New developments in the diagnosis and treatment of urinary tract infections.

Not long ago it was customary to expect an atmosphere of confrontation and controversy when urologic surgeons and infectious disease physicians met at conferences and symposia. Even more often each group exhibited an attitude of "benign neglect" towards the other. Times have changed in large part because of the efforts of the leadership group in urology. The establishment of an Advisory Board for this Journal, drawn from non-urological disciplines, is one example of the effort being made to draw together all concerned with urinary tract infections and pyelonephritis. I have been asked to revie

Urinary tract infection in neurogenic bladder.

The most frequent medical complication in patients with neurogenic bladder dysfunction is urinary tract infection (UTI). In the acute phase of neurogenic bladder, aseptic intermittent catheterization should be applied. After this phase, patients are subject to UTIs leading to febrile diseases. In the chronic phase of neurogenic bladders, febrile infections are often accompanied with hypertonic bladder. Some causes or symptoms prior to febrile attack are recognized in many of the cases. Antimicrobial chemotherapy has to be started in time without delay. Newer generation cephalosporins and carba

Bladder neck contracture: incidence, mechanisms, and therapeutic advances.

Bladder neck contracture (BNC) is a progressive narrowing of the bladder neck and adjacent posterior urethra, primarily caused by fibrotic tissue proliferation and scar formation. It commonly occurs following prostate surgery or radiotherapy. Its pathogenesis involves acute and chronic inflammatory responses triggered by surgical or radiation-induced injury, transdifferentiation of fibroblasts into myofibroblasts, and excessive extracellular matrix (ECM) deposition, mediated by the interplay of transforming growth factor beta (TGF-β)/Smad signaling and mechanotransduction feedback. Clinically,

Phytochemical investigation of Phyllanthus emblica (Amla) for therapeutic application in urinary tract infection (UTI) management and development of functional tea.

Phyllanthus emblica L. (amla) is a medicinal fruit, rich in secondary metabolites with reported anti-inflammatory and antimicrobial effects. The current study aimed to assess the impact of drying and extraction techniques on phytochemical recovery, the biological potential of P. emblica relevant to the management of urinary tract infection (UTI), and the development of functional tea. Two different processing methods were adopted. Method 1 involves sun-drying of P. emblica and extracting with methanol and ethanol using an orbital shaker at 40 °C. Method 2 involves freeze-drying of P. emblica a

Caspase-like activity is associated with bacterial infection of the urine in urinary tract diseases.

Urinary tract infections (UTIs) are a serious public health problem. They can be caused by a number of pathogens, but the most common are Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Enterococcus faecalis and Staphylococcus saprophyticus. Bacterial infection is diagnosed by examining a urine sample. The presence of bacteria or white blood cells is determined under a microscope or a urine culture is performed. In this study, we used a panel of chromogenic substrates for the qualitative determination of specific enzyme activity in the urine of patients with confirmed bacterial inf

Antimicrobial prophylaxis for vesicoureteral reflux: which subgroups of children benefit the most?

BACKGROUND: While the Randomized Intervention for Children with Vesicoureteral Reflux (RIVUR) trial found that long-term antimicrobial prophylaxis reduced the risk of urinary tract infection (UTI) recurrences by 50%, 10 children had to be treated for one to benefit (i.e., observed number needed to treat (NNT) of 10). Accordingly, we re-analyzed RIVUR data to systematically identify subgroups of children with vesicoureteral reflux (VUR) with a smaller NNT. METHODS: Using patient-level data from the RIVUR trial, we applied penalized regression methods including the baseline age, VUR grade, type

Short-course antibiotics for common infections: what do we know and where do we go from here?

BACKGROUND: Over the past 25 years, researchers have performed >120 randomized controlled trials (RCTs) illustrating short courses to be non-inferior to long courses of antibiotics for common bacterial infections. OBJECTIVE: We sought to determine whether clinical data from RCTs affirm the mantra of 'shorter is better' for antibiotic durations in 7 common infections: pneumonia, urinary tract infection, intra-abdominal infection, bacteraemia, skin and soft tissue infection, bone and joint infections, pharyngitis and sinusitis. SOURCES: Published RCTs comparing short- versus long-course antibi

Morganella morganii, a non-negligent opportunistic pathogen.

Morganella morganii belongs to the tribe Proteeae of the Enterobacteriaceae family. This species is considered as an unusual opportunistic pathogen that mainly causes post-operative wound and urinary tract infections. However, certain clinical M. morganii isolates present resistance to multiple antibiotics by carrying various resistant genes (such as blaNDM-1, and qnrD1), thereby posing a serious challenge for clinical infection control. Moreover, virulence evolution makes M. morganii an important pathogen. Accumulated data have demonstrated that M. morganii can cause various infections, such

[Nosocomial urinary tract infection in adults].

Urinary tract infection (UTI) is the most common infection in hospitalized adults. Nosocomial UTIs are mainly associated with the use of urinary catheters. Thus, the decision for catheterization should be made carefully and catheters removed in time. In order to prevent unnecessary antibiotic use in patients with urinary catheters correct diagnosis is crucial. Chinolones, broad-spectrum penicillins and third-generation cephalosporins are the mainstay of therapy. Comorbidities should be considered and potential obstructions of urinary flow removed. Economically important are the normally higher

Measuring antibiotic use in a long-term care facility.

BACKGROUND: In the long-term care facility setting, there is little information about correlation of antibiotic use with care delivered or with the occurrence of fever or use of Foley catheters. The objectives of this study were to compare various measures of quantitating antibiotic use and to correlate these measures with febrile morbidity and Foley catheter use in a hospital-based, long-term care facility. METHODS: This was a prospective study in which the number of residents with fever (rectal temperature of 100.5 degrees F or greater) or a Foley catheter was documented daily. Antibiotic u

Proposing the "Continuum of UTI" for a Nuanced Approach to Diagnosis and Management of Urinary Tract Infections.

PURPOSE: Patients with suspected UTIs are categorized into 3 clinical phenotypes based on current guidelines: no UTI, asymptomatic bacteriuria (ASB), or UTI. However, all patients may not fit neatly into these groups. Our objective was to characterize clinical presentations of patients who receive urine tests using the "continuum of UTI" approach. MATERIALS AND METHODS: This was a retrospective cohort study of a random sample of adult noncatheterized inpatient and emergency department encounters with paired urinalysis and urine cultures from 5 hospitals in 3 states between January 01, 2017, a

Occult bladder injury after laparoscopic appendicectomy.

Minimally invasive procedures have revolutionised surgery by reducing pain and the length of hospital stay for patients. These are not simple procedures and training in laparoscopic surgery is an arduous process. Meticulous preparation prior to surgery is paramount to prevent complications. We report a rare complication involving a 35-year-old patient who underwent a laparoscopic appendicectomy for a perforated appendix. Two days after surgery the patient experienced redness and swelling in the lower abdominal region and oliguria. A delayed computer tomography (CT) scan revealed contrast leaka

Should antibiotics be prescribed when urinary catheters are removed?

Each year around one million urinary catheters are inserted in NHS hospitals. In around 60% of cases prophylactic antibiotics are prescribed prior to removal. This practice aims to reduce infection risks, but is not supported by evidence. Inappropriate prescribing increases antibiotic resistance, already a serious and worsening problem, and exposes patients to the hazards of antibiotic prescribing. This practice also costs an estimated £1.35-£2.7 million pounds annually. Nurses at all levels can have a huge influence on catherterization rates and catheter management. This can reduce the risks

Effects of antibacterials on the unborn child: what is known and how should this influence prescribing.

Antibacterials are among the most commonly prescribed drugs worldwide. In general, infections occur in pregnant women at much the same rate as in the general population. However, as a result of physiological changes brought about by pregnancy, some infections, such as those of the urinary tract, may have an increased incidence. It is important to remember that almost every drug crosses the placenta, ensuring that the unborn fetus is also exposed. When prescribing an antibacterial agent to a pregnant woman, it is important that the mother is treated appropriately while at the same time protecti
