Posterior cortical atrophy (PCA) is characterized by bilateral parieto-occipito-temporal atrophy and hypometabolism. Neuropsychological impairments include complex visual disturbances, alexia, agraphia, finger agnosia, right-left disorientation and dyscalculia. A recent case study reported severe numerical deficits with some selectively preserved numerical skills in a patient affected by PCA [Delazer, M., Karner, E., Zamarian, L., Donnemiller, E., & Benke, T. (2006). Number processing in posterior cortical atrophy--a neuropsycholgical case study. Neuropsychologia]. In a functional magnetic res…

One year after a left posterior and thalamic stroke, a 52-year-old male participant was treated with 14 weeks of theta reduction neurofeedback training. Imaging studies revealed left temporal, parietal, occipital, and bilateral thalamic infarctions along the distribution of the posterior cerebral artery. Neuropsychological testing demonstrated severe verbal memory, naming, visual tracking, and fine motor deficits. Additionally, alexia without agraphia was present. A pretraining quantitative electroencephalograph (QEEG) found alpha attenuation, lack of alpha reactivity to eye opening, and exces…

Gerstmann syndrome (GS) is a rare neurological disorder characterized by the presence of agraphia, acalculia, right-left disorientation, and finger agnosia. In older adults, the isolated presentation of this syndrome may represent an early sign of an ischemic cerebrovascular event, particularly in patients who have cardioembolic risk factors such as atrial fibrillation (AF). We report the case of a 72-year-old male with a history of hypertension, type 2 diabetes, AF without anticoagulant treatment, and chronic alcohol use who presented with a five-day history of behavioral changes and difficul…

Immunotherapy with chimeric antigen receptor T (CAR-T) cell therapies has brought substantial improvement in clinical outcomes in patients w … t in CAR-T therapy recipients including impaired attention, difficulty in item naming, and writing, agraphia, and executive dysfunction. In this review, we aim to present the diagnostic methods and tests that have been used for the recognition of cognitive impairment in these patients. Moreover, up-to-date data about the duration of cognitive impairment sy …

A 70-year-old man with known cardiovascular risk factors, presented with acute onset expression aphasia, agraphia, dyscalculia, right-left disorientation and finger agnosia, without fever or meningeal signs. Stroke was thought to be the cause, but cerebrovascular disease investigation was negative. Interviewing the family revealed he had undergone yellow fever vaccination 18 days before. Lumbar puncture revealed mild protein elevation. Cultural examinations, Coxiella burnetti, and neurotropic virus serologies were negative. Regarding the yellow fever virus, IgG was identified in serum and cere…

We describe a right-handed patient (M.B.), who developed left hemianopsia and a severe reading impairment after right occipital-parietal hemorrhage. The pattern of his reading deficit was very similar to that of pure alexia (alexia-without-agraphia): extremely slow reading times with frequent grapheme substitutions and omissions. A test of letter reading while controlling for saccadic eye movements and hemifield of presentation ruled out hemianoptic alexia. Although there have already been reports of reading impairments in right handers following right- hemispheric lesions, ours is, to the bes…

Comments on the original article by Marcus, Fulton, and Edens (see record 2011-23134-001). The term "syndrome" derives from the Greek, meani … pathology. For example, Gerstmann's syndrome in neurology is marked by four indicators: dysgraphia/ agraphia, dyscalculia/acalculia, left-right disorientation, and finger agnosia (Benton, 1992). Across the general population, these four indicators are essentially uncorrelated. Nevertheless, when observed together, this constellation of four signs generally …

A 68-year-old man was referred to our hospital with tension-type headaches and a 1-year history of dementia. On neurologic examination, he had ideomotor apraxia and incomplete Gerstmann syndrome that was characterized by acalculia, agraphia, and finger agnosia. On imaging, multiple cystic lesions reported as "unusually dilated perivascular spaces" were observed along the medullary arteries in the left hemisphere; some of them had adjacent hyperintense areas in fluid attenuated inversion recovery images. We assumed that the multiple cystic lesions caused his higher cerebral dysfunction, because…

BACKGROUND: It remains unclear from lesion studies whether the four signs of the Gerstmann syndrome (finger agnosia, acalculia, agraphia, and right-left confusion) cluster because the neuronal nets that mediate these activities have anatomical proximity, or because these four functions share a common network. If there is a common network, with degeneration, as may occur in Alzheimer's disease, each of the signs associated with Gerstmann's syndrome should correlate with the other three signs more closely than they correlate with other cognitive deficits.
METHODS: Thirty eight patients with pro…

We report on a left-handed patient with an ischemic infarction affecting exclusively the total length of the corpus callosum. This lesion clinically correlated with an almost complete callosal disconnection syndrome as described in callosotomy subjects, including unilateral verbal anosmia, hemialexia, unilateral ideomotor apraxia, unilateral agraphia, unilateral tactile anomia, unilateral constructional apraxia, lack of somesthetic transfer and dissociative phenomena. Despite the patient's left-handedness, his pattern of deficits was similar to the disconnection syndrome found in right-handers…

A 49-year old right-handed taxi-driver experienced right upper limb weakness and global aphasia following internal carotid artery occlusion. Five months later, aphasia and hemiparesis had resolved but he complained of difficulties in his daily activities, termed "inner conflict". Specific testings disclosed typical features of a callosal syndrome: left unilateral ideomotor apraxia, left hand agraphia, left tactile dysnomia, right hand constructional difficulties, and left ear extinction on dichotic listening. Naming and recognition of tachistoscopically presented images or words was similar to…

We report the clinical findings in a case of alexia without agraphia, due to a left posterior cerebral artery infarction, in which C.T. showed occipital and splenial lesions. There were in addition a right homonymous hemianopsia and agnosia for colors, objects and pictures, with both associative and disconnective aspects. The hypothesis of an occipito-occipital disconnection, depriving the left visual associative areas of inputs, could explain this agnosia. The most striking findings in this case was, besides an anomia for faces, a peculiar difficulty to recognize her own face. The patient see…

We report a 77-year-old male who presented with acute cerebral infarction in the left posterior central gyrus and left superior parietal lobule, and left supramarginal gyrus and its subcortical white matter, resulting in bilateral agraphaesthesia, bilateral astereognosis, and bilateral limb-kinetic apraxia. It is generally known that limb-kinetic apraxia and sensory dysfunction appear on the contralateral side of the lesion, but in this case, limb-kinetic apraxia and some sensory dysfunction appeared on the ipsilateral upper limb as well as the lesion. This suggests that, similar to Liepmann's

BACKGROUND: Mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) with aphasia is a rare disorder, with the associated aphasia reported as either Wernicke's or Broca's. Herein, we report a patient with MELAS complicated by thalamic aphasia.
CASE: A 15-year-old right-handed girl presented with headache, nausea, right homonymous hemianopsia, and aphasia. She could repeat words said by others, but had word-finding difficulty, paraphasia, and dysgraphia. Brain MRI revealed abnormal signals from the left occipital lobe to the temporal lobe and left thalamus, but Wernicke'

IMPORTANCE: Students with dysgraphia often receive test accommodations to enhance their handwriting legibility or speed. However, these accommodations are seldom individualized to ensure that they actually improve the performance of a specific student. Therefore, a need exists for an evidence-based protocol (henceforth, the Protocol) for selecting individually tailored test accommodations for students with dysgraphia.
OBJECTIVE: To establish the Protocol's content validity, interrater reliability, and preliminary outcomes through a pilot study.
DESIGN: Descriptive and exploratory design.

Taking into account findings in the literature, the author aimed to test whether specific graphical characteristics of handwriting can distinguish patients diagnosed with schizophrenic disorders from healthy controls. Handwriting samples (one sample from each person) from 60 outpatients (29 women, 31 men; age M = 28.5, SD = 5.4) with paranoid schizophrenia were analyzed by three documents examiners and were compared to samples from 60 controls (30 men, 30 women, age M = 28.0, SD = 3.0) without psychiatric disorders. Document examiners assessed 32 graphical features potentially related to schiz

We report on a patient with left hemispheric thromboembolic stroke whose writing performance on single word dictation following recovery from an aphasic syndrome remained severely impaired but fluent. Having only very fragmentary command of the target's written spelling she produced neologistic nonwords which were approximately the same length and contained, in addition to perseverative intrusions and unidentifiable errors multiple insertions, deletions, transpositions, and substitutions but only few unpronounceable combinations. The patient was tested 4 and 10 months postonset, before and aft

Following stroke or other causes of brain damage, patients may demonstrate visual disorders of higher cortical function. These defects involve visual attention, oculomotor skills, visuospatial orientation and object recognition. It is important for optometrists to understand these conditions and consider their presence in individuals with persistent visual complaints despite a normal exam, or in patients who fail to respond to magnification in low vision rehabilitation. The diagnosis and management of these higher order visual disturbances are discussed.
