The use of antidepressants in bipolar disorder.

BACKGROUND: Whether or not to use antidepressants in patients with bipolar disorder is a matter of debate. Antidepressant treatment of bipolar depression has been associated with manic switch and cycle acceleration. Furthermore, recent studies have argued against the efficacy of antidepressants in the treatment of bipolar depression. Nevertheless, many clinicians continue to employ antidepressants, especially in the management of severe depression that is unresponsive to mood stabilizers alone. OBJECTIVE: Because of the unclear risk-to-benefit ratio of antidepressants in bipolar disorder, we

Incorporating trial data into bipolar disorder management.

Evidence-based medicine integrates data from clinical trials into clinical care while efficacy studies assess simplified interventions in simplified populations. The findings of such studies are useful in obtaining Food and Drug Administration approval of medications but often fail to generalize to the more complex populations and interventions commonly seen in clinical practice. Recently, an increased emphasis has been placed on effectiveness studies (also referred to as pragmatic or practical studies) designed to yield findings that can be generalized to the clinical management of patients w

Introduction to bipolar disorder and comorbid conditions.

Bipolar disorder presents with symptoms that overlap with other, often comorbid, psychiatric disorders, such as borderline personality disorder, substance use disorder, and unipolar depression, and also often co-occurs with medical disorders. Appropriate screening for manic and hypomanic symptoms may lessen the frequency of misdiagnosis of bipolar disorder as unipolar depression, which has serious treatment implications. Further, following the appropriate guidelines for monitoring the physical health of patients with bipolar disorder, especially those treated with mood stabilizers and atypical

Lamotrigine in bipolar disorder: efficacy during pregnancy.

OBJECTIVE: Clinical management of bipolar disorder (BPD) patients during pregnancy is a major challenge. The high risk of bipolar depression during pregnancy encourages consideration of lamotrigine (LTG). We therefore compared recurrence risks among pregnant women with BPD treated with LTG to those discontinuing mood stabilizer therapies. METHODS: We compared risks and weeks to new DSM-IV illness-episodes among 26 initially clinically stable pregnant women diagnosed with DSM-IV BPD who continued LTG treatment to those discontinuing all mood stabilizer treatment during pregnancy. RESULTS: The

Predictors of trait aggression in bipolar disorder.

OBJECTIVES: Although aggressive behavior has been associated with bipolar disorder (BD), it has also been linked with developmental factors and disorders frequently found to be comorbid with BD, making it unclear whether or not it represents an underlying biological disturbance intrinsic to bipolar illness. We therefore sought to identify predictors of trait aggression in a sample of adults with BD. METHODS: Subjects were 100 bipolar I (n = 73) or II (n = 27) patients consecutively evaluated in the Bipolar Disorders Research Program of the New York Presbyterian Hospital-Payne Whitney Clinic.

Preschool Bipolar Disorder: Brazilian children case reports.

OBJECTIVE: This study describes the clinical phenomenology and family history of preschool age onset Bipolar Disorder (BD). METHODS: Eight children and adolescents out of 118 cases (6.78%), both genders, meeting current DSM-IV criteria diagnosis of BD were described. The clinical assessment, CBCL, DICA-IV and CGAS were performed directly with each patient and their parents. RESULTS: Most (87.5%) presented classical symptoms of mania: euphoria, grandiosity, irritability, psychomotor agitation and agitated sleep or, in the same proportion, sleeplessness. Hyperactivity and increase of energy we

[Anticonvulsants in the treatment of bipolar disorder].

Starting with carbamazepine and valproate in the eighties, several anticonvulsant have been established as a treatment option in bipolar disorder. They may constitute an alternative to lithium for prophylactic treatment as well as to classical and atypical antipsychotics for the treatment of acute mania. Only for the acute treatment of bipolar depression they could not demonstrate convincing results so far. However, not every anticonvulsant is at the same time a mood stabilizer; there are clear differences in efficacy and tolerability also within the group of anticonvulsants used in bipolar di

Determinants of social functioning in bipolar disorder.

BACKGROUND: Evidence indicates that social outcomes in bipolar disorders (BP) are poorer than previously assumed. This study explores determinants of the quality and quantity of social activity. METHODS: Seventy-seven subjects with BP completed self-report and observer-rated questionnaires assessing demographic and clinical factors, personality and social functioning. RESULTS: Quality of functioning and spectrum of activity were found to be independent. The best determinants of the quality of functioning, accounting for 33% of the variance, were premorbid neuroticism and the current level of

Quetiapine for acute mania in bipolar disorder.

PURPOSE: The efficacy and tolerability of quetiapine in the treatment of acute mania were reviewed. SUMMARY: Five randomized, placebo-controlled trials involving quetiapine as monotherapy or adjunct therapy in combination with either divalproex or lithium in the treatment of bipolar mania in either adolescents or adults were identified and reviewed. The primary outcome measure used in the trials was a change in Young Mania Rating Scale total scores. Monotherapy trials evaluated quetiapine, lithium, haloperidol, and placebo. Quetiapine was superior to placebo in both trials. Quetiapine and lit

Genome-wide scan of bipolar II disorder.

OBJECTIVE: Bipolar disorder (BD) II is characterized by recurrent hypomanic and depressive episodes and has been somewhat of a controversial diagnosis since its description in the 1970s. Clinical opinions notwithstanding, the biological validity of BD II was supported in a genetic study of 58 multiplex bipolar families wherein the statistical evidence for linkage derived from BD II sibling-pairs sharing marker alleles on chromosome 18q. The BD II phenotype alone has never been studied in a genome-wide scan analysis in the current or other bipolar family samples. We have performed genome-wide n

Concurrent medical conditions with pediatric bipolar disorder.

PURPOSE OF REVIEW: Pediatric bipolar disorder is a serious mental illness with significant morbidity and mortality. A variety of medical and psychiatric conditions occur concurrently with bipolar disorder. These conditions have been more frequently reported in adults. There prevalence in pediatric bipolar disorder is less known. This report is particularly relevant and timely due to the chronic nature of bipolar disorder and the profound impact on health that its treatments can bring. This evolving area needs to be understood to maximize clinical outcomes. RECENT FINDINGS: While little has be

Lithium in maintenance therapy for bipolar disorder.

Lithium has been used for the medical treatment of bipolar disorder for over 50 years. The purpose of this paper is to review the evidence base supporting the use of lithium in the maintenance phase of treatment of bipolar disorder and the limitations which reduce the clinical effectiveness of this medication. A selective review of the relevant literature was carried out. It is concluded that lithium is efficacious in the maintenance phase treatment of bipolar disorder, with the available evidence supporting a beneficial effect which prevents recurrence of mania; it is less clear if lithium pr

A review of bipolar disorder in adults.

OBJECTIVE: This article reviews the epidemiology, etiology, assessment, and management of bipolar disorder. Special attention is paid to factors that complicate treatment, including nonadherence, comorbid disorders, mixed mania, and depression. METHODS: A Medline search was conducted from January of 1990 through December of 2005 using key terms of bipolar disorder, diagnosis, and treatment. Papers selected for further review included those published in English in peer-reviewed journals, with preference for articles based on randomized, controlled trials and consensus guidelines. Striatopallidal regulation of affect in bipolar disorder.

BACKGROUND: Evidence from the neuroimaging literature suggests that the basal ganglia plays an important role in the regulation of affect. This conclusion stems almost exclusively from group comparisons and it remains unclear whether previous findings can be confirmed from a longitudinal study of mood change. The aim of this study was to increase our understanding of the functional role of the basal ganglia and thalamus in relation to change in affect in patients with bipolar disorder. METHODS: Ten bipolar disorder subjects participated in a functional MRI study. We used a simple motor reacti

[Use of antiepileptic drugs in bipolar disorder].

OBJECTIVE: Bipolar disorder is a chronic disease difficult to treat that generates a high degree of incapacity. Although lithium remains the first choice drug, some patients do not respond and others show adverse reactions. One alternative to lithium is the use of certain antiepileptic drugs. Data on the efficacy of old and new antiepileptic drugs in bipolar disorder obtained in controlled clinical trials are reviewed. DEVELOPMENT: Results in many clinical trial support the use of some old antiepileptic drugs such as carbamazepine and sodium valproate in monotherapy in the acute treatment of

[Mixed episodes in bipolar disorder: a review].

Affective mixed states in bipolar disorders are a current matter of scientific debate and represent a complex clinical picture with coexisting manic and depressive symptoms. Treatment of mixed states is regarded as an important challenge. As diagnostic uncertainties complicate systematic clinical evaluations of this patient group, generally accepted clinical treatment guidelines are lacking yet. In this review the significance as well as the problems and risks of new treatment options are discussed.

Atypical antipsychotics: efficacy across bipolar disorder subpopulations.

Atypical antipsychotic medications, used as monotherapy or as adjunctive therapy with mood stabilizers, have shown efficacy and tolerability for 4 subpopulations of patients with bipolar disorder: patients with mixed mania, patients with psychotic episodes, children and adolescents, and the elderly. Patients experiencing mixed mania generally respond poorly to lithium therapy and are more difficult to treat than patients with pure mania. Atypical antipsychotics are increasingly being considered for this bipolar subpopulation because of their efficacy as antimanic agents, and because they are l

Diagnosing and treating comorbid (complicated) bipolar disorder.

Comorbidity is the rule, not the exception, in bipolar disorder. The most common mental disorders that co-occur with bipolar disorder in community studies include anxiety, substance use, and conduct disorders. Disorders of eating, sexual behavior, attention-deficit/hyperactivity, and impulse control, as well as autism spectrum disorders and Tourette's disorder, co-occur with bipolar disorder in clinical samples. The most common general medical comorbidities are migraine, thyroid illness, obesity, type II diabetes, and cardiovascular disease. Bipolarity is a marker for comorbidity, and comorbid
