We describe the case of a 20-year-old male who developed right-arm choreic movements secondary to a giant unruptured aneurysm impinging upon the left thalamus, putamen, globus pallidus, cerebral peduncle, midbrain, and subthalamic nucleus. The aneurysm was treated successfully with coils and a supraclinoid balloon. Abnormal movements initially failed to ameliorate, but within a few months, it was possible to discontinue symptomatic haloperidol therapy, with only mild residual abnormal movements.

We present a case of unilateral terminal transverse forearm deficiency with subterminal digit-like nubbins, identified in a fetus from a pregnancy terminated electively in the second trimester because the distal right arm and hand could not be seen by ultrasound and were presumed to be absent. This report describes a mother with a balanced intrachromosomal insertion of band q22 on chromosome number 2 into band p24 on the same chromosome. She had had four spontaneous abortions and two induced abortions. One foetus had a suspected obstruction of the uretero-pelvic part of the urinary tract and monosomy of band 2q22, the other foetus had anencephaly and trisomy of band 2q22. By microdissection we have generated a painting probe from the mother's abnormal short arm of chromosome 2 (der2p probe). This family specific probe will be used in future pregnancies for precise diagnosis.

It is estimated that 1.2 million workers are now exposed to hazardous levels of vibration in their work. In the year 1995-1996, over 3,000 new cases of hand-arm vibration syndrome were assessed by the Department of Social Security (DSS) under the Industrial Injuries Disablement Benefit Scheme. Formerly called vibration white finger, it is the most common of all diseases assessed by the DSS, with the majority of workers affected having up to 14 per cent disability. This article explains the syndrome and its management.

Four-fragment fractures have to be accorded great importance because they are so severe. The morphology allows differentiation of two types, the dorsolateral fracture type and the impacted fracture type. Over a 4-year time span, 13 of these fractures were treated with minimal osteosynthesis. This involve the open anatomical reduction and allogeneic spongiosa grafting and retention of the fracture with screws and, if necessary, K-wires and tension banding. Experience is reported in the use of eight fascio-cutaneous cross-arm and forearm flaps in three patients. This has proved to be a reliable and satisfactory means of providing skin cover in reconstruction of the hand, giving flaps of a length to breadth ratio of at least two to one. The method was particularly useful where multiple defects were encountered, although in one case where a single defect required reconstruction, the ability to tube the pedicle of the flap allowed full mobility of the injured hand. A 14-year-old boy presented with physical signs of the Silver-Russell syndrome. He further had low-set, large protruding ears, ptosis, broad nasal base, thick protruding lips, pronounced caried, malaligment of teeth, micrognathia and mental retardation. Chromosome examination showed deletion of the short arms of chromosome No. 18. It is concluded that considering the previous findings of chromosome No. 18 aberrations in some patients with Silver-Russell syndrome such patients should always have chromosome examination made.
