type Anthony A and B. Four main cell types were recogniz

The magnetic resonance imaging (MRI) findings of 27 histologically proven acoustic schwannomas in 24 patients (13 men, 11 women, age 20-79 yr) are described in detail. Three patients had bilateral tumors. Twenty-two tumors (82%) had intra- and extracanalicular components, three tumors (11%) were limited to the internal auditory canal (IAC) and two tumors (7%) were limited to the cerebellopontine angle (CPA). The diameters of extracanalicular lesions in the CPA ranged from 12 to 50 mm, and most of them were round in shape. All IAC portions of CPA tumors had a funnel-shaped appearance on the axi

Bilateral acoustic neurinomas are found in almost all patients with neurofibromatosis 2 (NF2). With better knowledge of the disease and development of diagnostic procedures, an increasing number of patients have been detected at an early stage of the disease. Preservation of hearing in these cases has become more feasible. Between 1979 and 1992, 110 acoustic neurinomas in 74 NF2 patients have been resected in this department. All tumours but 11 were completely removed. In 11 cases deliberate subtotal tumour removal was performed: bilaterally in one deaf patient for brainstem decompression, and
