[Diagnostic importance of analyzing the expired air in alcoholism].

The new highly effective method of alcoholism diagnosis was proposed. It included gas-chromatography analysis of breathed out air. The alcoholism was diagnosed if ethane and paraldehyde in this air to be less than 0.47 nM 0.20 nM respectively, pentane level was more that 0.47 nM while ethanol content was found in the range of 0.73-1.96 nM. If ethane concentration proved to be higher that 0.47 nM alcoholism diagnosis was established when ethylene level was found between 0.01-0.19 nM. The assay's diagnostic importance was confirmed by observation of 44 alcoholic patients as well as 26 healthy ma…

Morphological changes in the corpus callosum in chronic alcoholism.

In a group of 66 patients (age 24 to 78 years) with the clinical diagnosis of chronic alcoholism the changes in the corpus callosum were evaluated. The period of alcohol abuse varied from 3 to over 30 years. In 57 cases atrophy of the corpus callosum was noted. The trunk was involved most frequently. Myelin sheaths exhibited abnormalities from slight pallor to total destruction. In 19 cases the damage of myelin sheaths was restricted to disseminated, perivascular, spongy degenerations. The vessels were sclerotic, especially periventricular ones exhibited degenerative changes. Perivascular glio…

Relationship between cardiomyopathy and liver disease in chronic alcoholism.

Based on anecdotal impressions, there is a common clinical perception that alcoholics with liver disease do not develop cardiomyopathy and that those with alcohol-induced cardiac disease are spared cirrhosis. To determine the relationship between alcoholic cardiomyopathy and cirrhosis, we carried out a prospective cross-sectional study that included: (1) 30 alcoholic men with cardiomyopathy; (2) 30 alcoholic men without cardiomyopathy (left ventricular ejection fraction > 55%); (3) 20 actively drinking alcoholics with cirrhosis; (4) 15 abstaining alcoholics with cirrhosis; and (5) 15 nonalcoho

[A case of alcoholism presenting pellagra and hypokalemic myopathy].

A 32-year-old man with chronic alcoholism over 10 years developed skin eruptions, dark-red tongue and severe watery diarrhea, followed by weakness of bilateral lower extremities. Physical examination revealed hyperpigmented skin eruptions with scales on the dorsa of his hands and extensor aspects of his forearms. Neurological examination showed proximal muscle weakness of both lower extremities, hyperactive knee and ankle jerks, positive Chaddock reflexes and stocking type sensory disturbances. Laboratory data revealed elevation of myolytic enzyme, hypokalemia and decrease of niacin level in t…

Interhemispheric electroencephalographic coherence as a biological marker in alcoholism.

Electroencephalographic coherence scores in 21 teetotaler first-degree relatives of alcoholics, 27 subjects with alcohol dependence and 21 healthy subjects without a family history of alcohol abuse were compared. The relatives had significantly higher coherence scores in the frontal and parietal leads than the alcoholics and in the frontal and centroparietal leads than in the healthy subjects. This might represent a trait marker of resilience in subjects at high risk for the development of alcoholism.

[Main involvement of the central nervous system in alcoholism].

The effects of alcohol on the central nervous system can be subdivided into three main categories: the effects of acute intoxication (drunkenness, acute encephalopathy, stroke), the effects of tolerance and ethanol withdrawal (delirium tremens, seizures) and the delayed manifestations of chronic alcohol consumption (cerebellar degeneration, Wernicke's encephalopathy, dementia).

Social support and depression in men during alcoholism treatment.

Psychiatric comorbidities have been shown to be important predictors of the outcome of alcoholism treatment. This study examines whether perceived lack of social support can be identified as an independent predictor of symptoms of depression experienced during alcoholism treatment over and above the effects of personality characteristics and the severity of alcohol and psychiatric history. We studied 189 alcoholic men in treatment at a rural midwestern Department of Veterans Affairs medical center. Multiple regression analyses found that reduced social support significantly predicted depressio…

[Cardiac arrhythmias in patients with chronic alcoholism during abstinence].

A study was undertaken to examine 25 chronic alcoholics in abstinence. A 24-hour monitoring was performed during and extra this state. Chest x-ray study, bicycle ergometry, ultrasonic study of the heart, transesophageal pacing and blood thyroid hormone levels were applied to exclude other causes of cardiac arrhythmias. In alcoholic abstinence, various cardiac arrhythmias appeared as ventricular premature beats (including those of high grade), supraventricular premature beats, supraventricular tachycardias and paroxysmal atrial fibrillation were recorded in all 25 patients. When the abstinence

Involvement of tobacco in alcoholism and illicit drug use.

Survey data from the United States indicate that tobacco use is associated with the initiation of use of other addicting substances, and that increasing levels of tobacco use are associated with increasing levels of use of other psychoactive substances. Furthermore, factors affecting initiation, abstinence, and relapse to the use of tobacco, alcohol, and opioids are similar in nature. In addition, there are similarities in the addictive process underlying the use of these substances. Taken together, these data suggest that tobacco use is involved, possibly more than by simple association, in t

The influence of alcoholism on the course of depression.

The clinical course of 289 patients with primary non-bipolar major depression without concurrent alcoholism was compared with that of 79 patients with non-bipolar major depression with concurrent alcoholism. Neither patient group suffered from dysthymia or current drug abuse. Contrary to expectations, the two groups did not differ on time to recovery from the major depression, time to relapse into a subsequent major depression, or various cross-sectional clinical ratings at 2 years. The two groups did differ on psychosocial status. Although they were equally impaired at index, the alcoholism g…

Treatment of chronic organic mental disorders associated with alcoholism.

Considering the magnitude of the problem, the treatment of alcoholism-associated chronic organic mental disorders has not been extensively studied. Alcoholic organic brain disease is heuristically viewed as the admixture of clinical syndromes of impairment [alcoholic amnestic disorder or Korsakoff's psychosis (KP) and dementia associated with alcoholism (DAA)], each with its distinguishing cognitive, neuropathological, and neurochemical characteristics. Differences between KP and DAA are highlighted by studies that compare KP patients with those having Alzheimer's disease or depression. Furthe…

[Histochemical study of the skeletal muscle in chronic alcoholism].

Twenty-two chronic alcoholic patients were assessed by neurologic examination and muscle biopsy. The patients manifested proximal muscular weakness to a variable extent. One case presented as an acute bout of myopathy, according to the Manual Muscle Test, MMT. The most prominent histologic feature observed was muscle atrophy (95.3%) better evidenced through the ATPase stain with the predominance of type II A fibers (71.4%). Lack of the mosaic pattern (type grouping) seen in 76% of the cases and an important mitochondrial proliferation with intrasarcoplasmatic lipid accumulation in 63% of the p

[Symptoms of alcoholism from the viewpoint of the dermatologist].

There is a wide range of dermatological symptoms that are indicative of alcoholism. In the literature little attention is paid to these manifestations. In the present study 201 alcoholics were examined before beginning treatment and were compared with 42 teetotallers. The symptoms observed and their causes are discussed.

Effect size estimates in chemical aversion treatments of alcoholism.

This study found that aggregate studies on alcohol aversion therapy tended to support a moderate level of treatment impact that may have noteworthy practical import. Emetics appear to generate fairly consistent findings; a paralysis-inducing chemical may produce variable results.

Early alcoholism treatment: the Emmanuel Movement and Richard Peabody.

The history of alcoholism treatment in the early twentieth century is outlined. The methods of the Emmanuel Movement and of Richard Peabody are described, biographical details of their main practitioners are given, the populations treated are described, and the predecessors and successors of the two methods are discussed. In addition, the two methods are compared with each other and with the methods of Alcoholics Anonymous and Freudian psychoanalysis. The founder of the E. Movement was a clergyman, Dr. Elwood Worcester, whose method was designed to treat a variety of neurotic disorders. He fel…
