Sequencing of cognitive-behavioral treatments for bulimia nervosa.

OBJECTIVE: This study investigated the efficacy of a stepped-care trial of brief group psychoeducation (PE) followed by individual cognitive-behavioral therapy (CBT) in the treatment of bulimia nervosa. METHOD: Fifty-six subjects first completed PE and were then randomly assigned to either a 16-week trial of CBT (PE + CBT) or to no further treatment follow-up (PE-alone). RESULTS: Compared to PE-alone, PE + CBT produced significantly greater reductions in the specific eating symptoms of binging and purging and significantly higher remission rates that were maintained over a 16-week follow-up.…

MRS of the brain in patients with anorexia or bulimia nervosa.

Twenty patients with anorexia or bulimia nervosa were prospectively investigated by magnetic resonance spectroscopy (MRS) of the brain. Compared to healthy controls, MRS of those with eating disorders revealed metabolic changes, which seem to be a consequence of their nutritional deficiency.

Comparison of cognitive style in bulimia nervosa and depression.

This study investigated the global and specific cognitive style associated with bulimia nervosa. Three groups of women (women with bulimia nervosa, women with major depression, and controls) completed measures of eating disorder severity, depression, dysfunctional cognitions and irrational beliefs. The control group was found to report significantly lower levels of cognitive distortions and irrational beliefs overall than both women with bulimia nervosa and women with depression. However, no difference was found between the latter two groups. Furthermore, the pattern of individual cognitions a…

Rapid responders to intensive treatment of bulimia nervosa.

OBJECTIVE: The purpose of the study was to examine patterns of response to treatment in bulimia nervosa and to consider the utility of differentiating rapid from slower responders. METHOD: Participants were 166 female patients with bulimia nervosa who received specialized day hospital treatment for their eating disorder and provided complete data on symptom frequencies over the course of treatment. Symptoms and psychological functioning were assessed at the beginning and end of treatment and 2-year follow-up information was available for a subsample of 57 patients. RESULTS: A large subgroup …

Lifetime anxiety disorders in women with bulimia nervosa.

We examined the prevalence and ages at onset of additional childhood and adult psychiatric disorders in women with bulimia nervosa and evaluated the differential impact of a mood or anxiety disorder on the presentation of bulimia nervosa. One hundred fourteen women participating in a clinical trial of cognitive-behavioral therapy for bulimia nervosa were assessed at pretreatment with structured diagnostic methodology. Although mood disorders were the most frequently occurring additional psychiatric disorder (75%), 64% experienced an additional anxiety disorder. Age at onset of the anxiety diso…

What's New in the Treatment of Anorexia Nervosa and Bulimia?

The 3 eating disorders--anorexia nervosa, bulimia nervosa, and binge eating--occur at a frequency far greater than usually realized. Anorexia has been found to be present in up to 1% of teenage and young adult women, whereas estimates of bulimia have ranged up to 5%. The prevalence of binge-eating disorder is not known, but may be higher than bulimia. Anorexia nervosa is characterized by weight loss, body image disturbance, and a morbid fear of weight gain. Bulimia nervosa is characterized by binge eating and compensatory purging by vomiting; use of laxatives, diuretics, or diet pills; exercis…

The significance of a prior history of anorexia in bulimia nervosa.

OBJECTIVE: Some authors stratify women with current, normal-weight bulimia nervosa into groups defined by the presence or absence of a past history of anorexia nervosa. Unlike the distinction between current anorexia nervosa with or without bulimic symptoms, fewer studies have investigated the significance of stratifying bulimic women by a past history of anorexia. METHOD: One hundred and fourteen women with bulimia nervosa in a clinical trial were studied via structured clinical interviews covering Axis I and II disorders and measures of personality and symptomatology. Prospective, 14-day di…

Gastrointestinal symptoms in bulimia nervosa: effects of treatment.

OBJECTIVES: The aim of this study was to characterize the frequency and severity of gastrointestinal symptoms in bulimic patients and to determine their response to treatment of the eating disorder. METHODS: Forty-three consecutive bulimic patients admitted to the inpatient Eating Disorders Unit of the Psychiatry Service were asked to fill out a gastrointestinal symptoms questionnaire, an Eating Disorders Inventory, and a Zung Depression Inventory on admission and discharge. Thirty-two age- and sex-matched healthy volunteers completed the same questionnaire. RESULTS: In bulimic patients, the

Bulimia in a male alcoholic: a symptom substitution in alcoholism.

In recent years, evidence points to a relation between bulimia nervosa and alcoholism. A significant number of bulimics have been reported to abuse alcohol and drugs. Additionally, there is a high prevalence of alcoholism in the families of bulimic patients. In the light of the proposed hypothesis concerning symptom substitution between the dyscontrol syndromes, we present an alcoholic patient who developed bulimic symptoms in the abstinence period. In this patient bulimic manifestations disappeared as he restarted drinking again, suggesting that these two symptoms serve as a substitute for ea…

Validating a multidimensional model of the psychopathology of bulimia nervosa.

In a recent preliminary report, a multidimensional model for bulimia nervosa was proposed with latent dimensions body dissatisfaction, restricting behaviors, bulimic behaviors, and affective and personality disorder. It was suggested that body dissatisfaction clearly represented a distinct dimension of the disorder. In the current investigation, we cross-validated the prior results on a larger sample with a different set of variables and tested the fit against alternative models. Subjects were 497 women diagnosed as having bulimia nervosa. We performed a confirmatory factor analysis using the …

Temperament, character, and personality disorder in bulimia nervosa.

In a sample of 76 women participating in a clinical treatment trial for bulimia nervosa, we examined the clinical differences between subjects with and without concurrent personality disorders and the ability of "self-directedness" (a character scale of Cloninger's Temperament and Character Inventory) to predict the presence of personality disorder. Sixty-three percent of the sample had at least one personality disorder diagnosis. Fifty-one percent of personality disorders were in cluster C, 41% were in cluster B, and 33% were in cluster A. The presence of personality disorder was associated w…

Pharmacotherapy of bulimia nervosa and binge eating disorder.

This paper reviews the use of medication in the treatment of bulimia nervosa and of binge eating disorder. There is now compelling evidence from double blind, placebo-controlled studies that antidepressant medication is useful in the treatment of bulimia nervosa. What is less clear is which patients are most likely to benefit from antidepressant medications and how to best sequence the various therapeutic interventions available. The utility of antidepressant medications in bulimia nervosa has led to their evaluation in binge eating disorder. The limited information currently available suggest…

Advances in the Treatment of Anorexia Nervosa and Bulimia Nervosa.

During the last decade, much investigation into possible pharmacotherapy for eating disorders has been undertaken, especially for bulimia nervosa.Intensive hospital treatment compromising a combination of individual, group and family therapies with or without adjunctive pharmacological treatments are usually offered in anorexia nervosa. Osteoporosis and delayed gastric emptying are 2 medical complications that should be addressed as early as possible in the course of the disorder. Although not yet confirmed in controlled clinical trials, there is preliminary support for the use of hormone repl…

A controlled study of light therapy for bulimia nervosa.

OBJECTIVE: Winter worsening of mood and eating symptoms, similar to that of seasonal affective disorder, has recently been reported in patients with bulimia nervosa. To assess the effectiveness of light therapy for treatment of bulimia nervosa, the authors conducted a study of light therapy during winter comparing an active (bright white light) condition to a control (dim red light) condition in bulimic patients who were not selected for a seasonal pattern of bulimia. METHOD: After a 2-week baseline assessment, 17 female patients with a DSM-III-R diagnosis of bulimia nervosa underwent early m…

Perceptions of nonshared childhood environment in bulimia nervosa.

OBJECTIVE: The aim of this research was to identify perceptions of nonshared childhood environmental factors associated with bulimia nervosa and the comorbid personality traits associated with this disorder. METHOD: Twenty-nine individuals with bulimia nervosa and 27 normal controls compared themselves to a sibling on a measure specifically designed to assess perceptions of nonshared environmental factors in parental, peer, and sibling relationships. Also, bulimic individuals' primary clinicians completed a series of ratings for each of the DSM-III personality disorders. RESULTS: Bulimic ind…

Melatonin disturbances in anorexia nervosa and bulimia nervosa.

The pineal gland releases melatonin into the blood stream in response to sympathetic noradrenergic stimulation of pinealocytes. This process … lso occur in depression. In this review, studies of melatonin in patients with anorexia nervosa and bulimia nervosa are considered in relation to potential abnormalities of noradrenergic function and circadian rhythm. The influence of weight loss, binging and purging, and depression on melatonin is discussed. Other studies involving the assessment of mela …

Antidepressants vs. psychotherapy in the treatment of bulimia nervosa.

Studies of the effectiveness of psychotherapy and pharmacotherapy in the treatment of bulimia nervosa have been accumulating over the last decade. Only recently, however, have investigations been reported comparing the two modalities directly, assessing whether one might be more effective than the other, or whether a combination of the two approaches might be superior to either alone. This paper reviews the three combined treatment studies that have been published to date and discusses their implications for clinical practice and for future research.

The relationship of depression to dissociation in patients with bulimia nervosa.

Dissociative Experiences Survey scores were compared between bulimics with and without depression, and among depressed patients with and without bulimia. An association is noted between depression and dissociation, confounding the previously noted relationship between bulimia nervosa and dissociation.
