A double blind trial involving 21 patients on carbenoxolone and 23 patients on placebo was conducted under endoscopic control to assess the healing of duodenal ulcers over a period of 6 weeks. The two groups were adequately matched with the exception that larger ulcers predominated in the carbenoxolone group. Relief of symptoms showed poor correlation with ulcer healing. Fourteen of the 21 patients in the carbenoxolone group and 7 of the 23 in the placebo group showed endoscopic evidence of complete healing. Endoscopy has become the first and primary diagnostic and therapeutic modality in the management of patients with severe gastrointestinal bleeding. Panendoscopy, push enteroscopy, and colonoscopy provide the diagnostic, prognostic, and therapeutic elements to improve patient outcomes and to reduce morbidity and mortality from severe GI hemorrhage. Recent improvements in endoscopic hemostatic techniques and in imaging modalities using wireless capsule endoscopy suggest that diagnostic and therapeutic endoscopy will be even more important in determining patient outcomes in the future.

Hemangiomas of the small bowel are an uncommon and difficult-to-diagnose cause manifesting with bleeding of the midgut. We report an 18 -year- old male under study due to iron deficiency anaemia. A video-capsule endoscopy was performed, revealing multiple raised lobulated violaceous lesions in the small intestine of 5 to 15 mm diameter, compatible with hemangiomas. Endoscopic therapy was carried out using an injection of 1% polidocanol, achieving the sclerosis of the lesion without complications and the correction of the anaemia.

Since 1.5 years wireless enteroscopy with the GivenM2A-capsule has been tested clinically. Wireless capsule-enteroscopy (WCE) has already contributed significantly to the understanding of patients with obscure intestinal symptoms. Series of occult bleeders show that WCE detects lesions in 60%, whereas enterography only in 15%, and push-enteroscopy in 25%.
