Steroid-induced ptosis: case studies and histopathologic analysis.

PURPOSE: The purpose of this study was to review patients who developed ptosis after subtenon's steroid injection and to study the mechanism of steroid-induced ptosis in an animal model. METHODS: Part 1. Twenty-two patients with uveitis who had received posterior subtenon's triamcinolone acetonide injections were retrospectively reviewed. Demographics, type of uveitis, type and number of surgeries, pre and postoperative marginal reflex distance (MRD1), and clinical outcomes were evaluated. Part 2. Study of rabbit levator muscle and aponeurosis histopathology after subtenon's triamcinolone inj…

[Percutaneous needle aponeurotomy for Dupuytren's contracture].

Dupuytren disease is a benign fibroproliferative disease of the palmar aponeurosis, which can cause considerable functional deficiencies for the person concerned. Partial aponeurectomy is the gold standard in primary surgery. Because it is minimally invasive and has short recovery and low complication rates, the importance of needle aponeurotomy under specific indications has been increasing in the last years. Needle aponeurotomy is a cost-effective treatment with low complication rates. The revision rate compared to partial aponeurectomy is higher. Under consideration of specified indications…

Fasciitis-panniculitis syndrome simulating deep venous thrombosis.

Fasciitis-panniculitis syndrome (FPS) is a condition characterized by inflammation of the subcutaneous cellular tissue, the aponeurosis and, eventually, the muscles. A 49-year-old female who was previously diagnosed as having retroperitoneal fibrosis presented recurrent episodes of intermittent inflammation of her limbs that simulated deep venous thrombosis (DVT). Biopsy revealed panniculitis with involvement of the fascia, characterizing FPS. Clinical improvement was achieved after corticosteroid therapy. Prompt recognition of FPS is important in order to avoid inadequate therapeutic measures…

The functional palatorrhaphy in the treatment of obstructive sleep apnoea.

We have developed a new operation for the surgical treatment of obstructive sleep apnoea, which we called functional palatorrhaphy. This preserves the posterior border of the soft palate and allows a controlled repair of the aponeurosis of the soft palate muscle. We combine the palatorrhaphy with a standard or a modified chin osteotomy with advancement of the base of tongue. In our first five treated and re-evaluated patients with severe obstructive sleep apnoea syndrome, who did not respond to conservative treatment, the operation was successful. All patients but one were considerably improve…

Calcaneal abnormalities in articular disorders. Rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis, and Reiter syndrome.

The calcaneus of normal control patients, and those with rheumatoid arthritis, ankylosing spondylitis, psoriasis, and Reiter syndrome was studied. Pathological abnormalities of the plantar aponeurosis, Achilles tendon, and retrocalcaneal bursa account for the radiographic alterations. In normal individuals, lateral radiographs show the thickness of the Achilles tendon to be not greater than 8 mm; the retrocalcaneal bursa creates a radiolucency which extends at least 2 mm below the superior surface of the calcaneus. In patients with any of these 4 articular disorders, Achilles tendinitis produc…

The Cutaneous Branches of the Median and Ulnar Nerves in the Palm.

PURPOSE: The dermatomal distributions of the ulnar and median nerves on the palmar skin of the hand have been studied thoroughly. However, t … of branches originating between the superficial palmar arch and the transverse fibers of the palmar aponeurosis (these nerves had a longitudinal trajectory and were shorter than the branches originating proximal to the palmar arch); and a metacarpophalangeal group, composed of short perpendicular branches originating on the palmar surface of the proper palmar …

The rise of the longitudinal arch when sitting, standing, and walking: Contributions of the windlass mechanism.

The original windlass mechanism describes a one-to-one coupling between metatarsal joint dorsiflexion and medial longitudinal arch rise. The description assumes a sufficiently stiff plantar aponeurosis and absence of foot muscle activity. However, recent research calls for a broader interpretation of the windlass mechanism that accounts for an extensible plantar aponeurosis and active foot muscles. In this study, we investigate the rise of the arch in response to toe dorsiflexion when sitting, standing, and walking to discuss the windlass mechanism's contributions in static and dynamic load sc…

Changes in carpal tunnel compliance with incremental flexor retinaculum release.

BACKGROUND: Flexor retinaculum transection is a routine surgical treatment for carpal tunnel syndrome, yet the biomechanical and clinical sequelae of the procedure remain unclear. We investigated the effects of flexor retinaculum release on carpal tunnel structural compliance using cadaveric hands. METHODS: The flexor retinaculum was incrementally and sequentially released with transections of 25, 50, 75, and 100 % of the transverse carpal ligament, followed by the distal aponeurosis and then the antebrachial fascia. Paired outward 10 N forces were applied to the insertion sites of the transv…

[Axillary lymphadenectomy with muscular padding, without drainage].

OBJECTIVE: Axillary padding without drainage appeared to be a valuable alternative technique to vacuum drainage. The technique employs local muscles or the axillary aponeurosis for padding. We report here the clinical evaluation of muscular padding without drainage. The analysis of these results prompted us to also do a literature search for other alternatives aimed at reducing morbidity due to vacuum drainage. PATIENTS AND METHODS: Muscular padding was prospectively performed by 8 different surgeons on a total of 152 patients at the Centre Rene-Huguenin (Saint-Cloud, France). Follow-up has a…

[Fibronectin in Dupuytren contracture].

The distribution of fibronectin in the palmar aponeurosis of patients with Dupuytren's contracture was compared with that of normal controls, using an immunofluorescent technique. The tissues were also investigated with type-specific anticollagen antisera (anti-type I and anti-type III collagen). Tissues from Dupuytren's contracture patients showed intense fibronectin fluorescence in the whole range studied. Collagen fibre bundles are characterized by a peripherally increased fibronectin concentration. Type I collagen is uniformly distributed, while type III collagen also seems to be enriched …

Surgical anatomy of the orbital region, local anesthesia and general considerations.

Orbital region may be divided into four sectors: medial canthus, lateral canthus, lower lid and upper lid. A vertical section of a lid shows 5 layers: skin, subcutaneous tissue with orbicularis muscle, fibrous layer (tarsus-aponeurosis), plain fibers muscle layer (Muller's), conjunctiva. The first two layers form the "anterior lamella" and the other ones the "posterior lamella", divided by the grey line. In this review Authors describe with all details the orbital region and the eyelid layers, considering the most important concepts for surgery. Finally they resume basics of local anesthesia i…

A Conservative Crease-Height-Adjustable Asian Double-Eyelid Surgery Technique Based on Live Anatomical Studies.

BACKGROUND: Ideal methods for double-eyelid crease creation in Asian upper eyelids remain controversial because of the complexity of the Asian upper eyelid anatomy. Key issues include the underestimation of tarsal height and the septum/aponeurosis fusion point height (FPH), which may underlie the unnaturally high creases in many classic double-eyelid procedures. METHODS: A total of 1272 patients had tarsal heights measured. Among 386 patients with orbital septa opened, FPHs and other anatomic findings were documented. Based on the anatomic findings, a novel method for double-eyelid crease for…

Characteristics of Upper Eyelid Thickness in Asians.

PURPOSE: This study examines the characteristics of upper eyelid thickness in Asians microscopically. METHODS: Histological evaluations of … erately thick type (5 eyelids in 4 cadavers) exhibited preaponeurotic fat fully reaching the septum-aponeurosis junction with minimal anterior muscle displacement, with less thick subcutaneous tissue. The thin type (8 eyelids in 6 cadavers) demonstrated preaponeurotic fat failing to reach the septum-aponeurosis junction, with thinner subcutaneous tissue. Subc …

A rare discovery of Spigelian-cryptorchidism syndrome in adult.

Spigelian hernia (SH) occurs due to the protrusion through a congenital or acquired defect or weakness in the Spigelian aponeurosis. SH accounts for only 0.1-0.4% of occurrence and a 17-24% risk of strangulation. We hereby report a case of a 34-year-old gentleman presented with concomitant incarceration of the omentum with small intestine and testis in Spigelian hernia sac. We have successfully operated on this patient via a transperitoneal approach with a small incision over the hernia site. This incision could be an alternative to midline laparotomy as a safe and effective method in managing…

Gravity Equinus Position to Control the Tendon Length of Reversed Free Tendon Flap Reconstruction for Chronic Achilles Tendon Rupture.

Repair of chronic Achilles tendon rupture is a surgical challenge. We describe the use of a free turndown tendon flap augmentation raised from the proximal gastrocnemius aponeurosis. To control optimal tension or the reconstructed Achilles tendon length, we used an original method by referring to the gravity planter flexion ankle angle of the contralateral limb. Key aspects of the technique are described. A retrospective analysis of the short-term outcomes achieved in a case series (n = 56) is presented. The postoperative anthropometric findings are also presented to indicate the successful ou…

Stener-like lesions due to radial collateral ligament rupture in the metacarpophalangeal joints of two adjacent fingers: A case report and review of literature.

Rupture of the collateral ligaments in the metacarpophalangeal (MCP) joint of a finger is less common than in the thumb. Literature about these lesions is sparse but the number of chronic injuries reported underlies the number of cases overlooked by physicians at the first visit. A Stener-like lesion is characterized by interposition of the extensor hood or the sagittal band between the torn collateral ligament of the MCP joint of a finger and its insertion. It was originally described as interposition of the adductor aponeurosis of the thumb. We present the first case of a Stener-like lesion …

Normal Variants: Accessory Muscles About the Ankle.

Accessory muscles around the ankle are commonly encountered as incidental findings on cross-sectional imaging. Mostly asymptomatic, accessory muscles sometimes mimic mass lesions. They have been implicated as the cause of tarsal tunnel syndrome, impingement of surrounding structures, and chronic pain. Distinguishing these muscles can be challenging, because some travel along a similar path. This article describes these accessory muscles in detail, including their relationships to the aponeurosis of the lower leg. An imaging algorithm is proposed to aid in identification of these muscles, provi…

[The quadriga phenomenon of the extensor tendon system and the superficial transverse metacarpal ligament].

The aponeurosis of the common extensor tendons is connected by intertendinous connections proximal to the metacarpophalangeal joints. These fibers are responsible for tenodesis effects in case of maximal finger extension and flexion and by this means they diminish the function of the other metacarpophalangeal joints. This mechanism can be described as the Quadriga-Syndrome of the extensor apparatus. These connecting structures also help to stabilize the transverse metacarpophalangeal arch and the position of the extensor tendons over the metacarpal heads. Cadaver dissections as well as operati…
