Combined spinal and epidural anesthesia for cesarean section in a parturient with moyamoya disease.

We present the case of a parturient with moyamoya disease admitted to the hospital for elective cesarean section. Combined spinal and epidural technique was chosen because it allows better analgesia than epidural anesthesia and more hemodynamic stability than either general or spinal anesthesia. Ropivacaine was the local anesthetic of choice for the epidural portion because of the wide sensory-motor dissociation, thus preserving adequate respiration in the case of a high block.

[Cesarean section under spinal anesthesia for a patient with chronic renal failure].

A 37-year-old primipara was scheduled for cesarean section under spinal anesthesia in the 38th gestational week because of cephalo-pelvic disproportion. She had suffered from chronic renal failure since 27 years of age, and had been undergoing hemodialysis three times per week. The frequency of hemodialysis was increased to 4-6 times per week during pregnancy to maintain blood urea nitrogen and serum creatinine concentration below 60 mg.dl-1 and 6.0 mg.dl-1, respectively. Since she had no bleeding tendency, spinal anesthesia with 8 mg of tetracaine was successfully performed after hemodialysis

[Comparative study of general and spinal anesthesia in elderly women in hip surgery].

The choice between regional versus general anaesthesia for elderly patients undergoing hip surgery is debated. It is vitally important to see if the type of anaesthetic administered affects per- and postoperative morbidity and mortality. Seventy women more than 75 yr old suitable for spinal anaesthesia were included in this study. They were randomly divided into two groups: in one (n = 35), general anaesthesia was given with sodium thiopentone, fentanyl, enflurane and vecuronium; in the other (n = 35), spinal anaesthesia was performed with 3 ml 0.5% bupivacaine in isobaric solution. During the

Impact of general anesthesia and spinal anesthesia on postal delirium and risk factors in elderly patients undergoing hip fracture surgery.

OBJECTIVE: To compare the effects of general anesthesia (GA) and spinal anesthesia (SA) on postoperative delirium (POD) in elderly patients undergoing hip fracture surgery and to identify associated risk factors. METHODS: A retrospective study was conducted on 186 elderly patients who underwent hip fracture surgery at the Affiliated Hospital of Gansu University of Chinese Medicine from January 2021 to January 2023. Patients were categorized into GA and SA groups. The incidence of POD, postoperative pain control, and cognitive function changes were compared. Univariate and multivariate Logisti

Influence of spinal anesthesia versus general anesthesia on postoperative delirium in patients with hip fractures: A systematic review and meta-analysis.

BACKGROUND: Anesthesia type for hip fracture surgery and its relation to postoperative delirium (POD) is a topic of ongoing debate. This study aimed to elucidate whether there is a significant difference in POD incidence, mortality, and length of hospital stay between general anesthesia (GA) and spinal anesthesia (SA). METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials that compared the outcomes of GA and SA in hip fracture surgeries, adhering to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Data was extracted from 4 da

Anesthesia characteristic of an algorithm of bupivacaine dose based on height in caesarean section under spinal anesthesia: a retrospective cohort study.

BACKGROUND: An algorithm of bupivacaine dose based on height is applied to reduce maternal hypotension in caesarean section under spinal anesthesia. This study is designed to further verify whether the algorithm of bupivacaine dose based on height is suitable. METHODS: The parturients were grouped according to height. The comparison of anesthesia characteristic among subgroups was carried out. The univariate and multivariate binary logistic regressions were executed to reanalyze the interference factor for the anesthesia characteristic. RESULTS: When the dose of bupivacaine was adjusted by u

[Efficacy of spinal anesthesia with small-dose lidocaine in ambulatory prostate biopsy procedures for elderly patients : comparison with propofol anesthesia].

BACKGROUND: Opinions differ as to what is more appropriate anesthetic technique for elderly patients undergoing ambulatory surgery. The objective of this study is to compare the efficacy and safety of spinal anesthesia with small-dose lidocaine and those of propofol general anesthesia in elderly patients undergoing ambulatory prostate biopsy. METHODS: Eighty ASA physical status I x II patients aged older than 65 year were randomized to receive either spinal anesthesia with 1% hyperbaric lidocaine 1 ml (10 mg) (group L) or general anesthesia with 1.0 mg x kg(-1) of propofol injected at 180 mg

Incidence and Management of Pneumocephalus, an Uncommon Complication of Spinal Anesthesia: A Case Report.

The presence of air in the epidural, subdural, or subcranial space within the brain parenchyma or ventricular cavities is a relatively uncommon side effect of spinal anesthesia. In this report, we aimed to describe a 37-year-old male patient who presented with continuous headaches, nausea, and vomiting during spinal anesthesia for joint replacement. A 37-year-old male patient was admitted for elective total hip arthroplasty. The surgery was performed under spinal anesthesia. Within 24 h after surgery, the patient presented with a history of continuous headaches, nausea, and vomiting. A CT scan

Neuraxial anesthesia in obstetric patients with rare spinal pathologies: a scoping review.

BACKGROUND: Neuraxial anesthesia remains the primary approach for obstetric patients, emphasizing the need for a successful blockade. However, many anesthetists view spinal pathologies as contraindications for regional anesthesia. The existing literature on the use of neuraxial anesthesia in obstetric patients with rare spinal pathologies is largely limited to case reports and case series. The goal of this investigation is to evaluate the success rates and complications associated with neuraxial anesthesia in these obstetric patients. METHODS: We searched the databases: PubMed, Embase, Scopus

A Narrative Review on Multimodal Spinal Anesthesia: Old Technique and New Use.

Spinal anesthesia has a long history in providing safe and effective anesthesia and analgesia. Its continuous evolution in block placement techniques with or without ultrasound assistance, options of spinal needles, local anesthetics and adjuvants adopted based on spinal anesthesia onset rate, duration of action, and side effect profiles, makes spinal anesthesia an important modality in current early recovery after surgery for a broad-spectrum of surgical procedures including ambulatory lower extremity orthopedic procedures and major chest, abdominal, and spine surgeries.

Impact of Anesthesia on Brain Functional Networks in Moyamoya Disease and Spinal Lesions.

AIMS: To analyze the effects of intravenous propofol combined with remifentanil on whole-brain functional networks in patients with ischemic moyamoya disease (IMMD) and intraspinal space-occupying lesions (SOLs) using resting-state functional magnetic resonance imaging (rs-fMRI). METHODS: Ten patients with IMMD and 10 sex- and age-matched patients with lumbar SOL (normal cerebrovascular findings on preoperative MRI) were recruited. General anesthesia was administered using propofol and remifentanil. rs-fMRI imaging was performed in both awake and anesthetized states. Whole-brain functional ne
