Long-term survival of an infant with diffuse brainstem lesion diagnosed by prenatal MRI: a case report and review of the literature.

BACKGROUND: We describe a unique case of expansive diffuse brainstem lesion diagnosed prenatally by magnetic resonance imaging (MRI) with long-term survival. Findings of fetal and postpartum MRI were highly consistent with the characteristics of diffuse brainstem glioma. METHODS: Diagnosis was based on the features of MRI, and histopathology was not confirmed by biopsy. Although the prognosis of diffuse brainstem tumor is usually poor, this child was asymptomatic at birth and the neurological condition is still normal at 4 years of age without any treatment. RESULTS: During routine imaging f

Clinical Care for People Who Survive Childhood Cancer: A Review.

IMPORTANCE: An estimated 15 000 children and adolescents aged 0 to 19 years are diagnosed with cancer each year in the US, and more than 85% survive for at least 5 years. By 45 years of age, approximately 95% of people who survive childhood cancer will develop a significant health problem related to the childhood cancer diagnosis or its treatment. OBSERVATIONS: Approximately 500 000 people currently alive in the US have survived childhood cancer. The most common severe or life-threatening chronic health problems related to childhood cancer or its treatment are endocrine disorders such as hypo

Prediction of auditory brain stem responses damage in patients with head-and-neck cancers receiving radiotherapy using the functional assays of normal tissue complication probability models.

AIM: The purpose of this study was to set four NTCP models on clinical data and develop a model that calculates the possibility of hearing damage due to irradiation of healthy and at-risk brainstem tissue. MATERIALS AND METHODS: ABR tests were performed on 50 head-and-neck cancer patients three years after radiotherapy for evaluation of lesions in a part of the auditory nerve or the auditory pathway in the brainstem. RESULTS: It indicated a significant difference in the latency of the waves assessed by the ABR test between the two groups. The paired sample t-test indicated the latency time o

The Emergence of Stem Cell-Based Brain Organoids: Trends and Challenges.

Recent developments in 3D cultures exploiting the self-organization ability of pluripotent stem cells have enabled the generation of powerful in vitro systems termed brain organoids. These 3D tissues recapitulate many aspects of human brain development and disorders occurring in vivo. When combined with improved differentiation methods, these in vitro systems allow the generation of more complex "assembloids," which are able to reveal cell diversities, microcircuits, and cell-cell interactions within their 3D organization. Here, the ways in which human brain organoids have contributed to demys

Neuromyelitis optica with brainstem lesion mistaken for brainstem glioma. Case report.

Neuromyelitis optica (NMO) is a severe demyelinating syndrome defined principally by its tendency to affect optic nerves and the spinal cord selectively. Asymptomatic brain lesions have recently become a common finding in NMO, and symptomatic brain lesions do not exclude the diagnosis of this entity. The authors describe the case of a 12-year-old girl suffering from an unusually atypical form of NMO in which a brainstem lesion was mistaken for a brainstem glioma. Brainstem involvement in NMO exhibits variable features on neuroimaging and is confused with brainstem glioma in cases of extensive

Ganglioglioma in children. A review of 32 cases at the University of Florida.

The entity ganglioglioma in children seems to be an increasingly more common tumor. Most of these lesions are pathologically and clinically benign but have a pathological and clinical continuum in their behavior. Gross total resection, especially involving the lesions in the supratentorial compartment, is often curative. The most common symptom, seizures, in this group of lesions, can often be alleviated with removal of the tumor. This may be enhanced with the use of corticography at the initial surgery. The group of lesions located in the brainstem are more difficult to control and may requir

[Preoperative embolization in meningioma of the ponto-cerebellar angle. Indications and advantages].

Preoperative embolization of meningiomas has been performed in order to reduce surgical hemorrhage during the removal of these vascularized tumors. In this paper we emphasize the fact that occlusion of the tumoral vessels by artificial emboli produces an ischemic necrosis that greatly helps tumor exeresis. This is especially useful in meningiomas of certain localizations requiring complex surgical approaches. In our case, a giant ponto-cerebellar meningioma was dried up totally and then removed with relative ease through a conventional suboccipítal lateral craniectomy. The technique, indicatio

The duality of testosterone in glioblastoma: From targeted therapy to modulation of the tumor microenvironment and neuroprotection.

Glioblastoma (GB) remains one of the most aggressive and lethal human tumors, displaying a pronounced predominance among men that suggests a potential involvement of sex hormones, particularly testosterone (TST), in its pathogenesis. The present review summarizes and integrates current evidence regarding the dual role of testosterone in GB biology. On the one hand, testosterone promotes oncogenic progression by stimulating the proliferation, survival, and invasion of cancer stem cells (CSCs) through the activation of androgen receptors (ARs) and subsequent conversion to estradiol, which trigge

Central nervous system infections in cancer patients.

With improved therapies, patients with cancer survive longer. However, both the acute complications of intensive therapies and the risks of chronic immunosuppression have led to an increased incidence of central nervous system (CNS) infections. The presentation and course of common infections may be different from those in patients without cancer, and new syndromes related both to the underlying diseases and to their treatment have complicated the differential diagnosis. Noninfectious disorders such as drug treatment complications, vascular lesions, and radiation effects can mimic CNS infectio

Congenital ocular motor apraxia and brainstem tumor.

Congenital ocular motor apraxia is considered a benign disorder of horizontal saccadic eye movements. A case is presented of an 8-month-old boy with this clinical picture in whom, four months later, signs of increased intracranial pressure from a large, cystic neoplasm of the rostral part of the brainstem developed, the second such case reported to our knowledge. Occasional abnormalities of the nervous system in patients with this disorder are reviewed. Careful neurologic examination and continuing reevaluation of the conditions of these patients are encouraged, with consideration of computeri

Peritoneal metastasis of a brainstem anaplastic ganglioglioma in a 2-year-old boy: case report and literature review.

Anaplastic gangliogliomas (AGG) are rare tumors of the central nervous system (CNS) that commonly affect children and young adults, with an unusual infratentorial presentation, which is related to hydrocephalus and a worse prognosis. We report a case of a brainstem AGG in a 2-year-old boy who underwent a ventriculoperitoneal shunting (VPS) and later presented peritoneal metastasis. We also reviewed the related literature. Even though rare, disease dissemination through VPS should be sought in patients with CNS tumors and VPS who develop new abdominal symptoms. The early diagnosis and intervent

ACTH-producing hypophyseal carcinoma monitored by computed tomography.

A case of malignant ACTH-producing hypophyseal tumor with serial CT scan and histological investigation is discussed. The neoplasm became evident with a Cushing's syndrome and was treated with adrenalectomy and radiotherapy. Nelson's syndrome subsequently developed due to histologically atypical adenoma. Subarachnoid metastases occurred after 4 years, finally causing a fatal intracranial hemorrhage.

Gliomas (astrocytomas) of the brain-stem with spinal intra- and extradural metastases: report of three cases.

Astrocytomas of the pons and medulla oblongata ('brain-stem gliomas') while often invasive locally, do not as a rule seed and metastasize along the spinal meninges. Three cases are here reported (two adults, one child) in whom astrocytoma of the brain-stem metastasized along the spinal cord. The dura mater itself and the spinal epidural space were invaded in two cases. The child and one adult had a pontine astrocytoma, the other adult's tumour originated in the medulla oblongata. In the two cases that came to necropsy the tumour of the brain-stem was much better differentiated than the meninge

Microsurgical anatomy of the region of the tentorial incisura.

The microsurgical anatomy of the tentorial incisura was evaluated in 25 adult cadavers using X 3 to X 40 magnification. The area surrounding the incisura is divided into the anterior, middle, and posterior incisural spaces. The anterior incisural space is located anterior to the brain stem and extends upward around the optic chiasm to the subcallosal area; the middle incisural space is located lateral to the brain stem and is intimately related to the hippocampal formation in the medial part of the temporal lobe; and the posterior incisural space is located posterior to the midbrain and corres
