Lipid Profiles in Lyme Borreliosis: A Potential Role for Apheresis?

Dyslipidemia and dyslipoproteinemia are common causes of metabolic and cardiovascular diseases. On the other hand, intracellular bacteria, such as Borrelia burgdorferi, utilize host lipids to survive and disseminate within the host. Recent data suggest that elevated lipids are a contributing factor to the maintenance and severity of Lyme disease and its complications. Here we review and discuss the role of lipids in Borreliosis and report on a pilot trial to examine the potential roles of circulating lipids and lipoproteins in patients with Borrelia infection. In this analysis we assessed the

[The role of PCSK9-inhibitors and of lipoprotein apheresis in the treatment of homozygous and severe heterozygous familial hypercholesterolemia: A rivalry, or are things quite different?].

PCSK9-inhibitors belong to the new class of hypolipidemic agents. They enhance catabolism of low density lipoprotein cholesterol (LDL-C) through inhibiting activity of proprotein convertase subtilisin/kexin type 9 (PCSK9). They are monoclonal antibodies (alirocumab, evolocumab etc). Under clinical development are also other types of PCSK9-inhibitors which act at a subcellular level. The treatment with PCSK9-inhibitors can be beneficially combined with lipoprotein apheresis (LA). If such treatment using PCSK9-inhibitors is possible with regard to an individual patients genotype, the combination

Efficacy and safety of granulocyte adsorption apheresis in elderly patients with ulcerative colitis.

BACKGROUND: Elderly ulcerative colitis (UC) is increasing. Elderly UC differ from younger UC with respect to the course of their disease. Granulocyte adsorption apheresis (CAP) is often used to treat elderly UC. We retrospectively analyzed the cases of elderly UC who underwent CAP for remission induction therapy in a comparison with younger UC. METHODS: 96 patients with UC underwent CAP. Patients who concurrently received tacrolimus, biological agents, or high-dose steroid therapy were excluded. The remaining 80 patients were evaluated. We divided them into an elderly group (aged ≥65 years) a

Long-term strategies for the treatment of Refsum's disease using therapeutic apheresis.

Refsum's disease is a rare autosomal recessive disorder of fatty acid metabolism. Poorly metabolized phytanic acid accumulates in fatty tissues, including myelin sheaths and internal organs, leading to retinitis pigmentosa, peripheral polyneuropathy, cerebellar ataxia, and renal, cardiac or liver impairment. Dietary restriction of phytanic acid in some cases is not sufficient to prevent acute attacks and stabilize the progressive course. Phytanic acid bound to large low density lipoproteins (LDL) and very low density lipoproteins (VLDL) molecules offers the possibility of extracorporeal elimin

Plasma exchange and immunoadsorption for autoimmune neurologic diseases - current guidelines and future perspectives.

There is increasing interest from neurologists to use therapeutic apheresis in autoimmune neurologic diseases due to growing knowledge of pathogenic relevance of autoantibodies. Developments in that field have been summarized in this review focusing on German guidelines and recent results from clinical research. Therapeutic apheresis can offer a therapeutic armamentarium with rapid response for severe acute neurologic symptoms, and a drug-free option for clinical courses being refractory to drug based strategies or complicated by drug side effects. Plasma exchange (PE) as the classical method

Efficacy of granulocyte and monocyte adsorption apheresis for three cases of refractory pyoderma gangrenosum.

Pyoderma gangrenosum presents with chronic skin ulcers and is histologically characterized by neutrophil infiltration throughout the dermis. It is also occasionally associated with ulcerative colitis, a type of inflammatory bowel disease, against which granulocyte and monocyte adsorption apheresis (GCAP) has recently shown remarkable efficacy. We performed GCAP on three refractory cases of pyoderma gangrenosum with painful bilateral leg ulcers and hereby report the results obtained. Patient 1 was a 43-year-old woman with a four-year history of recurrent painful skin ulcers treated with prednis

Factors for PBPC collection efficiency and collection predictors.

Factors affecting collection efficiency of peripheral blood stem cells (PBSCs) include patient's age, diagnosis, preceding chemoradiotherapy, disease invasion of the bone marrow and mobilizing chemotherapy in PBSC collection for autologous transplants. Mobilizing cytokines, timing for apheresis, machines and operating software would affect mobilization and collection of PBSCs both for autologous and allogeneic transplantation. Also donor's age and gender would affect PBSC yield for allogeneic transplantation. Surrogate markers including peripheral blood CD34+ cell counts before mobilization an

Preparation of a superantigen-adsorbing device and its superantigen removal efficacies in vitro and in vivo.

OBJECTIVE: A new superantigen-adsorbing device (SAAD) was developed, and its characteristics and efficacy in septic animals were evaluated. METHODS: The SAAD was prepared by stepwise chemical modification of a polystyrene-based composite fiber reinforced with polypropylene. Adsorption affinities for several factors and the biological effect of superantigen (SAg) removal were measured in vitro. Also, superantigen-infused rabbits were treated with SAAD, and the efficacy was evaluated in vivo. RESULTS: When the SAAD was evaluated for its ability to adsorb SAg in human plasma (1 ng/mL each), the

Hemodynamic Effect of Different Replacement Fluid Protocols During Therapeutic Apheresis Evaluated With CRIT-LINE.

In therapeutic apheresis (TA) procedures, it is essential to administrate human albumin solution (HAS) to prevent hypotensive episodes. The CRIT-LINE monitor is used in hemodialysis to estimate changes in relative blood volume (RBV) to prevent such hypotensive episodes. The aim of this study was to evaluate the change in RBV during therapeutic plasma exchange (TPE) and double filtration plasmapheresis (DFPP) according to different fluid replacement protocols in order to identify the best protocol. A retrospective case series study was conducted on three patients with CIDP at a tertiary center.

Mitochondrial DNA levels in plasma products: A comparative study of apheresis plasma from COVID-19 convalescent donors and apheresis and fresh frozen plasma from non-infected donors.

BACKGROUND: Cellular and mitochondrial stress can trigger the extracellular release of mitochondrial DNA (mtDNA) as damage-associated molecular patterns (DAMPs), which may amplify inflammatory responses and contribute to transfusion-related adverse events. This study aimed to quantify mtDNA levels in plasma products, assess the effects of storage duration and preparation method (apheresis vs. FFP). STUDY DESIGN AND METHODS: A cross-sectional analysis was conducted from March 2022 to March 2023. Plasma samples included 16 units of fresh frozen plasma (FFP) obtained via whole blood centrifugati

Head-To-Head Intra Individual Comparison of Three Therapeutic Apheresis Modalities in Chronic Auto-Immune Neuropathy: A Real-World Experience.

Therapeutic apheresis (TA) is effective in autoimmune neurological disorders and is increasingly used due to intravenous immunoglobulin (IVIg) shortages. Although three modalities exist-membrane-based therapeutic plasma exchange (mTPE), centrifugal therapeutic plasma exchange (cTPE), and double-filtration plasmapheresis (DFPP)-comparative real-world data, especially within the same patients, remain scarce. We conducted a retrospective, intraindividual comparison of mTPE, cTPE, and DFPP in 72 sessions from five clinically stable chronic inflammatory demyelinating polyneuropathy patients. Outcom

Apheresis-Triggered Torsades De Pointes in Congenital Long QT Syndrome Type 2: Citrate-Related Arrhythmia Risk and Safe Modality Switching During Myasthenic Crisis.

Citrate anticoagulation during therapeutic plasma exchange (TPE) chelates divalent cations and can precipitate torsades de pointes (TdP) in patients with pre-existing repolarization vulnerability. We report a 44-year-old woman with genetically determined long QT syndrome Type 2 (LQT2) presenting in myasthenic crisis due to anti-MuSK-positive myasthenia gravis. TPE was initiated urgently before confirmatory antibody results were available, as alternative rapid immunomodulatory options were not immediately accessible. Despite prophylactic electrolyte supplementation, the corrected QT interval (Q

Pelacarsen and lipoprotein(a) apheresis in secondary prevention: the Lp(a)FRONTIERS APHERESIS trial.

BACKGROUND AND AIMS: Lipoprotein apheresis (LA) is the only approved treatment for patients with elevated lipoprotein(a) [Lp(a)]. The Lp(a)FRONTIERS APHERESIS trial investigated whether pelacarsen reduces the need for LA in patients from Germany with elevated Lp(a) and established cardiovascular disease (CVD). METHODS: Adult patients with Lp(a) levels >60 mg/dl who had undergone ≥35 LA sessions in the prior year were randomized to receive pelacarsen 80 mg or placebo every 4 weeks for 52 weeks. Weekly LA sessions were performed if the Lp(a) measurement from the prior visit was >60 mg/dL. The p

Successful autologous T cell apheresis in pediatric patients with hematologic malignancies, solid tumors, and brain tumors.

BACKGROUND: Chimeric antigen receptor (CAR) T cell therapy is being used to target a range of pediatric cancers. Starting materials are critical for successful CAR T cell manufacturing, most often obtained via autologous mononuclear cell (auto-MNC) apheresis. While apheresis strategies are established for patients with B cell acute lymphoblastic leukemia (B-ALL), these approaches are not standardized and there are limited data on how these practices apply across other disease groups. Here, we report our experience with apheresis for CAR T cell production across various pediatric cancer diagnos
