Diagnosing high-grade pancreatic intraepithelial neoplasia in surgically altered anatomy using pancreatic juice cytology.

BACKGROUND AND AIMS: Diagnosing early-stage pancreatic ductal adenocarcinoma, particularly high-grade pancreatic intraepithelial neoplasia (HG-PanIN), remains challenging. Serial pancreatic juice aspiration cytologic examination (SPACE) using an endoscopic nasopancreatic drainage tube has demonstrated high diagnostic accuracy, but its application in surgically altered anatomy is technically demanding. We present a case in which balloon enteroscopy-assisted SPACE led to the diagnosis of HG-PanIN and successful resection. METHODS: A 70-year-old man with a history of distal gastrectomy and Roux-…

Use of enteroscopy for the detection of malignant and premalignant lesions of the small bowel in complicated celiac disease: a meta-analysis.

BACKGROUND AND AIMS: Enteroscopy (wireless or wired) is the reference standard for small-bowel (SB) diseases, and it has been applied to detect SB malignancies in complicated celiac disease (CD) with heterogeneous results. The aim of this meta-analysis was to obtain a diagnostic yield (DY) by pooling the data of studies that investigated the use of enteroscopy to detect SB adverse events in CD. METHODS: We performed an online search for studies estimating the DY of wireless and wired enteroscopy in predicting the presence of SB premalignant and/or malignant lesions. The DerSimonian and Laird

Protein C deficiency related obscure gastrointestinal bleeding treated by enteroscopy and anticoagulant therapy.

Obscure gastrointestinal bleeding is an uncommonly encountered and difficult-to-treat clinical problem in gastroenterology, but advancements in endoscopic and radiologic imaging modalities allow for greater accuracy in diagnosing obscure gastrointestinal bleeding. Ectopic varices account for less than 5% of all variceal bleeding cases, and jejunal variceal bleeding due to extrahepatic portal hypertension is rare. We present a 47-year-old man suffering from obscure gastrointestinal bleeding. Computed tomography of the abdomen revealed multiple vascular tufts around the proximal jejunum but no e

Management of Biliopancreatic Limb Bleeding after Roux-en-Y Gastric Bypass: A Case Report.

The Roux-en-Y gastric bypass is one of the most extensive surgical treatments for obesity. The treatment of upper gastrointestinal bleeding after Roux-en-Y gastric bypass is complex due to the difficulty of accessing the excluded gastric antrum and duodenal bulb. There is no consensus regarding the management of this complication. While various techniques have been described to access the biliopancreatic limb, double-balloon enteroscopy is the most commonly used. If double-balloon enteroscopy is unavailable, a pediatric colonoscope may be used as an alternative; however, its use in such cases …

Endoscopy in inflammatory bowel disease.

Small bowel imaging and endoscopy in inflammatory bowel disease (IBD) underwent a lot of change and advancement in the recent years. Modalities have shifted from gastroscopy, colonoscopy and small bowel follow through, to ileo-colonoscopy, computed tomography (CT) or magnetic resonance (MR), enteroscopy, wireless video capsule endoscopy and balloon assisted enteroscopy. Nowadays endoscopy has a major role in the diagnosis of IBD, assessing its extent, treating some of its complications (stricture, bleeding), assessing the success of various treatments (mucosal healing), and as a predictor of d

Video push enteroscopy in the investigation of small bowel disease: defining clinical indications and outcomes.

BACKGROUND: Push enteroscopy is a new technique for investigation of the small intestine. The clinical indications are still being defined. It also offers the potential for therapeutic intervention in suitable cases. AIMS: To evaluate further the role of push enteroscopy in the diagnosis and treatment of patients with suspected or known small bowel disease. METHODS: A prospective record was kept of all patients having enteroscopy at Royal Prince Alfred Hospital between March 1995 and July 1997. The procedure was performed 73 times in 68 patients. Indications and diagnoses were noted. The out

Small bowel tumors.

Although rare, small bowel tumors may cause significant morbidity and mortality if left undetected. New endoscopic modalities allow full examination of the small bowel with improved diagnosis. However, isolated mass lesions may be missed by capsule endoscopy or incomplete balloon-assisted enteroscopy. Therefore the use of radiologic imaging and intraoperative enteroscopy for diagnosis should not be forgotten. Endoscopic resection of small bowel polyps and certain vascular tumors is possible but requires proper training. Advances in endoscopic tools are likely to broaden the endoscopic manageme

Patient-reported experience in device-assisted enteroscopy: Conscious sedation vs. general anesthesia.

BACKGROUND AND STUDY AIMS: Device-assisted enteroscopy (DAE) plays a critical role in evaluation and treatment of small bowel disorders, with double-balloon enteroscopy (DBE) being the most widely used technique. Sedation strategies for enteroscopy remain a topic of debate, with conscious sedation (CS) and general anesthesia (GA) both being utilized. Our aim was to assess patient-reported experience measures (PREMs) for DAE, comparing CS and GA to determine their impact on patient satisfaction, procedure success, and pain perception. PATIENTS AND METHODS: PREMs were prospectively collected fr…

Comparison of magnetic resonance and balloon enteroscopic examination of the small intestine in patients with Crohn's disease.

BACKGROUND & AIMS: Magnetic resonance (MR) enterography is a recommended imaging technique for detecting intestinal involvement in Crohn's disease (CD). However, the diagnostic accuracy of MR enterography has not been compared directly what that of enteroscopy of the jejunum and proximal ileum. We evaluated the usefulness of MR enterocolonography (MREC) by comparing its findings with those from balloon-assisted enteroscopy. METHODS: In a prospective study, MREC and enteroscopy were performed within 3 days of each other on 100 patients. Ulcerative lesions and all mucosal lesions were evaluated

Current insights into the diagnosis and management of small bowel bleeding.

Small bowel bleeding accounts for 5-10% of gastrointestinal (GI) hemorrhage and remains a diagnostic challenge because of its diverse etiologies and subtle clinical presentation. Advances in small bowel imaging have refined the diagnostic approach and have led to the replacement of the term obscure GI bleeding with suspected small bowel bleeding (SSBB) when findings on upper and lower endoscopy are unrevealing. The initial evaluation requires careful assessment of clinical history, comorbidities, medication use, and physical findings, which may help direct suspicion toward specific etiologies

[A Case of Primary Small Intestinal Cancer Diagnosed by Double-Balloon Endoscopy and Treated with Single-Incision Laparoscopic Surgery].

A 73-year-old woman was admitted to our hospital with a 5 months history of epigastric discomfort. Abdominal CT performed on admission revealed localized dilation of the small intestine. The patient was diagnosed with subileus and treatment was initiated with fasting and intravenous fluids. An upper gastrointestinal series revealed an apple core sign in the upper jejunum. Double-balloon enteroscopy identified a type 2 tumor in the upper jejunum, which was marked with ink. A biopsy of the tumor revealed a Group 5 tumor, with no distant metastasis. The patient was diagnosed with primary small in…
