Diagnosing and understanding the maxillary lip-tie (superior labial, the maxillary labial frenum) as it relates to breastfeeding.

Successful breastfeeding is dependent upon an infant's ability to correctly latch onto a mother's breast. If an infant is born with oral soft tissue abnormalities such as tongue-tie or lip-tie, breastfeeding may become challenging or impossible. During the oral evaluation of an infant presenting with breastfeeding problems, one area that is often overlooked and undiagnosed and, thus, untreated is the attachment of the upper lip to the maxillary gingival tissue. Historically, this tissue has been described as the superior labial frenum, median labial frenum, or maxillary labial frenum. These te

Upper Lip Tie and Breastfeeding: A Systematic Review.

BACKGROUND: Upper lip tie (ULT) articles have been recorded in Medline since 1998, while "labial frenum" articles have been recorded since 1946. OBJECTIVE: to study the existing medical literature on ULT (or labial frenum or fraenum) as they relate to breastfeeding. MATERIALS AND METHODS: Medline search engine was used to determine and subsequently retrieve all articles published on ULT from 1946 to 2018. Key-words of upper lip tie OR labial frenum were used for the search. We also used Google Scholar and Embase to widen our search, and used the PRISMA criteria for systematic reviews (SRs).

Van der Woude syndrome with an unusual intraoral finding.

Orofacial manifestations of Van der Woude syndrome (VWS) include cleft lip or palate, lower lip pits, hypodontia, hypernasal voice, cleft or bifid uvula, syngnathia, narrow high arched palate, and ankyloglossia. Extraoral manifestations include limb anomalies, popliteal webs, accessory nipples, congenital heart defects, and Hirschsprung disease. We report an interesting case of VWS with characteristic orofacial features along with an unusual additional finding of fusion of primary mandibular left lateral incisor and canine in a 7-year-old boy.

The platysma myocutaneous flap. Indications and caveats.

The platysma myocutaneous flap has enjoyed limited popularity despite its versatility, dependability, and ease of harvesting. In this article, we describe 12 consecutive patients who underwent platysma flap reconstruction of various oral cavity and oropharyngeal defects. Complications included loss of one skin paddle, one pharyngocutaneous fistula, and one intraoral wound dehiscence. In all patients, bare bone was covered or appropriate spacing between the tongue and other structures was maintained to avoid ankyloglossia. We discuss specific indications and caveats.

Linkage of an X-chromosome cleft palate gene.

Many congenital malformations, such as cleft palate and neural tube defects, have a multifactorial origin involving both environmental and genetic factors. Conditions such as these may be exclusively monogenic, polygenic or environmental, but in most cases both genetic and environmental factors are involved. This study describes the sub-chromosomal localization of a single gene defect causing cleft palate and ankyloglossia (tongue-tied) in a large Icelandic family. This defect is a model for the analysis of other neural-crest malformations that show a more complex multifactorial inheritance pa…

Antimicrobial Resistant Pattern and Capsular Typing of Streptococcus Pneumoniae Isolated from Children in Sistan -Baluchestan.

BACKGROUND: Ankyloglossia is an anatomic developmental anomaly determining by thick and short, fi brotic ferenum. Tongue changes have severe eff ects on occlusion and oropharyngeal structures. The aim of this study was to evaluate the position of hyoid in children 7-11 years` old with ankyloglossia in lateral cephalometric radiographs. MATERIALS AND METHOD: In this study 260 nasopharyngeal swabs were taken from non-vaccinated healthy children between 6 months to 6 years old at medical centers in Sistan-Baluchestan during August 2013 to January 2014. These samples were cultured on blood agar. …

Syndromology: an updated conceptual overview. VI. Molecular and biochemical aspects of dysmorphology.

The role of chance using a stochastic single gene model has been shown to generate a continuous liability curve resembling that obtained fro … Gene analysis is discussed and illustrated. Using polymorphic DNA probes to study cleft palate and ankyloglossia in males and ankyloglossia only in females in a large Icelandic family, the responsible gene was found to be located on the long arm of the X chromosome in the Xq21.1 region. In addition to gene analysis, some of the implications of transgenic analy …

Acquired synechia of the tongue to the mouth floor.

Sodium hydroxide or caustic soda is a corrosive agent that can cause extensive damage to the oral mucosa, lips, and tongue when ingested either accidentally or intentionally. These injuries include microstomia, shallow vestibule, ankyloglossia, speech impairment, loss of teeth and impairment in facial expression. In the present article, we report a unique case of tongue adhesion to the mouth floor and its surgical management in a 66-year-old female patient, who had a history of caustic soda ingestion.

Oral care and dental management for patients with epidermolysis bullosa.

Dental treatment is an important part of the multidisciplinary care of patients with epidermolysis bullosa (EB). Routine dental treatment can be difficult to provide in patients with severe tissue fragility caused by mucosal sloughing, microstomia, ankyloglossia, and scarring. This article provides a review of dental practices, exploring different areas of oral management and highlighting the importance of early referral, preventive programs, and close follow-up to maintain oral health. It also discusses treatment modifications and precautions needed for each of the 4 mayor types of EB.

Tongue-tie in the Neonatal Intensive Care Unit Compared to Healthy Newborns.

OBJECTIVE: We sought to characterize the prevalence of ankyloglossia in our neonatal intensive care unit (NICU) population and to determine characteristics of this cohort compared to infants in the birth center (BC). METHODS: Prospective data were collected using a standardized flow sheet. Breastfeeding infants undergoing evaluation for tongue-tie in the BC and NICU were included. Coryllos type, tip to frenulum length, tongue function, frequency of frenotomy, and breastfeeding outcomes were compared. RESULTS: Of 20,879 infants birthed at or admitted to the institution during the study period…

Frenotomy Revision Rate in Breastfeeding Infants: The Impact of Early Versus Late Follow-Up.

Purpose: The protocol for postoperative follow-up time after lingual frenotomy in breastfeeding infants with ankyloglossia was changed from 2 weeks to 1 week at our institution. This study examined the impact of this change in practice on frenotomy revision rate. Materials and Methods: A retrospective chart review of breastfeeding infants who underwent lingual frenotomy for ankyloglossia from January 2016 to December 2017 was performed. Subjects were divided into 1-week (1-9 days) and 2-week (10-20 days) follow-up groups. Statistical analyses were performed to investigate the relationship betw…

TBX22 and tongue-tie.

OBJECTIVE: To resolve if TBX22 mutations cause isolated tongue-tie in the Finnish population. DESIGN: Mutation analysis of the coding region of the TBX22 gene in 50 Finnish isolated tongue-tie patients and 61 control samples. RESULTS: One putative sequence variation was identified from two male patients, but whether this represents a polymorphism or causative mutation remains unknown. CONCLUSIONS: Mutations in the coding region of the TBX22 gene are not a major cause of ankyloglossia in the Finnish population and do not explain the sex difference or inheritance of tongue-tie.
