A bicentre retrospective study of features and outcomes of patients with reactive arthritis.

OBJECTIVE: Reactive arthritis (ReA) is a sterile arthritis following an extra-articular infection, usually of the gastrointestinal or genitourinary tract. The aim of this study was to assess the incidence and the clinical and therapeutic characteristics of ReA and to compare them with those of a historical cohort. We hypothesised that improved hygiene together with prevention and treatment of sexually transmitted infections may have decreased the incidence of ReA. METHODS: All patients with ReA diagnosed in the University Hospital Centres of Lyon Sud and Besançon from January 2002 to December

A Case of Reactive Arthritis Associated With Lymphogranuloma Venereum Infection in a Woman.

We report the first case of reactive arthritis associated with lymphogranuloma venereum (LGV) in an Italian human immunodeficiency virus-negative woman with urogenital and rectal Chlamydia trachomatis L2 serovar infection. The LGV-associated arthritis has to be considered even when classic symptoms of arthritis are missing and in case of asymptomatic or cryptic LGV localizations.

A clinical profile of reactive arthritis in a peruvian series: a pilot study.

Enteric pathogens are an endemic cause of acute diarrheal diseases among Peruvians. The frequency of HLA-B27 in the general population (0%-3%) is lower than that among Caucasians in the United States, Canada, and Europe (6%-14%). It was believed that both of these factors were likely to interact to modulate the clinical expression of reactive arthritis (ReA) or Reiter's Syndrome (RS). The objective of our study was to define the clinical characteristics of ReA in a group of patients treated in a general hospital in Lima, Peru. Between December 1, 1976, and June 30, 1993, using a standardized p

[Involvement of the foot in reactive arthritis. A retrospective study of 105 cases].

The foot is among the sites most often affected in spondyloarthropathies, whose diagnostic criteria include heel pain and sausage-like swelling of the toes. Few studies have systematically analyzed foot manifestations in reactive arthritides. We retrospectively reviewed 143 patients fulfilling Amor's criteria. One hundred five patients (73%) exhibited inflammatory involvement of one (n = 47) or both feet. In 8 cases no other articular sites were affected. Heel pain was reported by 36% of patients (52/143), within the first six months in half the cases. Both heels were painful in 26 patients. H

Association between Giardia and arthritis or joint pain in a large health insurance cohort: could it be reactive arthritis?

This study aimed to assess the association between giardiasis and subsequent development of arthritis or joint pain using a retrospective cohort of individuals from a large administrative claims database in the United States. Using 2006-2010 data from MarketScan Commercial Claims and Encounters, we conducted a retrospective cohort study in people with an ICD-9-CM code for giardiasis (n = 3301) and persons without giardiasis (n = 14 612) individually matched on age, sex, and enrolment length. We used conditional logistic regression to model the association between giardiasis and arthritis or jo

Prodrug-loaded micelles targeting articular cavity reactive oxygen species for rheumatoid arthritis treatment.

Oxidative stress plays an important role in the pathogenesis of rheumatoid arthritis (RA). Excessive reactive oxygen species (ROS) activate the NLR family pyrin domain containing 3 (NLRP3) inflammasome, release multiple inflammatory mediators, stimulate macrophage polarization, and exacerbate the inflammatory response. In response to the pathological features of RA, we designed a dihydroartemisinin (DHA)-ROS sensitive prodrug loaded into carriers of Soluplus and Vitamin E polyethylene glycol succinate (TPGS) using a thin film hydration to expect that the nano-micelles could target for delivery

Reactive arthritis in connective tissue diseases; one should be vigilant for joint tuberculosis.

The incidence of tuberculosis (TB) is increasing worldwide. Bone and joint TB are the most common types of extra-pulmonary TB, but can easily be misdiagnosed, since articular TB commonly mimics other joint diseases, including reactive and degenerative arthritis, and articular manifestations associated with systemic autoimmune diseases. Furthermore, patients with systemic auto-immune diseases are often more prone to mycobacterial infection. The diagnosis of joint TB can be made using a combination of the following methods: clinical evaluation, imaging, tuberculin skin test, interferon-gamma rel

Reactive Arthritis Resulting From Postoperative Complications of Third Molar Extraction: A Case Report.

The role of bacterial infections as potential triggers for chronic rheumatic diseases is well-documented. Oral infections such as periodontitis may play a particularly important role in the development of autoimmune diseases, with the oral cavity serving as a reservoir for pathogenic bacteria. These bacteria may trigger dysregulatory immune responses through mechanisms like molecular mimicry, whereby bacterial peptides resemble self-peptides. Genetic factors may also predispose individuals to specific autoimmune diseases, most notably human leukocyte antigen B27 (HLA-B27) in the case of ankylo

Glycan Activation of Clec4b Induces Reactive Oxygen Species Protecting against Neutrophilia and Arthritis.

Animal models for complex diseases are needed to position and analyze the function of interacting genes. Previous positional cloning identified Ncf1 and Clec4b to be major regulators of arthritis models in rats. Here, we investigate epistasis between Ncf1 and Clec4b, two major regulators of arthritis in rats. We find that Clec4b and Ncf1 exert an additive effect on arthritis given by their joint ability to regulate neutrophils. Both genes are highly expressed in neutrophils, together regulating neutrophil availability and their capacity to generate reactive oxygen species. Using a glycan array

[Reactive arthritis due to Clostridium difficile: Report of two cases and literature review].

INTRODUCTION: Reactive arthritis due to Clostridium difficile infection is uncommon. CASE REPORTS: We report two cases of reactive arthritis associated with C. difficile infection that occurred in a 40-year-old woman and a 47-year-old man. The literature review between 1976 (date of first reported case) and 2016 retrieved 54 observations (including our two cases). Reactive arthritis occurs most frequently in young subjects after a variable time frame according to C. difficile infection, involving one or several articulations. The issue is generally favourable with or without anti-inflammatory

[Haemophilus influenzae type b meningitis in a vaccinated, immunocompetent infant with reactive arthritis].

Due to the excellent immunogenicity of the Haemophilus influenzae type b (Hib) conjugate vaccines, vaccine failures are rarely seen in patients following the recommended national immunization programmes. We present an infant with Hib meningitis despite relevant prophylaxis, without known risk factors such as medical co-morbidity, immunosuppression, immunoglobulin deficiency or prematurity. Later, a reactive arthritis developed. In conclusion, Hib-meningitis can occur in vaccinated, immunocompetent patients, and antibiotics covering Hib should be chosen in patients presenting with meningitis.

High Chlamydia Burden Promotes Tumor Necrosis Factor-Dependent Reactive Arthritis in SKG Mice.

OBJECTIVE: Chlamydia trachomatis is a sexually transmitted obligate intracellular pathogen that causes inflammatory reactive arthritis, spondylitis, psoriasiform dermatitis, and conjunctivitis in some individuals after genital infection. The immunologic basis for this inflammatory response in susceptible hosts is poorly understood. As ZAP-70(W163C) -mutant BALB/c (SKG) mice are susceptible to spondylo-arthritis after systemic exposure to microbial β-glucan, we undertook the present study to compare responses to infection with Chlamydia muridarum in SKG mice and BALB/c mice. METHODS: After gen

Reactive Arthritis Secondary to Cauda Equina Injury following Spine Fracture: A Case Report.

A 38-year-old man presented with a one-month history of muscle weakness and dysesthesia in the lower extremities, urinary retention, and urinary tract infection after lumbar burst fracture resulted from high fall. During the rehabilitation in our hospital, he had arthritis in both the ankle and knee. However, the patient was treated as gouty arthropathy initially. The arthritis was completely remitted in a few days after the patient was diagnosed as reactive arthritis and started with sulfasalazine therapy and there was no recurrence during 4 months of follow-up. Based on this case, early reco

Reactive arthritis after enteric infections in the United States: the problem of definition.

Bacterial enteric infections cause substantial morbidity in the United States both from acute illness and sequelae that follow. Reactive arthritis (ReA) is a poorly defined term that is used to describe a variety of rheumatologic phenomena that may occur after Salmonella, Shigella, Campylobacter, and Yersinia infection, as well as other types of infections (eg, Chlamydia). This review focuses on clinical and epidemiologic investigations of ReA following bacterial enteric infection in the United States. Only 2 population-based studies of ReA following enteric infection have been performed in th

Concurrent reactive arthritis, Graves' disease, and warm autoimmune hemolytic anemia: a case report.

Warm antibody autoimmune hemolytic anemia is due to the presence of warm agglutinins that react with protein antigens on the surface of red blood cells causing premature destruction of circulating red blood cells. We report the first case of concurrent reactive arthritis, Graves' disease, and autoimmune hemolytic anemia. A 40-year-old man with reactive arthritis, Graves' disease, type 2 diabetes mellitus, mitral valve prolapse, and Gilbert's disease presented with a one month history of jaundice, fatigue, and black stools. After diagnosis of warm autoimmune hemolytic anemia, the patient was st

Clinical aspects, outcome assessment, and management of ankylosing spondylitis and postenteric reactive arthritis.

The cause of ankylosing spondylitis remains unclear. Proof that this disorder is an autoimmune disease attributable to crossreactivity between bacteria and HLA-B27 is still lacking. Differences in endogenous peptide presentation by HLA-B27 subtypes might be relevant in the etiopathogenesis. Fractures of the osteoporotic spine contribute to morbidity. Spinal cord injury may occur. MR imaging enables identifying sacroiliitis earlier than plain radiography. Sweet syndrome has now been described in patients with ankylosing spondylitis and Crohn disease. Progress has been made in the assessment of
