Rapid onset of severe heat illness: a case report.

Aviators flying extended periods in hot environments are known to be at risk for heat-related illness. The risk when wearing chemical individual protective equipment (IPE) is increased even at relatively warm temperatures and light workloads. In this paper, we report the physiological responses of an aviator who had been flying a UH-1H helicopter up to 6 h/d clothed in full IPE on 6 consecutive days prior to the sudden onset of heat illness. His performance during the study was normal, and no clear physiological derangements were noted prior to his symptoms. The rapid evolution of his symptoms

From Wooden Mock-Ups to Hybrid Ecosystems: A Systems Architecture Approach to Next-Generation Helicopter emergency medical services/Search and Rescue Crew Training.

OBJECTIVE: Helicopter emergency medical services (HEMS) and search and rescue operations represent the intersection of aviation complexity and clinical criticality. Despite technological advances in simulation, most training programs remain fragmented across aviation and medical domains, failing to integrate the human factors that determine mission success. This program report describes the longitudinal development of an integrated HEMS simulation training system, from early low-fidelity cabin prototypes to contemporary multi-sensory and hybrid simulation environments, and proposes a systems a…

National emissions inventory and future trends in greenhouse gases and other air pollutants from civil airports in China.

Civil aviation is an important source of air pollutants, but this field has received insufficient attention in China. Based on the standard emissions model of the International Civil Aviation Organization (ICAO) and actual flight information from 241 airports, this study estimated a comprehensive emissions inventory for 2010-2020 by considering the impacts of mixing layer height. The results showed that annual pollutant emissions rapidly trended upward along with population and economic growth; however, the emissions decreased owing to the impacts of the COVID-19 pandemic. In 2020, the emissio…

Fatal light aircraft accidents in Ontario: a five year study.

Fatal civil aviation crashes in Ontario from 1985 to 1989 were studied. Data regarding accident circumstances, injury patterns and medical factors (disease, alcohol/drugs) which could have contributed to accident causation was obtained from a review of the files of the Chief Coroner for Ontario in Toronto and the aviation occurrence reports of the Transportation Safety Board of Canada. Forty-seven crashes involving mainly general aviation type aircraft but also 2 gyroplanes, 2 ultralights and a glider were reviewed. About half occurred during the cruise phase of the flight. Seventy (40 pilots;…

Bilateral Pneumothoraces in a Tandem Parachuting Passenger Without Traumatic Impact: A Case Report.

In parachuting, orthopedic and head injuries are well-documented risks associated with the parachute deployment and landing phases. Thoracic injuries have only been seen on rare occasion in conjunction with direct impact trauma. In this report, we detail a case of a young, healthy, tandem skydiving passenger who suffered bilateral pneumothoraces with delayed symptom onset, with no identifiable injury during the jump or landing. Exploring the forces of the parachute "opening shock," we suggest a plausible compressive mechanism for this novel presentation, as well as briefly discuss the options

Acute exposure to altitude.

Acute exposure to altitude principally encompasses aviation and space activities. These environments can be associated with very acute changes in pressure, oxygenation and temperature due to rates and magnitude of ascent that are not experienced in more chronic exposure such as mountaineering. The four key physiological challenges during acute exposure to altitude are: hypoxia (and hyperventilation), gas volume changes, decompression sickness and cold. The brief nature of aviation exposure to altitude provides little opportunity for acclimatisation, leading to markedly different effects when a…

Aortic valve replacement in a United States Air Force pilot: case report and literature review.

We report the first case of a United States Air Force pilot returned to flying status after cardiac valve replacement. A 34-yr-old C-130 pilot underwent placement of an aortic valve homograft as a result of an episode of acute bacterial endocarditis on a bicuspid aortic valve. After a period of observation and a careful evaluation, he was returned to flying status in low-G aircraft. While cardiac valve replacement has traditionally been considered disqualifying for military aviation duties, aortic valve homografts have been associated with a lower morbidity and a significantly improved prognos…

Assessing for preflight predictors of airsickness.

A total of 57 randomly selected male and female student pilots from an Introduction to Aviation private pilot course in a large aviation program of a midwestern university completed this study. These pilots were assessed for health status and surveyed for airsickness and associated symptoms by use of an Airsickness Inventory Questionnaire developed by the authors and a modified Self-Rating Anxiety Status Inventory Scale tool developed by W. K. Zung. Correlational analyses of scores from self-report inventories were used to evaluate strength of relationships between airsickness and anxiety. Mul…

Freon: an unsuspected problem.

Freon 113 is not usually considered a toxic chemical and is not generally included in toxicology text lists of dangerous chemicals. However, during one 7-year period, the U.S. Navy recorded 38 freon-related mishaps, including 16 chemical burns and 22 inhalation injuries. Several deaths were also recorded. Part of the problem seems to be that freon compounds are excellent degreasing and cleaning agents. Consequently, they are widely and inappropriately used by aviation ground crews for this purpose. Such use usually presents no problem, except when the agent is used in small, closed, unventilat…

Characterizing Aircraft Exhaust Emissions and Impact Factors at Tianjin Binhai International Airport via Open-Path Fourier-Transform Infrared Spectrometer.

The growth of the civil aviation industry has raised concerns about the impact of airport emissions on human health and the environment. The aim of this study was to quantify the emissions of sulfur dioxide (SO2), nitrogen oxides (NOX), and carbon monoxide (CO) from in-service aircraft via open-path Fourier-transform infrared (OP-FTIR) spectroscopy at Tianjin Binhai International Airport. The results suggest that the CO and NOX emission indices (EIs) for five common aircraft/engine combinations exhibited substantial discrepancies from those reported in the International Civil Aviation Organiza…

Functional Dyspepsia and Tricyclic Antidepressant Use in a Naval Flight Officer.

BACKGROUND: Functional dyspepsia is a disorder of gut-brain interaction that has the potential to impact aviation performance. Proton pump inhibitors are well-tolerated but are only effective in one half of cases. Second-line treatments, including tricyclic antidepressants, are associated with drowsiness and are not routinely approved for use in aviators. We present a case of a Naval Flight Officer with functional dyspepsia who was successfully treated with amitriptyline and returned to flying status.CASE REPORT: A 23-yr-old male Naval Flight Officer presented with postprandial fullness and ep…

Cervical Manipulation Leading to Cerebellar Stroke in a Pilot.

BACKGROUND: Stroke is a decidedly devastating event for any patient, but particularly for a military aviator in a single-seat aircraft. Incidence of acute ischemic infarct in men ages 25 to 29 ranges from 3.4 to 5.6/100,000. The neurological sequelae of stroke can have a lasting and profound impact on an aviator's career. Literature review revealed a relatively small number of cases where stroke was attributable to cervical manipulation. CASE REPORT: A 29-yr-old male jet pilot with a 2-wk history of cervicalgia following a mountain bike ride performed self-manipulation of his neck at home fol

A survey of pre-placement urinalysis drug findings.

In December, 1989, the Department of Transportation (DOT) in conjunction with the Federal Aviation Administration (FAA) mandated an extensive urine drug testing program for selected positions within the airline industry. At the end of 1 year we have tested 7,872 applicants under these rules, with a positive finding rate of 0.17%. We have also tested 32,157 applicants, including those applying for DOT-covered positions, with a positive rate of 2.82%. Considering only the two major drugs of abuse--marijuana and cocaine--we found the positive rate to be an order of magnitude greater than the rate…

Changing the role of the air traffic controller: how will free flight affect memory for spatial events?

At present, air traffic controllers (ATCOs) exercise strict control over routing authority for aircraft movement in airspace. The onset of a free flight environment, however, may well result in a dramatic change to airspace jurisdictions, with aircraft movements for the large part being governed by aircrew, not ATCOs. The present study examined the impact of such changes on spatial memory for recent and non-recent locations of aircraft represented on a visual display. The experiment contrasted present conditions, in which permission for manoeuvres is granted by ATCOs, with potential free fligh

Return to flight with multiple sclerosis: aeromedical considerations.

Multiple sclerosis (MS) is the most frequent demyelinating disease of the central nervous system, with versatile manifestations--relapsing-remitting or progressive--and an unpredictable course, with prognoses ranging from minimal neurological impairment to severely disabled. Disease modifying agents can minimize relapse rate and slow disease progression. Yet most patients suffer relapses and progression despite use of these agents. Several of the manifestations of MS may cause overall decrease in the performance of the aviator. These include cognitive impairment, fatigue, and depression. Episo

Profiles in patient safety: authority gradients in medical error.

The term "authority gradient" was first defined in aviation when it was noted that pilots and copilots may not communicate effectively in stressful situations if there is a significant difference in their experience, perceived expertise, or authority. A number of unintentional aviation, aerospace, and industrial incidents have been attributed, in part, to authority gradients. The concept of authority gradient was introduced to medicine in the Institute of Medicine report To Err Is Human, yet little has been written or acknowledged in the medical literature regarding its role in medical error. …

An exploration of the utility of mathematical modeling predicting fatigue from sleep/wake history and circadian phase applied in accident analysis and prevention: the crash of Comair Flight 5191.

On 27 August 2006 at 0606 eastern daylight time (EDT) at Bluegrass Airport in Lexington, KY (LEX), the flight crew of Comair Flight 5191 inadvertently attempted to take off from a general aviation runway too short for their aircraft. The aircraft crashed killing 49 of the 50 people on board. To better understand this accident and to aid in preventing similar accidents, we applied mathematical modeling predicting fatigue-related degradation in performance for the Air Traffic Controller on-duty at the time of the crash. To provide the necessary input to the model, we attempted to estimate circad…
