Local anesthetic blockade of Ca2+ -mediated action potentials in cardiac muscle.

The effects of local anesthetic agents (lidocaine, procaine, cocaine) and diphenylhydantoin (DPH) were studied on the slow electrical responses induced by isoproterenol or caffeine in cardiac muscle preparations rendered inexcitable by tetrodotoxin (TTX) or by partial depolarization with elevated K+ (26 mM). In such inexcitable cells, we previously demonstrated that addition of some positive inotropic agents, such as catecholamines, histamine, and methylxanthines, rapidly increase the number of available slow Ca2+--Na+ channels, thus allowing slowly rising electrical responses resembling the p

Perceptions and practices of epinephrine use with local anesthetics in end-arterial areas among Jordanian physicians: A cross-sectional survey.

Epinephrine is frequently used in combination with local anesthetic agents to enhance their efficacy and duration of action. However, its use in anatomical regions supplied by end arteries remains a topic of controversy. The primary aim of this study was to evaluate physicians' knowledge and practices regarding the use of epinephrine in combination with local anesthetics in end-arterial regions. We also aimed to estimate the prevalence of its use among Jordanian physicians and to examine variations in practice according to physician seniority and medical specialty. A cross-sectional study was

Safety and efficacy of dexmedetomidine in combination with local anesthetics for orthopedic nerve blocks: a systematic review and meta-analysis.

INTRODUCTION: Peripheral nerve block, a common technique for managing postoperative pain and providing intraoperative analgesia, often includes adjuncts like dexmedetomidine (DEX) to enhance the effectiveness of local anesthetics. DEX, known for its α2-adrenoceptor agonist properties, extends sensory blockade and improves postoperative analgesia while offering sedative benefits. The objective of this study is to rigorously assess the effectiveness and safety of perineural DEX injection in orthopedic nerve block procedures, focusing on orthopedic surgeries to minimize heterogeneity and provide

Local anesthetics versus systemic analgesics for reducing pain during Extracorporeal Shock Wave Lithotripsy (ESWL): A systematic review and meta-analysis.

Extracorporeal Shock Wave Lithotripsy (ESWL) is one of the treatment options for patients with renal and ureteral calculi. Even though the procedure is less invasive compared to others, pain caused by the procedure is a major concern. Several studies recommended the use of either local or systemic analgesia with varying results. We aimed to compare the use of local anesthetics and systemic analgesics from randomized controlled trials evaluating pain management during ESWL. A systematic search adhering to the Preferred Reporting Items for Systematic Review and Meta-Analysis protocol was perform

Is referring patients with a positive history of allergic drug reactions or atopy for allergy testing to local anesthetics justified?

Although no more than 1% of adverse reactions to local anesthetics (LA) are thought to be immunologically mediated, many patients continue to be referred to allergy clinics for allergy workup. We evaluated the impact of a history of drug hypersensitivity or atopy on results of allergy testing to LA, with the aim of determining the appropriateness of allergy testing to LA in such patients. We retrospectively analyzed medical records of 112 consecutive patients referred for allergy testing to LA in a 9-year period (1996-2005). Intradermal tests with diluted (1:10) LA were performed to identify p

Protective actions of various local anesthetics against the membrane dysfunction produced by in vitro ischemia in rat hippocampal CA1 neurons.

Local anesthetics block not only the Na(+) but also the K(+) and Ca(2+) channels in the mammalian neurons. It is well known that lidocaine has neuroprotective actions against the ischemic insult of neurons in the central nervous system. In order to elucidate how other local anesthetics as well as lidocaine show the neuroprotective effects against in vitro ischemic insult, intracellular recordings were made from CA1 pyramidal neurons in rat hippocampal slices. Superfusion with the medium deprived of oxygen and glucose (in vitro ischemia) produced a rapid depolarization after 5 min of exposure.

[Local anesthetics. CVI. Preparation and effect of (+/-)-trans-[2-(piperidinomethyl) cyclopentyl] alkyl, alkoxy and halogen substituted esters of phenylcarbamic acid].

Within the framework of the research of potential local anaesthetics, five compounds from the group of (+/-)-trans-[2-(piperidino-methyl)cyclopentyl]alkyl, alkoxy and chloroalkyl esters of phenylcarbamic acid were prepared. The final compounds were obtained by addition of (+/-)-trans-2-(piperidinomethyl)cyclopentanol to the corresponding substituted phenylisocyanates. They were isolated in the form of salts with hydrochloric acid. Their structure was confirmed on the basis of interpretation of the results of UV and IR spectra. A pharmacological evaluation revealed that the final compounds were

[Inhibition by local anesthetics and barbiturates of the active transport of H+ in the synaptic vesicle membranes of the brain].

The effects of local anesthetics and barbiturates on the ATP-dependent H+ transport in synaptic vesicle membranes from rat brain were studied using a fluorescent probe, acridine orange. Local anesthetics depressed the active H+ transport with the following order of potencies: tetracaine trimecaine lidocaine procaine. Respective IC50 values were 0.07, 0.28, 0.46 and 0.60 mM. The local anesthetics also disrupted the endogenous pH gradient seen in the absence of ATP. Barbiturates inhibited the active H+ transport showing IC50 values in the range of 2-5 mM except for benzobarbital and barbital cha

Continuous wound infiltration with local anesthetics in adult patients undergoing cardiac surgery: A PRISMA-compliant systematic review and meta-analysis.

BACKGROUND: Effective postoperative analgesia is critical to recovery after cardiac surgery. Continuous wound infiltration (CWI) with local anesthetics (LAs) has been proposed as an opioid-sparing adjunct, but its efficacy beyond pain control and its safety profile remain incompletely defined. METHODS: We conducted a PRISMA-compliant systematic review and meta-analysis of randomized controlled trials (RCTs) comparing CWI with control in adult cardiac surgery. PubMed, Embase, and Cochrane CENTRAL were searched from inception to August 30, 2025, and updated on April 2, 2026. Primary outcomes we

Pectoserratus Plane Block in Breast Surgery: Analgesic Doses of Local Anesthetics Can Be Used for Surgical Anesthesia-A Case Report.

Based on our experience, the local anesthetic concentrations used to date in fascial plane blocks may be excessively high. Local anesthetic concentrations previously considered only analgesic (eg, ropivacaine 0.3%) could now be considered anesthetic concentrations with the addition of the right adjuvants and with the correct execution of fascial plane blocks. In this article, we present the case of a frail woman, who underwent awake surgery for unilateral breast cancer, thanks to ultrasound-guided pectoserratus plane block, with ropivacaine 0.3% and the addition of dexamethasone and dexmedetom

Local anesthetics inhibit muscarinic acetylcholine receptor-mediated calcium responses and the recruitment of β-arrestin in HSY human parotid cells.

OBJECTIVES: Local anesthetics act on G protein-coupled receptors (GPCRs); thus, their potential as allosteric modulators of GPCRs has attracted attention. Intracellular signaling via GPCRs involves both G-protein- and β-arrestin-mediated pathways. To determine the effects of local anesthetics on muscarinic acetylcholine receptors (mAChR), a family of GPCRs, we analyzed the effects of local anesthetics on mAChR-mediated Ca2+ responses and formation of receptor-β-arrestin complexes in the HSY human parotid cell line. METHODS: Ca2+ responses were monitored by fura-2 spectrofluorimetry. Ligand-in
