The posterior chamber intraocular lens particularly for ciliary sulcus fixation is a very good solution of correction of aphakia in cases when any part of capsule is missing. With proper surgical technique the obtained results are very good and the number of complications reported can be minimized. The authors demonstrate it in a group of 38 eyes with ciliary sulcus fixation. A very low occurrence of complications and good visual acuity including long follow-up confirm, that this method can be successfully used in the case od correction of aphakic patients.

The Federal Aviation Administration allows civilian airmen with aphakia to fly with waivered certificates. This study analyzes, in the civil airman population, the distribution of aphakia and intraocular lens (I.O.L.) implant by type (unilateral, bilateral), class of airman medical certificate, and gender, for a 4-year period (1982-85). Medical records were individually evaluated for all certified airmen who were carrying FAA-specific codes for the pathology categories during the study period. The prevalence of both aphakia and I.O.L. increased most for bilateral type and second-class certific…

Epikeratophakia for the correction of aphakia, myopia, hyperopia and keratoconus has overcome many of the objections to earlier lamellar refractive procedures (keratophakia and keratomileusis), such as the lamellar dissection of the recipients' healthy cornea over the visual axis and the need for expensive surgical equipment. It is reasonably reversible and repeatable. The complications observed in a series of 119 operations performed at the 2nd Department of Ophthalmology, University of Vienna, are discussed and compared with those reported in the literature. Serious complications, both intra…

Fifty adult aphakic patients were treated surgically by epikeratoplasty. Forty-one eyes had posttraumatic aphakia including 26 eyes with corneal scars. Results are presented with a mean follow-up of 31 months. 64% of eyes achieved an uncorrected visual acuity of 0.2 or better. Best visual acuity was 0.2 or better in 92% of eyes (this percentage is similar to the preoperative figure) and 0.5 or better in 62%. 78% of eyes had a refraction within 3 diopters of emmetropia. 10% of eyes lost 2 or more lines of best visual acuity. Mean keratometric results and spheric equivalent of the refraction sho…

We have treated 12 unilaterally aphakic patients, with a manifest squint and binocular diplopia, with botulinum toxin injection to the appropriate horizontal rectus muscle, in an attempt to reduce the angle of squint and thereby resolve the diplopia. In all cases a short-term reduction in the angle of squint was achieved. In nine patients, whose aphakia was corrected with a contact lens, and eight of whom had had their lenses removed because of trauma, this reduction was only temporary. In three patients, however, who had had a non-traumatic cataract removed, replaced with a posterior chamber …

Over the last 20 years, the correction of aphakia has moved from spectacles and contact lens to intraocular lenses. Advances in lens design and implantation techniques are now in the exciting process of refinement--the aim being bionic perfection.

The practicality of extended-wear contact lenses in the refractive correction of pediatric aphakia was assessed with 240 eyes in 184 patients. Patient ages at the time of contact lens fitting ranged from 18 days to 9.8 years. One hundred forty-one eyes were fit from 1 day to 55 months postoperatively and then followed for 6 months to 5.7 years (average, 29 months). Only five patients lost more than five lenses. The overall loss rate was less than one lens per year of follow-up. No patient had contact lens-related complications with permanent visual sequelae. Only 14% of patients had contact le…

An evaluation of cases in which an expert opinion was obtained during the last 2 years shows that only one-third of patients with traumatic aphakia and a 20/30 loss of vision are adequately assessed. If there are further consequences of the accident (complicated aphakia) they should also be taken into account when assessing loss of vision. A simple method of doing this is described on the basis of three examples.

Thanks to modern methods of correction in aphakia, (precornea lenses, and intra-ocular implants), the patient is less and less concerned by the major inconveniences linked to aphakia. It is, therefore, desirable to fix, in common law, the rate of partial permanent incapacity, according to whether the aphakic patient is well or poorly compensated by a prothesis. The residual global incapacity rate, would be the sum of the two following figures: 1 - A basic disability figure, of five to eight per cent taking into account the discomfort brought by the accessory consequences of the aphakic conditi…

The rationale, indications, contraindications, and technique for the use of the Choyce Mark IX anterior chamber implant are discussed. The results of 130 operations are presented. Excluding eyes with pre-existing pathology, 98.3 per cent achieved 6/12 or better. The Choyce Mark IX anterior chamber implant is relatively easy to insert, provided proper attention is given to surgical technique, it gives good results used secondarily or primarily, can be used in conjunction with a surgeon's routine technique, and has a complication rate at least as low as that of other forms of intraocular implant

The insertion of an intraocular lens at the time of cataract surgery has become a very popular method of correcting aphakia. However, it is not done without adding extra risks to the operation and postoperative course. The use of a contact lens, though not quite as effective from a visual standpoint, involves fewer risks. The use of the extended-ear soft contact lens will further enable to correction of aphakia. The more the extended-wear lens becomes successful, the less need will there be for an intraocular lens.

A case is reported of a young man in whom blunt injury caused a horizontal corneal tear superiorly. Aniridia, aphakia, cyclodialysis with angle recession, minimal hyphaema, vitreous haemorrhage, choroidal rupture and striae, and scleral buckling resulted. Vision improved to 6/12+.

Seveteen children under the age of ten years with unilateral traumatic aphakia, who had been fitted with contact lenses, were reviewed by an ophthalmologist and an orthoptist, on average three years after the injury. Thirteen children were still using their lenses. Of the seventeen, none were deeply amblyopic. Only one had orthophoria. Twelve of the seventeen had heterotropia, which in most cases was moderate. Six of the children achieved stereopsis in the synoptophore, only four by the Titmus sterotest, all of these were six years old or more at the time of the injury, and had had lenses fitt…
