The role of spiral CT for the assessment of the intracranial circulation in suspected brain-death.

BACKGROUND AND PURPOSE: The aim of this study was to assess the role of spiral CT for the diagnosis of brain death. METHODS: Over a 12-month period, 15 patients that fulfilled the clinical criteria of brain death were referred from the intensive care unit to evaluate remaining intracranial blood flow by spiral CT. The clinical diagnosis was confirmed by an apnea test in all cases. Two phases of spiral CT were performed at 20 and 60 seconds after the start of contrast media injection. Qualitative analysis included the evaluation of vessel opacification (arteries and veins) by two radiologists

[Physiopathologic changes in brain death. Their importance for management and organ donation decisions].

Technological advances in the medical area have allowed for development of useful techniques to treat patients with diverse diseases. For example, at intensive care units this technology allows maintenance blood flow and tissue oxygenation even when brain death (BD) is already established and the individual cannot function. The function of heart, lungs, and other organs can be maintained with different devices, but maintenance of cerebral functions is not not yet possible. Therefore, when a subject fulfills legal and medical requirements for BD, we must be clear that any patient procedure will

Maternal brain death, pregnancy and the foetus: the medico-legal implications for Ireland.

This paper examines some of the medico-legal issues that arose as a result of a situation which occurred in May 2001 in Ireland when a woman who was a British citizen and who was fourteen weeks pregnant collapsed and suffered a brain haemorrhage. She was taken to hospital where she was placed on life support but declared brain-dead. As a result of the uncertainty regarding the hospital's obligation to the foetus, life-support was maintained until further opinion was sought. After two weeks the foetus died and life support was only then discontinued. In Ireland there currently exists neither me

No evidence of NO-induced damage in potential donor organs after brain death.

Brain death induces multiple-organ dysfunction, with undesirable consequences for organ transplantation. However, the mechanisms are not completely clear. In the hearts, lungs, livers, and kidneys of rats, we investigated whether brain death leads to changes in nitric oxide (NO) production or to the formation of nitrotyrosine (the footprint of peroxynitrite, formed from NO and superoxide) or to lipid peroxidation products. To produce a rat model of brain death, we inflated a subdurally placed balloon catheter. We used the Griess reaction to assay plasma nitrite and nitrate. Proteolytic digesti

[Congestion in the area of the head and changes in the brain caused by suicidal hanging death].

In a prospective study of 204 unselected cases of suicidal hanging congestion bleedings of different degrees were found in 51%. In typical symmetrical hanging (n = 38) they occurred in 42% of cases, in atypical suspension (n = 152) in 53%. The difference had no statistical significance. Statistically, no connection could be secured between the outer congestion bleedings including those within the neck soft tissue on the one hand, and the form of hanging, the kind of loop or the breadth of the strangulation tool on the other. Only a slight dependence on the blood alcohol content was established

Large language model responses to questions about brain death/death by neurologic criteria: ChatGPT 4o-mini and Gemini 3 Flash (Fast).

BACKGROUND: Public understanding about brain death/death by neurologic criteria (BD/DNC) is generally poor. With the rising popularity of utilization of large language model (LLM) chatbots to answer medical questions, we sought to determine the quality of information about BD/DNC provided by ChatGPT 4o-mini and Gemini 3 Flash (Fast). METHODS: With the assistance of a family advocate, we developed 45 open-ended questions about BD/DNC and submitted them to ChatGPT 4o-mini and Gemini 3 Flash (Fast) in January 2026. We recorded response word count, Flesch-Kincaid Readability Score and source repu

Donation After Circulatory Death Yields Survival Rates Similar to Donation After Brain Death Liver Transplant, Which Effectively Expands the Donor Pool.

OBJECTIVES: Liver allograft shortage has necessitated greater use of donations after circulatory death. Limited data are available to compare recipients' health care utilization for donation after circulatory death versus brain death. MATERIALS AND METHODS: Liver transplant data for our center from November 2016 until May 2019 were obtained (208 donations after brain death and 39 after circulatory death). We excluded patients <18 years old and multiorgan transplants; for cost data only, we also excluded retransplants. Primary outcome was recipients' health care utilization in donation after c

Liver transplant after donation from controlled circulatory death versus brain death: A UNOS database analysis and publication bias adjusted meta-analysis.

BACKGROUND: Donation after circulatory death (DCD) is an increasingly utilized alternative to donation after brain death (DBD) to expand the liver donor pool. We compared the outcomes of liver transplant (LT) after DCD versus DBD. METHODS: A PRISMA-compliant systematic literature review was performed using the PubMed, Cochrane Library, and Embase databases (end-of-search-date: July 2, 2020). US outcomes were analyzed using the UNOS database (February, 2002-September, 2020). Pooled hazard ratios (HR) for patient and graft survival were obtained through random effect meta-analyses and adjusted

Studies of isolated global brain ischaemia: I. Overview of irreversible brain injury and evolution of a new concept - redefining the time of brain death.

Despite advanced cardiac life support (ACLS), the mortality from sudden death after cardiac arrest is 85-95%, and becomes nearly 100% if ischaemia is prolonged, as occurs following unwitnessed arrest. Moreover, 33-50% of survivors following ACLS after witnessed arrest develop significant neurological dysfunction, and this rises to nearly 100% in the rare survivors of unwitnessed arrest. Although, whole body (cardiac) survival improves to 30% following recent use of emergency cardiopulmonary bypass, sustained neurological dysfunction remains a devastating and unresolved problem. Our studies sug

Improved Outcomes in Severe Primary Graft Dysfunction After Heart Transplantation Following Donation After Circulatory Death Compared With Donation After Brain Death.

BACKGROUND: Primary graft dysfunction (PGD), the leading cause of early mortality after heart transplantation, is more common following donation after circulatory death (DCD) than donation after brain death (DBD). We conducted a single-center, retrospective cohort study to compare the incidence, severity and outcomes of patients experiencing PGD after DCD compared to DBD heart transplantation. METHODS AND RESULTS: Medical records were reviewed for all adult heart transplant recipients at our institution between March 2016 and December 2021. PGD was diagnosed within 24 hours after transplant a

Comparison Between Kidney Transplantation After Circulatory Death and After Brain Death: A Monocentric Retrospective Study After 1 Year of Follow-up.

BACKGROUND: Donation after circulatory death (DCD) is a solid resource to widen the kidney donor pool. Italian activity has grown in the last years with encouraging results. Our center has been active in DCD kidney transplantation (KTX) since November 2017, providing 22.5% of Italian DCD donations in 2018. We present a single-center retrospective analysis after a 1-year follow-up comparing DCD and donation after brain death (DBD) KTX outcomes. METHODS: DCD (controlled only) and DBD KTX performed in our center from November 2017 to December 2018 were considered. All DCDs underwent in situ norm

[Intensive care strategies for organ protection in patients with severe brain damage and imminent or confirmed irreversible brain death].

BACKGROUND: The 2025 revision of the donor identification guidelines by the German Medical Association reaffirms the importance of organ-protective intensive care or the application of intensive medical measures to maintain organ function in cases of imminent or established irreversible loss of brain function (brain death). These measures aim to preserve the possibility of organ donation in patients who have declared a willingness to donate or in whom the donation status remains uncertain. Currently, no evidence-based guidelines exist in Germany regarding the intensive care management in this

The observation period after clinical brain death diagnosis according to ancillary tests: differences between supratentorial and infratentorial brain injury.

OBJECTIVE: To determine the optimal observation period (OBP) in adults with a clinical diagnosis of brain death (BD) using electroencephalography (EEG) or computerized tomography angiography (CTA). METHODS: We conducted a retrospective observational analysis of adult patients with a diagnosis of BD from January 2000 to February 2017. The optimal OBP was defined as the minimum time interval from the first complete clinical neurological examination (CNE) that ensures that neither a second CNE nor any ancillary test (AT) performed after this period would fail to confirm BD. RESULTS: The study s

Brain-stem auditory evoked potential monitoring. The increase of the stimulus artifact in the development of brain death: a biological phenomenon?

Brain-stem auditory evoked potentials (BAEPs) were recorded in 12 dead subjects (mean age, 72.6 +/- 14.8 years), 30.6 +/- 19.5 hours (range 9-70) after abolished systemic circulation. Death was due to cardiac failure (n = 10), intracerebral hemorrhage (n = 1) and larynx cancer (n = 1). The presence and amplitude of the stimulus artifact were evaluated. The mean (+/- SD) amplitudes of the stimulus artifact was 0.03 +/- 0.02 microV on the left side and 0.01 +/- 0.02 microV on the right side. These findings in accordance with previous studies on comatose patients and brain dead subjects confirm t

[Relationship between cerebral circulatory arrest and loss of brain functions--analysis of patients in a state of impending brain death].

To clarify the temporal discrepancies between cerebral circulatory arrest and loss of brain functions in relation to neuroradiological differences of brain damage, we analyzed 100 cases of impending brain death evaluated by neurological findings and continuous, simultaneous neuromonitoring of somatosensory and brainstem auditory evoked potentials (SEP, BAEP), compressed spectral arrays (CSA), and transcranial Doppler sonography (TCD). A) Cases of supratentorial lesions: 1) Loss of brain functions after supratentorial circulatory arrest (TCD) were advanced rostrocaudally -(SEP N20 > CSA) > (BAE

Investigating the Impact of Donor Brain Death Duration on Outcomes after Lung Transplant.

BACKGROUND: Donor brain death induces haemodynamic and inflammatory changes that may influence organ quality and post-transplant outcomes. The impact of donor brain death duration on outcomes after lung transplantation remains uncertain. METHODS: We conducted a retrospective cohort study using the United Network for Organ Sharing database, including adult first-time lung transplant recipients from 2010 to December 2024. Donor brain death duration was defined as the interval between confirmation of donor brain death and donor cross-clamp. Missing data were addressed using multiple imputation.

Cerebral edema and brain death following intravenous N-acetylcysteine overdose: a case report.

N-acetylcysteine (NAC) infusions are rarely associated with cerebral edema. We present a patient that received 1242.2 mg/kg intravenous NAC over 8.3 h who developed seizures, cerebral edema, and died. A 17-24-year-old female (48.3 kg) presented after an acute acetaminophen ingestion. Their initial presumed 5 h acetaminophen concentration was 591.7 μmol/L. Initial alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were 16 and 21 IU/L, respectively. Due to inconsistencies in the time of ingestion, IV NAC was initiated as a two-step protocol of 150 mg/kg over 1 h followed by 15 m

Orbital Doppler Ultrasonography and Optic Nerve Sheath Diameter in Pediatric Brain Death Evaluation.

Background/Objectives: Brain death determination in children is clinically challenging. When standard clinical examination cannot be completed or reliably interpreted, ancillary testing is required-yet many established methods depend on infrastructure or patient transport that may not be feasible in critically ill pediatric patients. Orbital ultrasonography is bedside-applicable and non-invasive, but remains poorly characterized in children. Methods: We conducted a single-center retrospective study of 28 pediatric patients evaluated for suspected brain death between January 2021 and February 2

Therapeutic donor hypothermia following brain death to improve the quality of transplanted organs.

RATIONALE: Solid organ transplantation is the optimal treatment for patients living with end-stage organ failure. Globally, the majority of organ donors are deceased, most commonly following brainstem death. The process of brain death is known to cause significant injury to organs. Therefore, efforts to optimise the pre-donation care of these donors should be maximised to achieve improved recipient outcomes. OBJECTIVES: To assess the benefits and harms of therapeutic donor hypothermia in recipients of organs donated from brain-dead donors. SEARCH METHODS: We searched Cochrane Kidney and Tran
