Atlanto-axial fusion for instability.

The problem of how best to treat a patient with instability of the atlanto-axial complex is still somewhat controversial. In this follow-up study of fifty-nine patients, nineteen were treated by a plaster jacket and brace; eleven, by single midline wiring and onlay bone grafts; and thirty, by four circumferential wires around the posterior elements of the axis and atlas with two bone grafts wedged between these elements on each side according to the method described by Brooks and Jenkins. (One patient had both types of fusion.) Although direct comparison of the results of treatment in three gr

[An assessment of the results of corrective osteotomy for malunited fractures of the distal radius].

INTRODUCTION: Malunion is the most common and serious complications after fracture of the distal radius. Alterations of the normal biomechanics affect function of the wrist, which is associated with pain, disability and, in longer perspective, with arthrosis. Reestablishment of the normal anatomic relationships by corrective osteotomy is one of the most effective ways to prevent this scenario. PATIENTS AND METHODS: Twenty-five consecutive patients, 19 women and 6 men with the mean age of 50 years with malunited fractures of the distal radius underwent corrective osteotomy. Operations were per

Simplified internal fixation of fifth metacarpal neck fractures.

BACKGROUND: Fifth metacarpal neck fractures (boxer's fractures) are common injuries that contribute 20% of all hand fractures. Divergent percutaneous pinning (bouquet fixation) as described by Foucher has gained popularity but is challenging and at times arduous, as it requires the insertion of several slender K-wires into a narrow medullary canal. Here, we report on a simplified technique in which a single thick K-wire is inserted. TECHNIQUE: An 18/10 K-wire is bent at one end then mounted on a drill chuck. The incision is performed and the entry hole created using the K-wire, which is then

Biomechanical Investigation of Hallux Valgus Deformity Treated with Different Osteotomy Methods and Kirschner Wire Fixation Strategies Using the Finite Element Method.

The aim of this study was to propose a finite element method based numerical approach for evaluating various hallux valgus treatment strategies. We developed three-dimensional hallux valgus deformity models, with different metatarsal osteotomy methods and Kirschner wire fixation strategies, under two types of standing postures. Ten Kirschner wire fixations were analyzed and compared. The fixation stability, bone stress, implant stress, and contact pressure on the osteotomy surface were calculated as the biomechanical indexes. The results showed that the biomechanical indexes of the osteotomy a

Simplified protocol for treatment of malar fractures. Based on a 1,220-case, eight-year experience.

A simplified protocol of approach has evolved from the treatment of approximately 1,200 malar fractures in an eight-year period in the private practice of one of us (R.S.M.) and at the Los Angeles County/University of Southern California Medical Center. The method begins with the Gillies incison for reduction and routinely uses internal wire pin fixation tailored to the mechanical requirements of the malar fracture. It advances only if necessary to brow and infraorbital incisions, direct wiring, orbital exploration, or Caldwell-Luc. Antrostomy with antral packing. Occasionally, a transcutaneou

[Outpatient treatment of patients with fractures of hand tubular bones].

Results of treatment of 282 patients with 288 fractures of tubular bones of the hand were analyzed. In 190 (67.4%) cases conservative methods of treatment were used, in 92 (32.6%)--surgical methods. In cases of fractures without displacement plaster immobilization was used, in patients with stable fractures of finger phalanx and metacarpal bones with fragment displacement we made closed reposition and plaster immobilization. We applied intramedullary osteosynthesis with Kirschner's wire after closed reposition in unstable oblique and transverse fractures with fragment displacement. In open fra

Technique Tricks for the Gleich-Koutsogiannis Surgical Procedure for Correction of the Adult Acquired Flatfoot.

Gleich-Koutsogiannis is a widely known extra-articular closing calcaneal osteotomic technique for correction of the adult flatfoot. Absolute care must be observed during each procedural step to avoid undesirable drawbacks, such as nerves and tendons lesions, incorrect fixation, and hardware painful prominence. It is the purpose of this report to introduce tricks to critical steps in order to improve the technique and minimize potential surgical complications.

Perilunate Dislocations: Reduction and Stabilization.

BACKGROUND: The all-dorsal scapholunate reconstruction technique is indicated for the treatment of scapholunate injuries in cases in which the carpus is reducible and there is no arthrosis present. The goal of this procedure is to reconstruct the torn dorsal portion of the scapholunate ligament in order to stabilize the scaphoid and lunate. DESCRIPTION: A standard dorsal approach to the wrist, extending from the third metacarpal distally to the distal radioulnar joint, is utilized. The extensor pollicis longus is transposed and retracted radially, and the second and fourth extensor compartmen

Biomechanical comparison of intramedullary screw fixation, dorsal plating and K-wire fixation for stable metacarpal shaft fractures.

We compared four methods of metacarpal shaft fixation: 2.2 mm intramedullary headless compression screw; 3.0 mm intramedullary headless compression screw; intramedullary K-wire fixation; and dorsal plate fixation. Transverse mid-diaphyseal fractures were created in 64 metacarpal sawbones and were assigned into four groups. Peak load to failure and stiffness were measured in cantilever bending and torsion. We found that dorsal plating had the highest peak load to failure. However, initial bending stiffness of the 3.0 mm intramedullary headless compression screw was higher than that of the dorsa

Efficacy of Dual-Antibiotic-Loaded Bone Cement Against Multi-Drug-Resistant Staphylococcus aureus and Enterococcus faecalis in a Galleria mellonella Model of Periprosthetic Joint Infection.

Background: Antibiotic-loaded bone cement (ALBC) is widely used for local antibiotic delivery in joint arthroplasty to prevent and treat prosthetic joint infections (PJIs). In this study, we evaluated the efficacy of cemented Kirschner (K)-wires coated with various ALBC formulations using a Galleria mellonella infection model against multidrug-resistant (MDR) Staphylococcus aureus and Enterococcus faecalis. Methods: We tested commercially available bone cements, including gentamicin-only formulations (PALACOS R+G) and dual-antibiotic formulations, combining gentamicin with either clindamycin (

Polydioxanone sternal sutures for prevention of sternal dehiscence.

BACKGROUND: Sternal dehiscence and wound instability are troublesome complications following median sternotomy. Classic sternal approximation with stainless steel wires may not be the ideal approach in patients predisposed to these complications. We tested the efficacy of polydioxanone (PDS) suture in sternal closure and in prevention of complications in comparison to steel wires in high-risk individuals. METHODS: Three hundred sixty-six patients undergoing elective cardiac surgery with full median sternotomy and having body surface area (BSA) less than 1.5 m(2) were randomly assigned to rece

Total Loss of the Intermediate Cuneiform by Posttraumatic Avascular Necrosis: A Case Report.

We report a unique case of the total loss of the intermediate cuneiform by posttraumatic avascular necrosis resulting from a left foot open fracture and dislocation of the navicular bone and medial and intermediate cuneiforms at the Chopart and Lisfranc joints in a 64-year-old woman. The injury was managed with open reduction and internal fixation with Kirschner wires and cannulated screws. During postoperative follow-up, we observed avascular necrosis of the intermediate cuneiform and the total loss of the bone. An extensive English literature search revealed only one case report published on

Corrective Fifth Metacarpal Neck Osteotomy and Intramedullary Screw Fixation Technique: Technique and Case Report.

Malunion of metacarpal fractures can result in severe functional and esthetic impairment to the patient, thus requiring surgical correction. Traditionally, the fixation methods most commonly used for these fractures are the Kirschner wires or the plates and screws. However, one of the options for the fixation of metacarpal fractures is intramedullary fixation with a headless screw. This minimally invasive method can also be used for corrective metacarpal osteotomies. It has the advantages of being a minimally invasive technique and allowing the early range of motion. This minimally invasive me

[Fracture and dislocation fracture of the os pisiforme].

Four cases of pisiform fracture are presented. In one case the largest fragment of the pisiform bone with the articular surface was dislocated. After open reduction, osteosynthesis with K-wires was performed. Two patients had an isolated fracture without fragment dislocation. They had their lower arms immobilized with a plaster of Paris cast for 4 weeks. In the fourth case the fracture of the pisiform bone was associated with an intraarticular distal radial fracture and wrist dislocation. The typical traumatic cause of pisiform fracture is a fall onto the outstretched hand. Immobilization for
