Characteristics of Discharged Elderly Patients with Acute Heart Failure Followed by Board-Certified-Cardiologists in a Rural Area of Japan.

The worldwide incidence rates of heart failure (HF) are approaching pandemic status due to aging societies. Board-certified cardiologists (BCCs) of the Japanese Circulation Society (JCS) are cardiologists who have completed the respective fellowship program and passed the examination. However, in rural areas, patients have limited access to medical care for social or geographical reasons. The clinical features of the specialist's follow-up for HF patients in rural areas are unclear. This study consists of 205 consecutive discharged elderly patients who were admitted to our hospital due to acute…

Increased circulating cell-free DNA levels and mtDNA fragments in interventional cardiologists occupationally exposed to low levels of ionizing radiation.

Circulating cell-free DNA (ccf-DNA) and mtDNA (ccf-mtDNA) have often been used as indicators of cell death and tissue damage in acute and chronic disorders, but little is known about changes in ccf-DNA and ccf-mtDNA concentrations following radiation exposure. The aim of the study was to investigate the impact of chronic low-dose radiation exposure on serum ccf-DNA levels and ccf-mtDNA fragments (mtDNA-79 and mtDNA-230) of interventional cardiologists working in high-volume cardiac catheterization laboratory to assess their possible role as useful radiation biomarkers. We enrolled 50 intervent…

Knowledge, attitudes, and practices of cardiac rehabilitation and barriers to referral among cardiologists in Saudi Arabia: A cross-sectional survey.

BACKGROUND: Cardiac rehabilitation (CR) is an effective secondary prevention intervention, yet it is globally underutilized. Physicians play a key role in CR uptake by eligible patients through encouragement and referral to the program. This study assessed the knowledge, attitudes, and practices concerning CR among cardiologists in the Kingdom of Saudi Arabia (KSA), identified barriers to patient referrals to CR programs, and proposed strategies to increase service adoption. METHODS: We conducted an observational cross-sectional study in which an online questionnaire was distributed via email…

Current prenatal counseling of congenital heart disease in trisomy 18, pediatric cardiologists' perspective: a Fetal Heart Society Research Collaborative Study.

OBJECTIVE: Survival outcomes are shifting in trisomy 18 as cardiac disease is being repaired in infancy reminiscent of trisomy 21. The landscape of prenatal counseling related to cardiac disease and trisomy 18 is unknown. STUDY DESIGN: A survey was distributed to pediatric cardiologists presenting two scenarios of cardiac disease varying by genetic diagnosis: trisomy 18 vs 21. Respondents were asked if cardiac surgery would be offered and ranked the importance of various factors in decision-making. RESULT: Sixty three percent described surgery as an option in trisomy 18 compared to 97% in tr…

Guideline adhesion in the management of severe acute pulmonary oedema: a French survey involving 1048 cardiologists, emergency physicians, and intensivists.

BACKGROUND AND IMPORTANCE: Acute pulmonary oedema is a frequent and potentially life-threatening emergency. Its management targets four key objectives: improving oxygenation, reducing volume overload, maintaining adequate blood pressure, and treating the underlying cause. Severe cases are mainly handled by cardiologists, emergency physicians, and intensivists, which may lead to variations in care and thus nonadherence to guidelines. OBJECTIVE: To evaluate interspecialty differences in the management of patients with severe acute pulmonary oedema and compare physicians' practices to 2021 Europ…

[Cardiologists and mucopolysaccharidoses. Recommendations of GICEM (Cardiology Experts on Metabolic Disease Italian Group) for diagnosis, follow-up and cardiological management].

Mucopolysaccharidoses (MPS) represent a group of rare lysosomal storage disorders, with a heterogeneous clinical presentation in terms of inheritance (autosomal and X-linked recessive), age of onset (infants, children, and adults), systemic and cardiac manifestations (mild to severe disease forms). Evidence-based recommendations on the diagnosis and management of cardiovascular disease in MPS are scarce. GICEM (Gruppo Italiano Cardiologi Esperti Malattie Metaboliche) is a group of cardiologists, cardiac surgeons and pediatricians with a specific expertise in metabolic diseases including MPS. I…

Guideline adherence in the management of acute pulmonary oedema: Study protocol for a French survey involving cardiologists, emergency physicians and intensivists.

BACKGROUND: Because of their high morbidity and mortality, patients with acute pulmonary oedema (APE) require early recognition of symptoms, identification of precipitating factors and admission to specialized care units (cardiac critical care or intensive care). APE is at the crossroads of different specialties (cardiology, emergency medicine and intensive care medicine). Although multidisciplinary expertise and management may be a strength, it can also be a source of confusion, with unexpected heterogeneity in patient care. We hypothesized that the management of severe APE may be heterogeneo

The Evolution of Upper Extremity Vascular System for the Interventional Cardiologist: A 400-Million-Year Tale for Arm-Based Interventions.

From the delicate fish fins to the highly specialized human arm, the vascular architecture of the "upper" extremity has undergone significant remodeling throughout vertebrate evolution. This fascinating journey leaves behind a complex anatomy and sometimes puzzling vascular variants that are difficult to interpret without evolutionary insight. With the rise of transradial interventions and antebrachial venous access for right heart catheterization, understanding this evolutionary context can offer the interventional cardiologist valuable awareness about the anatomical constraints and potential

[ANMCO Position paper: Amyloidosis for the clinical cardiologist. A "clinical primer" from the ANMCO Rare Disease Working Group].

Cardiac amyloidosis, in the three forms of immunoglobulin light chain (AL), transthyretin (ATTR) wild type (ATTRwt) and mutated (ATTRv) amyloidosis, is an increasingly known and recognized disease in the cardiovascular setting. The first stage of the patient's journey is the clinical suspicion of the disease, which is placed, in presence of a hypertrophic phenotype, by the identification of red flags, both extracardiac and cardiac clues whose presence increase the probability of being faced with a patient with this disease. The second stage is represented by diagnosis, which occurs with certai
