Operative treatment of anterior ectopic anus: the efficacy and influence of age on results.

In our experience, anterior ectopic anus (AEA) is a common cause of constipation in children. We have performed 54 anoplasties for AEA in the past 8 years. Seventy-eight percent of the patients were girls. The average age at surgery was 23.8 months and the average stool frequency prior to surgery was once every 3.2 days. Eighty percent of the children had undergone attempts at medical therapy. We compared the functional results of anoplasty in children by age. The children who had surgery prior to the age of 6 months had a significantly better outcome than did older children. These data sugges

Quality of gastrointestinal surgical oncology care according to insurance status.

BACKGROUND: Despite efforts to expand insurance coverage, substantial inequalities persist, particularly in cancer treatment. This study aimed to evaluate whether quality disparities exist across major insurance plans for patients undergoing curative-intent resection for gastrointestinal (GI) cancers. METHODS: This was a retrospective study of adult patients in the National Cancer Database diagnosed with GI malignant neoplasms between January 1, 2004, and December 31, 2020. The primary tumor organ sites include the anus, colon, esophagus, gallbladder, liver, other biliary organ, pancreas, per

Cancer in persons diagnosed with facial nerve paresis: A hospital-based cohort study in Denmark.

BACKGROUND: Neoplasms can compress or infiltrate the facial nerve, leading to facial nerve paresis or palsy (FNP). As evidence regarding cancers outside the head, neck, and nervous system in persons initially diagnosed with FNP remains limited, we examined the overall and site-specific cancer rates among all patients diagnosed with FNP in Denmark over a 29-year study period. METHODS: We conducted a hospital-based cohort study in Denmark, 1994-2022, identifying FNP from inpatient, outpatient, or emergency room visits. We computed absolute risks (AR), and age- and sex-standardized incidence rat

Anal cytology as a screening tool for early detection of anal dysplasia in HIV-infected women.

BACKGROUND AND AIM: HIV-infected patients show a high rate of anal dysplasia and anal carcinoma but there is no gold standard for early detection. Therefore, the objectives of this prospective study were: a) evaluation of an anal screening using anal/perianal cytology; b) in case of a positive result to investigate its relation to immune status, clinical symptoms of HIV infection and antiretroviral therapy. PATIENTS AND METHODS: In every HIV-infected woman visiting our gynaecological outpatient clinic, an anal and perianal swab for anal cytology was taken. One experienced cytologist examined

Polypoid lesions at the anorectal junction: barium enema findings.

Although the anorectal junction is easily accessible, this area may be difficult to evaluate both radiologically and endoscopically because it is relatively collapsed. This pictorial essay illustrates the barium enema radiographic findings and reviews the radiologic and endoscopic pitfalls in the diagnosis of lesions of the anorectal junction.

Brain Metastasis From Anal Canal Squamous Cell Carcinoma: A Rare Case Report and Systematic Review.

Anal canal squamous cell carcinoma (ACSCC) is a rare malignancy, accounting for approximately 2% of all colorectal cancers. Brain metastases originating from this primary site are exceedingly uncommon and scarcely documented. The aim of this study is to review the literature-reported cases of brain metastases from ACSCC and to supplement this analysis with the description of a clinical case. A systematic review of the literature was conducted in accordance with PRISMA guidelines to identify published cases of brain metastases from ACSCC. The clinical and therapeutic features of these cases wer

MR Imaging of Anal Cancer.

Anal cancer represents a rare type of gastrointestinal malignancy with anal squamous cell carcinoma as the most common histologic type. This review aims to present up-to-date imaging recommendations for anal cancer, including for baseline staging and follow-up assessment after treatment, with an emphasis on the role of MR imaging. Treatment complications are also discussed, helping to familiarize radiologists to include them in their search patterns. Lastly, a short discussion on the future horizon of anal cancer imaging is presented.

Mucinous adenocarcinoma arising from a complex perianal fistula: a diagnostic and therapeutic challenge.

We present a case of a 64-year-old male with a history of perianal abscesses that have been surgically treated on 10 occasions. Eight months after the last drainage procedure, he presented with a new abscess. Drainage was performed, revealing a cavity with smooth walls, a chronic appearance, filled with mucoid material. An internal fistulous opening was identified at the 6 o'clock position above the anorectal line, which communicated with the described cavity, forming a trans-sphincteric fistula to the mid-anal canal. Biopsy with pathological anatomy showed a mucinous adenocarcinoma with possi

[Diagnostics, treatment and aftercare of anal cancer].

Despite the increasing incidence, anal cancer is still a rare gastrointestinal tumor, so that due to the broadness of the primary care there is often little experience in the care of affected patients. Squamous cell cancer (SCC) constitutes more than 90% of all anal cancers and is nearly always associated with an infection by the human papillomavirus. This article concentrates on SCC of the anal canal and anal margin. The focus is on the primary diagnostics, surgical treatment, response assessment and aftercare. Treatment is carried out according to the decision of the interdisciplinary tumor

The role of circulating viral and tumour DNA in the diagnosis and management of HPV associated anogenital cancers, a systematic review and meta-analysis.

BACKGROUND: Human papillomavirus associated anogenital cancers are a significant global burden. The detection of biomarkers (circulating tumour DNA; ctDNA or circulating HPV DNA; cHPV DNA) in blood referred to as "liquid biopsy" may support the early diagnosis and monitoring of affected individuals. METHODS: A systematic review, including meta-analysis of studies available in the literature on the utilization of ctDNA and cHPV DNA as diagnostic, predictive, and monitoring biomarker tests of HPV associated anogenital cancers was performed following the criteria of PRISMA. RESULTS: A total of

Quality of life in patients treated with radiochemotherapy for primary diagnosis of anal cancer.

Anal cancer and the related treatment are generally known to affect patients' quality of life. The aim of this study was to assess self-reported quality of life (QoL) of anal cancer patients after combined radiation and chemotherapy, and to identify patient-, disease-, and therapy-related factors associated with QoL. A total of 94 patients treated with definitive chemoradiation for anal cancer at our institution in the period from 2004 to 2018 were identified from our database. QoL was assessed in the remaining 52 patients using the EORTC QLQ-C30 questionnaire (cancer-specific QoL) and the new

[Image-guided radiotherapy contribution and patient setup for anorectal cancer treatment].

Intensity-modulated radiation therapy is recommended in anal squamous cell carcinoma treatment and is increasingly used in rectal cancer. It adapts the dose to target volumes, with a high doses gradient. Intensity-modulated radiation therapy allows to reduce toxicity to critical normal structures and to consider dose-escalation studies or systemic treatment intensification. Image-guided radiation therapy is a warrant of quality for intensity-modulated radiation therapy, especially for successful delivery of the dose as planned. There is no recommended international or national anorectal cancer

[Laparoscopic abdominoperineal resection after preoperative chemoradiotherapy for advanced carcinoma associated with anal fistula].

The patient was a 71-year-old man who was diagnosed with anal fistula 50 years previously. He complained of mucous and bloody stools. He was diagnosed with a carcinoma associated with anal fistula after biopsy. Image examination showed that the tumor was filled with mucinous substances and that it had invaded the levator ani muscle, with left external iliac and left inguinal lymph node metastases. Therefore, preoperative chemoradiotherapy for locally advanced cancer was administered. After chemoradiotherapy, the tumor and metastatic lymph nodes reduced in size. We performed laparoscopic abdomi
