Dose requirement of local anaesthetic to produce grand mal seizure during stellate ganglion block.

Two case reports illustrate that low doses of local anaesthetics such as bupivacaine 2.5 mg and a mixture of bupivacaine 1.25 mg and lidocaine 5 mg can induce grand mal seizures if injected into the vertebral artery during stellate ganglion block. The effect of the dose of local anaesthetic agent and technique of administration into the stellate ganglion region discussed as is the relationship between vertebral artery blood flow and cerebral intravascular local anaesthetic concentration required to produce seizure activity. Suggestions are made concerning possible modification of the technique

A single-blind randomized controlled trial of celiac plexus block for analgesia after whipple surgery.

BACKGROUND: Effective management of both incisional and visceral postoperative pain during open abdominal surgery is crucial for patient recovery. This study evaluated the effect of celiac plexus block on postoperative pain and recovery. METHODS: This single-center, patient-assessor-blinded, blank-controlled randomized clinical trial was conducted from March 9, 2022, to November 12, 2023. A total of 78 patients scheduled for open Whipple surgery were randomized. The intervention involved either receiving a celiac plexus block (Group NB) with 20 ml of 0.5% ropivacaine during surgery or not rec

Ultrasound-guided (needle-in-plane) perineural catheter insertion: the effect of catheter-insertion distance on postoperative analgesia.

BACKGROUND: When using ultrasound guidance to place a perineural catheter for a continuous peripheral nerve block, keeping the needle in plane and nerve in short axis results in a perpendicular needle-to-nerve orientation. Many have opined that when placing a perineural catheter via the needle, the acute angle may result in the catheter bypassing the target nerve when advanced beyond the needle tip. Theoretically, greater catheter tip-to-nerve distances result in less local anesthetic-to-nerve contact during the subsequent perineural infusion, leading to inferior analgesia. Although a potentia

[Functional autonomic blockade in the diagnosis of functional sinus node disorders].

The influence of the autonomic nervous system on sinus node automatism was assessed in 61 patients with suspect sinus node dysfunction. Cardiac frequency and corrected sinus node recovery time (CSNRT) were determined before and after functional autonomic blockade with intravenously administered propranolol (0,2 mg/kg) and atropine (0,04 mg/kg). A pathologic CSNRT was found in 59% of patients before and in 54% after autonomic blockade. In 44% pathologic CSNRT occurred during both conditions. In these patients intrinsic sinus node disease can be assumed. In 15% of patients an initially pathologi

Intraoperative Paragastric Vagal Nerve Block With Ropivacaine Reduces Postoperative Nausea and Pain on the Day of Surgery After Laparoscopic Sleeve Gastrectomy: A Retrospective Cohort Study With Propensity Score Matching.

BACKGROUND: Postoperative nausea and visceral gastric pain are major early complications after laparoscopic sleeve gastrectomy (LSG). We evaluated whether intraoperative paragastric infiltration of ropivacaine targeting the anterior vagal trunk reduces postoperative antiemetic and analgesic requirements. METHODS: This retrospective cohort study compared patients who underwent LSG with paragastric vagal nerve block (VNB group; 0.375% ropivacaine 20 mL; n = 123) against a historical control group (n = 159). Propensity score matching (PSM) on sex, BMI, age, and operative time yielded 92 matched

Comparison between radiofrequency ablation and chemical neurolysis of thoracic splanchnic nerves for the management of abdominal cancer pain, randomized trial.

BACKGROUND: Radiofrequency ablation (RFA) of the splanchnic nerves has been reported as a predictable and safe technique for abdominal pain management. We compare between RFA and chemical neurolysis of bilateral thoracic splanchnic nerves in the management of refractory cancer pain. METHODS: The study was conducted on 60 patients aged ≥18 years who suffered from abdominal pain (visceral pain, VAS ≥4) due to upper abdominal cancers. Participants were randomized into two groups. Group I (RF): 30 participants received a bilateral splanchnic nerve block at T10 and T11 levels using RFA. Group II (

Refractory Complex Regional Pain Syndrome: A Case Report and Review of Literature.

INTRODUCTION: Complex regional pain syndrome (CRPS) is characterized by extreme pain in a limb disproportional to the clinical history or physical findings accompanied by the signs of autonomic dysfunction. The pathophysiology of CRPS is obscure, making it challenging to treat. Treatment options include medications, physical therapy, and psychological support. In some cases, surgery or other minimally-invasive procedures such as nerve blocks may be recommended, while several novel treatments, such as ozone therapy, lack sufficient clinical evidence. CASE PRESENTATION: A 40-year-old man with C

4-aminopyridine-a review.

4-aminopyridine is the first of the aminopyridines to be used in clinical practice. It blocks potassium channels and thereby increases acetylcholine, and possibly noradrenaline, release at nerve terminals. In man the drug has a significant action at the neuromuscular junction, but has little effect on the autonomic nervous system or muscle (smooth, skeletal, or cardiac) although such actions have been demonstrated in animals. It may be clinically useful in the reversal of nondepolariser blockade and a role in antibiotic associated block has been proposed. It may be used effectively as an anale

Visceral afferents: their contribution to 'sympathetic dependent' pain.

For 80 years the sympathetic nervous system has been implicated in certain human pains, notably causalgia and reflex sympathetic dystrophy. This assumption has led to the accepted concept of 'sympathetic dependent' pain. In this critical review, the evidence for this assumption is assessed. It is found that the clinical phenomena suggesting sympathetic nerve involvement may be more satisfactorily attributed to effects of neuropeptides released from afferent C-fibres. The evidence also demonstrates that the effects resembling sympathetic dysfunction relate neither to the pain nor to pain relief

Effect of 5% lidocaine medicated plaster on pain intensity and paroxysms in classical trigeminal neuralgia.

OBJECTIVE: Trigeminal neuralgia (TN) is a neuropathic pain condition affecting one or more branches of the trigeminal nerve. It is characterized by unilateral, sudden, shock-like, and brief painful attacks, which follow the distribution of trigeminal nerve branches, and with no other accompanying sensorimotor or autonomic signs and symptoms. Current guidelines stipulate which therapies represent first-, second-, and third-line treatments for TN, but there is a consistent mismatch between the therapeutic guidelines and the patient's preferences and expectations. CASE SUMMARY: We report on 2 pa

The efficacy of Gasserian ganglion block and pulsed radiofrequency in the treatment of chronic cluster headache: A retrospective cohort study.

BACKGROUND AND PURPOSE: To assess the effectiveness of Gasserian ganglion block and pulsed radiofrequency in managing chronic cluster headache (CCH). METHODS: We enrolled 38 patients with treatment-resistant CCH who were admitted between March 2020 and January 2024 and underwent Gasserian ganglion pulsed radiofrequency (PRF) combined with a ganglion block. Patients were followed up for six months, with pain severity assessed using the 0-10 Numerical Pain Rating Scale (NPRS). RESULTS: Among 38 patients (mean age 30.8 ± 8.6 years), the mean headache duration before treatment was 29.72 ± 17.65

Nerve growth factor prevents neurotoxic effects of cisplatin, vincristine and taxol, on adult rat sympathetic ganglion explants in vitro.

Anti-cancer drugs, cisplatin, vincristine and taxol clinically induce toxic sensory as well as autonomic neuropathy. Administration of nerve growth factor (NGF) has been found to prevent experimental sensory neuropathies induced by these anti-cancer drugs, but the information about autonomic neuropathy is lacking. We developed an adult rat superior cervical ganglion (SCG) explant culture, which we treated with cisplatin, vincristine and taxol either in the presence or absence of NGF. The maximum length of regenerated neurites was shortened by cisplatin, vincristine and taxol in a dose-dependen

Pre- and postsynaptic effects of nicotine on the mouse phrenic nerve-diaphragm preparation.

Nicotine at less than or equal to 33 microM enhanced the single twitch response to indirect stimulation but potentiated the blocking effect of tubocurarine. Failure of tetanic contraction (tetanic fade) occurred on stimulation at 100 Hz. At 76 microM, nicotine induced a first phase rapid (10 min) inhibition of twitch response followed later (60-90 min) by a second phase complete block. Neostigmine partially restored the response at either phase of block whereas diaminopyridine completely antagonized the blockade. The end-plate was depolarized maximally by only 10-15 mV within 30 min with 43 mi

Thoracic pressure and nasal patency.

Maximum nasal flow rate in the right and left nostrils was simultaneously determined during expiration with the help of two flowmeters in 10 healthy subjects in different postures and in two patients, one with Horner's syndrome and the other with facial palsy. It was found that pressure on the hemithorax from any surface (i.e., lateral, anterior, posterior, or superior) leads to reduced patency of the ipsilateral nostril but increased patency of the nostril on the opposite site. In the patient with Horner's syndrome, the nostril on the affected side remained blocked even on compression of the
