BACKGROUND: The hereditary spastic paraplegias (HSPs) are pleiomorphic disorders of motor pathway and a large number of affected genes have been discovered. Yet, mutations in SPG4/SPAST represent the most frequent molecular etiology in autosomal dominant (AD) patients and sporadic cases. We describe a large, AD-HSP Sardinian family where 5 out of several living members harbored a novel deletion affecting also the 5'UTR of SPAST and resulting in reduced expression of DPY30, the gene located upstream SPAST in a head-to-head manner.

Here, we report the management of a severe necrotizing soft-tissue infection involving the entire lower limb of a man in his 70s. The infection originated from a lower leg hematoma that developed during anticoagulant therapy for deep vein thrombosis. Necrosis extended from the deep thigh to the lower leg, involving areas surrounding critical structures such as the femoral artery, sciatic nerve, and tibia. We present a unique case of displaced simultaneous bilateral fractures, Garden 3 type, in a 49 year woman treated with non-cemented total hip arthroplasty. The patient showed a Harris hip score of 86 on the right hip and a 81 on the left side on the fourth postoperative year, besides a bilateral Trendelenburg gait, more pronounced on the right side. Arthrodesis of the hip has been plagued by high rates of nonunion, and by complications associated with prolonged cast immobilization. To prevent these problems, we devised a procedure for hip arthrodesis using an external fixator in combination with internal fixation at the fusion site. We have treated nine patients with this technique. All of the arthrodeses were solidly united without wound infections at the most recent follow-up. Fruit pomace recycling is critical for green biomanufacturing. Lactococcus lactis, a common host for waste valorization, shows poor protein expression in conventional systems. We constructed T7RNAP-driven inducible and constitutive expression systems in L. lactis NZ9000. The two systems increased target gene transcription by 103-fold and 207-fold, and enzyme activity by 5.24-fold and 10.36-fold, respectively.
