For the past 20 years, internal anal sphincterotomy has generally been considered to be the standard operation for an anal fissure. We sought an alternative form of treatment because of the wound complications inherent in this operation. Anal dilatation, precisely performed with a Parks' retractor opened to 4.8 cm or with a 40-mm rectosigmoid balloon, has been found to cure successfully the fissure in 93 percent and 94 percent, respectively, of each group and to be associated with fewer complications.

Rectal induction of anaesthesia is a useful method but is not widely used in South Africa. We studied the onset of action and side-effects of 1% methohexitone administered rectally in a dose of 20 mg/kg to 110 preschool children. Ninety-one per cent were adequately sedated for inhalation induction by mask within 10 minutes, and all by 15 minutes, of drug administration. A 15-year review of children's hospital patients with cystic fibrosis (CF) who underwent surgery yielded 578 cases in 210 patients (mean 2.7 per patient). The median age was 16 years (range newborn to 43 years). Four hundred procedures were done under general anesthesia and 176 under local. The current communication presents a simple technique for treatment of complete rectal prolapse (CRP). Many techniques have been described for repair of complete rectal prolapse in adults. The results of abdominal approaches are superior to those of perineal approaches, but they carry the risks of major abdominal surgery.
