Comparative pharmacology of calcium antagonists.

OBJECTIVE: To compare clinically relevant pharmacokinetic, pharmacodynamic and toxico logical characteristics of calcium-modulating compounds used in ischemic heart disease. DATA SOURCES: A MEDLINE search (1990 pt B to 1991 pt A revised for 1993; 1991 pt B to 1992 revised for 1993; and January to May 1993) combining the search phrases 'calcium channel blockers', 'myocardial ischemia', 'pharmacodynamics' and 'pharmacokinetics', and a search in Compact Cambridge Drug Information Source vol-6 (revised 1992, fourth quarter) using the search phrase 'calcium antagonists' and medical subject heading…

[Children's requirements for dietary calcium].

Childrens' need for calcium is uncertain, and as a consequence there are large differences in the official recommendations for calcium intake. A low calcium intake is not followed by acute complications. A very low calcium intake (less than 200 mg/d) combined with a high intake of fibre and phytate can cause osteomalacia, as in rickets. Several studies support that calcium intake during growth is positively associated to bone mineralisation, and to peak bone mass, which is associated to the risk of developing osteoporosis. At present, we know very little about childrens' ability to adapt to a …

New CNS-specific calcium antagonists.

Ischemic insults to the brain in stroke or traumatic brain injury produce excessive release of glutamate from depolarized nerve terminals. This excessive glutamate release in turn stimulates massive calcium entry into nerve cells, activating a biochemical cascade that results in cell death. A major pathway of calcium entry into depolarized nerve cells is through voltage-sensitive, high threshold calcium channels. A large fraction of this calcium entry is mediated through "R-type" calcium channels, channels resistant to blockage by dihydropyridine calcium antagonists such as nimodipine. A newly…

[Phosphorus-calcium metabolism in hyperthyroidism].

Because of the bone remodelling it induces, hyperthyroidism modifies the parameters of calcium-phosphorus metabolism. For a better determination of the mechanism involved, we studied 13 patients with Graves' disease compared with 13 controls. We measured the various parameters of calcium-phosphorus metabolism, notably the levels of parathormone, 25-hydroxycholecalciferol, 1-25 dihydroxycholecalciferol and ostocalcin; 8 patients were re-examined in euthyroidism. Total and corrected values of calcaemia (P less than 0.05 and P less than 0.01), phosphoreamie (P less than 0.01), alkaline phosphatas…

Calcium antagonists in myocardial infarction.

Calcium antagonists are effective cardioprotective agents in experimental models of myocardial infarction. However, clinical trials in acute myocardial infarction and in postinfarction secondary prevention led to conflicting results related to the small size of the majority of the trials and possible differences among individual agents with distinct ancillary properties. Furthermore, one has to consider separately the trials in patients with Q-wave infarction and in others with non-Q-wave infarction. Q-wave patients do not seem to benefit from therapy with calcium antagonists. However, the eff…

Renal actions of calcium antagonists.

Calcium antagonists (CAs) interfere with the calcium entry of many cells and therefore affect various cells and properties of the kidney. At present, most information on the renal actions of CAs concerns their relaxing effects on vascular smooth muscle cells. Renal vasodilation and decreased renal vascular resistance are most prominent when CAs are used in hypertensive laboratory animals or patients. This results in an increase of renal blood flow and, at times, in an even greater rise of glomerular filtration rate (GFR). The augmenting effect on GFR is at present incompletely understood. It h…

Calcium--osteoporosis and blood pressure.

Calcium is the main essential mineral for building strong bones and sound teeth. Bone remodeling occurs throughout life; new bone is deposited and existing bone is replaced. Peak bone mass development is obtained up to 35 yr of age after which bone resorption is increased. It is therefore imperative that Ca intake meet the recommended dietary allowances not only during the growing period but throughout life. Milk and dairy products are the major food source of Ca in the US diet, supplying about 61% of our intake while other food groups supply the rest. It is almost impossible to supply the rec…

Cocaine-calcium channel antagonist interactions.

Diltiazem, a benzothiazepine calcium channel antagonist, was given to six healthy men as a single 60 mg oral dose 120 min before IV injection of cocaine (0.2 mg/kg) in a double-blind, placebo-controlled, two-session study. Diltiazem alone produced no significant effects. Cocaine increased blood pressure, heart rate, pupil size and subjective "high" ratings, and decreased skin temperature. Diltiazem pretreatment diminished the cocaine effect on skin temperature, but did not otherwise alter the response to cocaine. Calcium channel antagonists diminish the effects of cocaine in vitro and in anima…

Renal calcium metabolism and diuretics.

Diuretic agents have variable effects on calcium excretion as studied in vivo and in isolated kidneys and nephron segments. Generally, by increasing sodium and water excretion, diuretics will cause a concomitant increase in calcium excretion. As they diminish blood volume and alter renal hemodynamics, diuretics enhance calcium reabsorption in the proximal tubule, modulating their usual effects on calcium excretion. These general effects can be further modulated by additional metabolic actions. For instance, chronic administration of thiazide diuretics may diminish calcium excretion on the basi…

Calcium transport in nonmammalian vertebrates.

The title of this paper commemorating the contributions made by Professor Urist has an interesting bearing upon basic skeletal tissue biology. This is because the calcium-binding proteins (vitellogenins), upon which Professor Urist and Schjeide have focused much of their attention in non-mammalian vertebrates, although produced by the liver and present in the blood plasmas of non-mammalian vertebrates during vitellogenesis, are, in effect, substitutes for the protein casein present in the milk of mammalian vertebrates. Vitellogenins (180,000-250,000 daltons) appear to be produced solely for de…

Calcium antagonists and cardiovascular disorders.

All calcium antagonists have the ability to decrease the symptoms and signs in some patients with ischemic heart disease and help lower the blood pressure in hypertensive persons, but in clinical doses nifedipine does not exhibit antiarrhythmic properties, although these are an important part of the action of verapamil, diltiazem and some substances with a similar chemical structure. In certain disorders beta-adrenergic blocking drugs are useful adjuncts, and under some circumstances, particularly variant angina and supraventricular arrhythmias, specific calcium antagonists are the drugs of ch…
