Hyperthyroidism and the management of atrial fibrillation.

Atrial fibrillation is often induced in patients with hyperthyroidism and may trigger heart failure. Its prevalence and outcome were examined to obtain up-to-date information. Persistent atrial fibrillation was observed in approximately 1.7% of new hyperthyroid patients. It occurs more frequently in males (2.86%) than in females (1.36%), even though the number of male hyperthyroid patients is only one fifth of female patients. The rate increased with age, being 8% in the patients older than 70 years old. The initial treatment is to control the heart rate with routine pharmacologic therapy and …

Pacing for the prevention of atrial fibrillation.

Nonpharmacological methods are being evaluated for the prevention of atrial fibrillation. The role of atrial pacing has been extensively examined in a large number of retrospective and prospective studies, and recently in randomized multicenter trials. The populations examined have varied from patients with sick sinus syndrome with incidental AF, to patients with drug refractory AF. In addition to standard atrial pacing from the high right atrium, pacing from novel sites like the interatrial septum, Bachman bundle, and dual-site or biatrial pacing have been employed for the prevention of AF. T…

Lone atrial fibrillation in a dog.

An intact male Pointer aged 9 year was found to have atrial fibrillation (AF). Thoracic radiography and echocardiography as well as routine blood work revealed no cardiac disease. Thus, the dog was diagnosed as lone AF. The arrhythmia disappeared 6 hr after the initiation of the supportive therapy, and relapse was not observed.

Atrial fibrillation: are there gender differences?

The incidence of atrial fibrillation is greater in men than in women, but this gap closes with advancing age. More women with atrial fibrillation have underlying valvular disease, and more men with this condition have underlying coronary artery disease. Atrial fibrillation increases mortality and the incidence of stroke in both sexes. However, women in particular (especially those over 75 years old) may be at increased risk for embolism and long-term mortality. Gender is also an important feature affecting the selection of antiarrhythmic drugs for atrial fibrillation, because women are more li…

Epidemiology and classification of atrial fibrillation.

Atrial fibrillation (AF) is a common clinical problem, particularly in the elderly and in patients with organic heart disease. AF generally is classified into paroxysmal and chronic forms. Chronic AF can be the end result of paroxysmal AF in about 30% of patients. Paroxysmal AF can be defined as attacks of arrhythmia lasting < 7 days separated by prolonged periods of sinus rhythm. Chronic AF is AF established for > 7 days. Therefore, the differentiation of paroxysmal from chronic or established AF is based on the history of recurrent episodes and the duration of the current episode of AF. The …

Risk of complications of atrial fibrillation.

Atrial fibrillation is associated with three major risk of complications: thromboembolism, hemodynamic compromise, and arrhythmogenesis. In patients with chronic atrial fibrillation the incidence of embolization is about 5% per year. The risk of embolism and in particular of stroke can be reduced by warfarin anticoagulation. Aspirin is generally less effective than warfarin, although it is probably more effective than placebo. The hemodynamic complications which may occur during atrial fibrillation are mainly due to the loss of effective atrial contraction, the irregular ventricular rhythm, an…

[Epidemiology and therapy of atrial fibrillation].

Atrial fibrillation (AF) is the most common supraventricular arrhythmia in the Western world and its prevalence increases with age. The objectives of the pharmacologic treatment of AF are mainly three: to restore sinus rhythm, to maintain sinus rhythm, and to control ventricular rate during AF. Class 1C antiarrhythmic drugs (propafenone, flecainide) have been demonstrated to be the most effective drugs in restoring sinus rhythm with low risk/benefit and cost/benefit ratios and percentage efficacy rate between 50% and 60%. The long-term efficacy of antiarrhythmic therapy for preventing AF recur…

[Anticoagulation in patients with atrial fibrillation].

Nonvalvular atrial fibrillation is a frequent finding in elderly patients; the risk of thromboembolic complications is comparable to that reported in patients with rheumatic atrial fibrillation. Recently, 6 multicenter clinical trials (5 primary prevention, 1 secondary prevention trail) have been published which demonstrate equivocally that oral anticoagulation therapy significantly reduces the embolic risk in patients with nonvalvular atrial fibrillation by about 67% to 87%. The target INR of anticoagulation with warfarin in 2 of these trials was 1.4 to 2.8 ("low-dose" warfarin); interestingl…

The causes and management of atrial fibrillation.

Atrial fibrillation is present in approximately 10% of the population aged over 65 years, in whom its presence is associated with a significantly increased morbidity and mortality. Principal complications include reduced cardiac output and the precipitation of heart failure and thromboembolic phenomena, including strokes and peripheral emboli. In the majority of cases atrial fibrillation is associated with ischaemic heart-disease and hypertension. Other important causes are thyrotoxicosis, valvular heart-disease and atrial septal defect. The primary aims of treatment in atrial fibrillation are…

The pharmacologic treatment of atrial fibrillation.

The pharmacologic treatment of atrial fibrillation (AF) is aimed at controlling the ventricular response, restoring sinus rhythm, and preventing or delaying relapses. In the control of ventricular response, digitalis maintains a primary role when the arrhythmia is accompanied by heart failure. In ischemic, hypertensive, and degenerative (whose number is increasing at present) cardiopathies without evident ventricular dilatation, treatments with calcium antagonists (such as verapamil, gallopamil, or diltiazem) or beta-blocking agents must be preferred. In order to control the ventricular respon…

[Therapy and prevention of atrial fibrillation].

In the last decade, broader insights into the mechanisms and clinico-prognostic significance of atrial fibrillation have prompted a dramatic shift in the clinician's decision making process in terms of treatment and prevention of this common rhythm disturbance. In fact, evidence of short- or long-term untoward effects on cardiac function and the risk of cerebrovascular accidents, together with a much wider therapeutic armamentarium, form the rationale for the present far more aggressive attitude, aiming at restoring sinus rhythm in all patients in whom an even weak chance exists. The newer cla…

The drug treatment of atrial fibrillation.

1. Atrial fibrillation is an inefficient cardiac rhythm associated with impaired exercise tolerance, exertional dyspnoea, palpitation and a substantial risk of thromboembolism. 2. The first decision in management is to consider cardioversion which can be achieved in suitable cases electrically, or pharmacologically with a class Ic antiarrhythmic drug like flecainide or propafenone. 3. Prophylaxis in paroxysmal atrial fibrillation is best achieved with a class Ic drug or a class III drug such as sotalol or amiodarone. 4. Control of ventricular rate in chronic atrial fibrillation can be achieved…

Stroke in thyrotoxicosis with atrial fibrillation.

Chronic atrial fibrillation is associated with an increased risk of stroke. In elderly patients with thyrotoxicosis, atrial fibrillation is frequently encountered, and the true risk of cerebrovascular events in these patients is controversial. We retrospectively studied 610 patients with initially untreated thyrotoxicosis, 91 (14.9%) of whom had atrial fibrillation, with the highest frequency in the elderly patients. The risk of cerebrovascular events, with special attention to the first year after the diagnosis of thyrotoxicosis, was calculated using logistic regression methods with age, sex,…

The use of encainide in atrial fibrillation.

Based on its electrophysiologic characteristics, encainide, a new type 1 antiarrhythmic agent, was selected for a therapeutic trial in a case of refractory paroxysmal atrial fibrillation. This approach represents an attempt to manage the pathophysiologic disturbance of atrial fibrillation by selection of a drug that would be expected to have specific effects on the conduction system.

Paroxysmal atrial fibrillation in a racehorse.

A 4-year-old thoroughbred stallion with a history of loss of racing form was studied over a period of approximately 7 months. At the initial examination he showed positive T waves in 1 chest lead and wandering of the pacemaker. This was followed by an episode of paroxysmal atrial fibrillation which disappeared spontaneously and was replaced by significant T wave changes in all the chest leads. During a period when these changes persisted, there was progressive lengthening of the P wave and the P-R interval. Because the last ECG showed evidence of intra-atrial block, first degree A-V block and …

Atrial fibrillation and subtype of atrial fibrillation in cardiac amyloidosis: clinical and echocardiographic features, impact on mortality.

Background: Atrial fibrillation (AF) commonly affects patients with cardiac amyloidosis (CA). Amyloid deposition within the left atrium may be responsible for the subtype of AF in either permanent or non-permanent form. The prognostic implications of AF and its clinical subtype according to the type of CA are still controversial in this population. This study sought to investigate the prevalence, incidence and prognostic implications of AF and the clinical subtype of AF (permanent or non-permanent) in patients with CA. Methods: Two hundred and thirty-eight patients with CA and full medical rec…
