Macroscopic Fat in Adrenocortical Carcinoma: A Systematic Review.

OBJECTIVE. The objective of this systematic review was to determine the number and quality of reports of adrenocortical carcinoma (ACC) containing macroscopic fat; this information may inform guidelines for diagnosis and management of ACC. MATERIALS AND METHODS. A comprehensive search of databases of published studies was performed. Two reviewers independently selected original research, case series, or case reports of ACC with macroscopic fat on imaging and extracted data. Risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. RESULTS. Three c

What is the appropriate role of minimally invasive vs. open surgery for small adrenocortical cancers?

PURPOSE OF REVIEW: The role of endoscopic adrenalectomy for adrenocortical carcinoma is the most controversial and debated points in adrenal surgery. We reviewed the most recent literature on this topic. RECENT FINDINGS: From the amount of available data (even if not conclusive), the following could be extrapolated: first, for patients with apparently localized disease the adrenal gland should be removed en bloc with the entire retroperitoneal fat pad, which also includes some periadrenal lymph nodes, but no extended resection is necessary in absence of involvement of adjacent structures; sec

Histopathological and Molecular Characteristics of Pediatric Adrenocortical Tumors: where Do we Stand?

Pediatric adrenal cortical carcinoma (ACC) is a rare heterogeneous cancer type that is incompletely understood and differs from its adult counterpart in clinical presentation, histopathological characteristics, genomic landscape, and prognosis. Pediatric ACC has a bimodal age distribution for children with ACC, with the highest incidence in young children and adolescents. Tumors commonly present with an endocrine syndrome such as virilization or Cushing’s syndrome, associated with hormone overproduction. Surgical resection is the primary treatment. Adjuvant therapies such as mitotane or chemot

Diagnostic work-up in paediatric adrenocortical tumours: an international consensus.

OBJECTIVE: Paediatric adrenocortical tumours (pACTs) are rare but potentially life-threatening neoplasms, most of which are hormonally active and frequently present with androgen excess. Although a comprehensive evaluation is recommended, no consensus exists on the optimal diagnostic work-up. The goal of this document is to provide evidence-based recommendations to standardize the diagnostic evaluation of children with suspected pACTs. METHODS: We applied a modified three-step Delphi process with an international panel of pACT experts to develop consensus on the initial diagnostic work-up. Fo

Case Report: Androgen-secreting pediatric adrenocortical tumor of uncertain malignant potential presenting with virilization in a 19-month-old girl.

BACKGROUND: Virilization in girls younger than 2 years should prompt careful evaluation for pathological androgen excess and should be interpreted as a practical clinical reminder rather than as a stand-alone diagnostic tool. Pediatric adrenocortical tumors (ACTs) are rare but are usually hormonally active, and their interpretation requires pediatric-specific clinicopathological criteria rather than direct application of adult adrenal carcinoma criteria. CASE PRESENTATION: A 19-month-old girl presented with a 6-month history of progressive voice deepening, pubic hair growth, and clitoromegaly

Management of cushing's syndrome in patients with adrenocortical cancer: state of the art and future perspectives.

Adrenocortical cancers (ACCs) are rare tumours, with up to 50% of cases associated with hypercortisolism. Cortisol-secreting ACCs are characterized by a worse prognosis, and in these patients, the normalization of hypercortisolism is mandatory and requires an urgent approach to avoid complications related to glucocorticoid excess. Clinical and biochemical parameters, including hormonal values, can be used to define cortisol normalization. However, in patients on concomitant mitotane treatment, serum cortisol and ACTH levels may be falsely altered and thus unreliable for defining cortisol norma

Efficacy of metyrapone for symptoms of adrenocortical carcinoma.

Elevated cortisol by adrenocortical carcinoma leads to a variety of symptoms. We report on the efficacy of metyrapone in treatment of a variety of distressing symptoms caused by elevated cortisol in a patient who refused advanced treatment for adrenocortical carcinoma.

[Diagnosis of adrenocortical carcinoma].

Adrenocortical carcinoma is a rare disease with a poor prognosis. The prognosis of affected patients is determined by the early detection of the disease at its locoregional stage when surgical resection of the tumour can significantly increase the chances of cure. Determining the dignity of adrenal tumours is often a challenge during preoperative investigation. Diagnosis is aided by conventional radiological and laboratory tests. New biomarkers with considerable diagnostic accuracy have recently been identified and are expected to be incorporated into daily practice in the near future. Among t

Efficacy of Platelet-Rich Plasma Therapy in Oral Lichen Planus: A Systematic Review.

Background and Objectives: Oral lichen planus (OLP) is an autoimmune, mucocutaneous, oral potentially malignant disorder (OPMD), which characteristically manifests with chronic, recalcitrant lesions, with frequent flare-ups and remissions. The precise etiopathogenesis of OLP is still debatable, although it is believed to be a T-cell-mediated disorder of an unidentified antigen. Despite the availability of various treatments, no cure for OLP exists due to its recalcitrant nature and idiopathic etiology. Platelet-rich plasma (PRP) has antioxidant, anti-inflammatory, and immunomodulatory properti

Prenatal presentation of an adrenocortical tumor.

BACKGROUND: Prenatally identified suprarenal masses are most often found to be adrenal hemorrhage. The most common tumor in this situation is neuroblastoma. CASE PRESENTATION: We report the case of a rare adrenocortical tumor found prenatally on ultrasound. While most patients with adrenocortical tumors present with virilizing symptoms, our patient did not have evidence of virilization and was presumed to have neuroblastoma. CONCLUSION: Following a period of observation, our patient underwent surgical resection due to tumor growth revealing the unexpected diagnosis.
