Ticks (Ixodidae) on humans in South America.

Twenty eight species of Ixodidae have been found on man in South America (21 Amblyomma, 1 Boophilus, 2 Dermacentor, 2 Haemaphysalis, 1 Ixodes and 1 Rhipicephalus species). Most of them are rarely found on man. However, three species frequently parasitize humans in restricted areas of Argentina (A. neumanni reported from 46 localities), Uruguay (A. triste from 21 sites) and Argentina-Brazil (A. parvum from 27 localities). The most widespread ticks are A. cajennense (134 localities in Argentina, Bolivia, Brazil, Colombia, Ecuador, French Guiana, Guyana, Paraguay, Suriname and Venezuela), A. oval

Violence against women in North America.

Although North America is viewed as a place where women have equal rights and status, violence against women is still rampant. Forty to 51% of women experience some type of violence in their lifetime including child abuse, physical violence, rape and domestic violence. The perpetrator is most likely to be a current or former partner. Such violence stems from historical views of women as property and may flourish because of the public's reluctance to get involved in family matters. The concept of violence has been expanded to include non-traditional types such as sexual harassment, breeches of

Injection drug use in North America.

Injection drug use of psychotropic agents for nontherapeutic purposes is associated with some of the most pernicious infectious diseases seen in the United States. There is an inextricable link between infection, injection drug use (IDU) and other risk behaviors, especially those related to sexual activity. A number of national surveys now provide excellent databases to track the breadth, scope and impact of IDU across the United States. The prevalence of heroin use has increased over the past decade with larger numbers of users presenting for treatment of drug use disorders. A host of serious

The epidemiology of primary hyperparathyroidism in North America.

The impact of primary hyperparathyroidism (HPT) on the population has not been well documented, particularly with respect to asymptomatic HPT. Only 83 deaths were attributed to hyperparathyroidism in the United States in 1999, for a reported death rate of just 0.3 per million per year. Many more patients are affected, of course, and the national hospitalization rate for HPT was 8.0 per 100,000 in 1999, counting 22,000 "all-listed" diagnoses, and 1.8 per 100,000 counting only the 5000 admissions where HPT was the first-listed discharge diagnosis. Surgery was performed on approximately 12,000 ho

Hepatitis B epidemiology in Latin America.

The available data on prevalence of hepatitis B virus (HBV) infection in Latin America are incomplete and largely based on analysis of blood banks, which are not stratified by age or social class. An epidemiological study was recently undertaken in six countries in Latin America to update the data. The highest seroprevalence of antibody to the HBV core antigen (anti-HBc) was found in the Dominican Republic (21.4%), followed by Brazil (7.9%), Venezuela (3.2%) and Argentina (2.1%). The lowest seroprevalence was found in Mexico (1.4%) and Chile (0.6%). The seroprevalence in different regions of B

Rotavirus vaccines and vaccination in Latin America.

Worldwide, rotaviruses account for more than 125 million cases of infantile gastroenteritis and nearly 1 million deaths per year, mainly in developing countries. Rather than other control measures, vaccination is most likely to have a major impact on rotavirus disease incidence. The peak incidence of rotavirus diarrhea occurs between 6 and 24 months of age. In developing countries, however, cases are not uncommon among children younger than 6 months. G serotypes 1 to 4 are responsible for most disease, but there are indications that in Brazil that G type 5 is of emerging epidemiological import

Headache and cysticercosis in Ecuador, South America.

Intractable headaches have been described as the presenting complaint of many patients with T. solium neurocysticercosis. We conducted a house-to-house neuroepidemiological survey of 2,723 residents of an Andean community, known to be endemic for this infection. Migraine headaches were confirmed in 187 cases (68.7 per thousand), and tension headaches were diagnosed in 77 cases (28.3 per thousand). Fifty-seven migraine sufferers accepted computed tomography examination, and in 19 it revealed neurocysticercosis. In 11 out of 52 migraineurs who had their blood drawn, electron immunotransfer blot

Health advice for travelers to Latin America.

Practical and precise information about the potential infectious health hazards that travelers to Latin America may encounter is reviewed in this article. Some diseases are briefly described, others are only mentioned. The countries have been grouped into four geographical areas following the classification of the World Health Organization. The discussion on each area includes information on travelers' diarrhea, malaria, cholera, typhoid fever, yellow fever, and other diseases or special problems.

A case of cerebral sparganosis in South America.

Cerebral sparganosis (CS) is a parasitic infection caused by the larva of Spirometra mansonoides. Rarely it can affect the human brain. We report the case of a 24-year old man from Paraguay who suffered from seizures and headache for one year. A frontal tumor was diagnosed by CT-scan and was subsequently resected. The pathological examination revealed a larva with Sparganum characteristics. The evolution of the patient was satisfactory. As far as we know, this is the first case of CS reported in South-America.

Anorexia nervosa in 19th century America.

Compared with late 19th century publications in Great Britain and France, medical writers in the United States of the same era apparently had less interest in anorexia nervosa as a distinct clinical syndrome. A review of the literature of that period shows that American physicians very rarely referred to the "new" syndrome described in 1873 by Gull and Lasègue. Except for some short or oblique references, the first explicit clinical description of a case of anorexia nervosa by an American author (James Hendrie Lloyd) did not appear until 1893. The controversy about "fasting girls" and the all-

[3 problem vitamins in Latin America].

The list of vitamins recognized as essential in human nutrition is extensive. Only some of them, however, are attributed an important role in public health. The present paper deals with three of these selected because their deficiencies still prevail in important sectors of population in the Latin American Region: vitamin A, vitamin C and vitamin D. For each vitamin the paper discusses the scientific bases for their requirements, as well as pragmatic considerations to be taken into account for the derivation of recommended dietary intakes. Reference is made to the logic of applying the concept

The epidemiology of conjoined twins in Latin America.

Twenty-three cases of symmetrical conjoined twins were registered by the Latin-American Collaborative Study of Congenital Malformations (ECLAMC) in 1,714,952 births, which were observed during the 1967-1986 period in 95 maternity hospitals distributed in eleven Latin-American countries. This results in a birth prevalence rate of about 1/75,000 births. The secular and geographic distribution of this material do not depart from random in spite of one hospital with three cases, and two hospitals with two cases each, within a short time period. These 23 cases include one diprosopus, 3 dicephalus,

[Nutritional pathology in Latin America and the Caribbean].

The Latin American and Caribbean Region has different general characteristics as compared to other regions of the world. These peculiarities have determined certain food and nutritional problems which require specific programs and policies. Even though the universal coverage of basic needs is desirable, this is not the situation in the Region, and it is possible to identify integral vulnerable groups in every population. The study of problems related to food and nutrition requires a global and multidisciplinary approach considering ecological, economical, social and cultural factors affecting

Epidemiology of ambiguous genitalia in South America.

We present descriptive epidemiologic data on ambiguous genitalia (AG), obtained in the period 1967-1982 by the Latin American Collaborative Study of Congenital Malformations (ECLAMC) through 70 participating hospitals located in 34 cities of 9 South American countries. The observed prevalence rate for isolated AG was about 1/20,000 births, with at least 1/4 and probably as much as 1/2 due to congenital adrenal hyperplasia. When AG cases associated with other congenital anomalies were also considered, the overall frequency of AG was 1/6,900 total births (1/10,000 for live and 1/350 for stillbir

Prevalence rates of microtia in South America.

A high frequency of microtia in Quito (Ecuador) was detected by the Latin American Collaborative Study of Congenital Malformations (ECLAMC), and the data available on file were used to define the cluster and to test some potential risk factors. The Quito material consisted of 46 041 livebirths from two hospitals. The rest of the sample consisted of 553 068 livebirths from 58 hospitals located in 24 cities of six other South American countries. The observed prevalence rate for microtia was over five times higher in Quito (17.4/10 000) than in the other cities (3.2/10 000). The microtias from Qu
