Telemedicine has played an important role in delivering healthcare for primary care, chronic disease patients, and those with solid organ malignancies. However, its application in subspecialties such as hematologic malignancies, hematopoietic cell transplantation (HCT), or chimeric antigen receptor-T cell (CAR-T) therapy is not widespread since physical examination is a vital component in delivering care. During the COVID-19 pandemic, we widely used telemedicine, since protecting our immunocompromised patients became our top priority. The employment of HCT and CAR-T therapies continues to grow…

BACKGROUND AND OBJECTIVES: Autologous hematopoietic stem cell transplantation (aHSCT) is increasingly used to treat aggressive forms of multiple sclerosis (MS). This procedure is believed to result in an immune reset and restoration of a self-tolerant immune system. Immune reconstitution has been extensively studied for T cells, but only to a limited extent for B cells. As increasing evidence suggests an important role of B cells in MS pathogenesis, we sought here to better understand reconstitution and the extent of renewal of the B-cell system after aHSCT in MS.
METHODS: Using longitudinal …

OBJECTIVE: Extranodal NK/T-cell lymphoma (ENKL) is aggressive and resistant to chemotherapy and radiotherapy. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a potentially curative treatment for high-risk lymphomas owing to its associated graft-versus-lymphoma (GVL) effect. However, its application to ENKL is limited. We aim to summarize the characteristics of allo-HSCT for ENKL and, more importantly, evaluate whether allo-HSCT could offer any benefits for ENKL.
MATERIALS AND METHODS: A systematic review and data analysis were performed to evaluate the performance of allo-HS…

Decreased cytomegalovirus (CMV)-specific immunity after hematopoietic cell transplantation (HCT) is associated with late CMV reactivation and increased mortality. Whether letermovir prophylaxis-associated reduction in viral exposure influences CMV-specific immune reconstitution is unknown. In a prospective cohort of allogeneic HCT recipients who received letermovir, we compared polyfunctional CMV-specific T-cell responses to those of controls who received PCR-guided preemptive therapy before the introduction of letermovir. Thirteen-color flow cytometry was used to assess T-cell responses at 3 …

Allogeneic hematopoietic stem cell transplantation (HSCT) is, to date, the only curative treatment for sickle cell disease (SCD). Because an human leukocyte antigen (HLA)-matched sibling donor is not always available, alternative stem cell sources such as unrelated or haploidentical related donors have been explored. To date, few series of SCD patients transplanted with an unrelated donor, cord blood, and haploidentical related donor have been reported, but the high rates of rejection and chronic graft versus host disease have limited their widespread application. We describe the outcomes of a…

PURPOSE OF REVIEW: Mantle cell lymphoma (MCL) is a disease predominantly affecting elderly patients with bad prognosis. Recently, a number of new agents have been shown to be active in this disease. This article reviews this data from the standpoint of everyday practice.
RECENT FINDINGS: Front-line regimens combining rituximab with CHOP, cytarabine, bendamustine or lenalidomide, frequently followed by rituximab maintenance, remain the standard. Choice depends on the aggressiveness of the disease, patient characteristics and local availability. BTK inhibitors have emerged as most important age

Objective: To evaluate clinical outcomes of autologous (auto-HSCT) and allogeneic hematopoietic stem cell transplantation (allo-HSCT) for angioimmunoblastic T-cell lymphoma (AITL) . Methods: From June 2007 to June 2017, clinical data of AITL patients who underwent HSCT in eight hospitals were assessed retrospectively. Results: Of 19 patients, 13 male and 6 female with a median age of 50 (32-60) years old, 12 auto-HSCT and 7 allo-HSCT recipients were enrolled in this study, all donors were HLA-identical siblings. Two of allo-HSCT recipients were relapsed auto-HSCT ones. There were 5 patients (5…

IMPORTANCE OF THE FIELD: Peripheral T-cell lymphomas (PTCL) are a heterogeneous group of non-Hodgkin's lymphomas in which conventional chemotherapy has been characterized by poor outcomes when compared with most aggressive B-cell lymphomas. Autologous, and to a lesser extent allogeneic, hematopoietic stem-cell transplant (HSCT) have been advocated as potential means to improve and consolidate remissions in PTCL.
AREAS COVERED IN THIS REVIEW: Given the absence of large-scale randomized clinical controlled trials of transplantation for PTCL, we review existing data addressing the role and timin

Mantle cell lymphoma (MCL) is a type of non-Hodgkins lymphoma (NHL) associated with poor progression-free and overall survival. There is a high relapse rate with conventional cytotoxic chemotherapy. Intensive combination chemotherapy including rituximab, dose intense CHOP- (cyclophosphamide-doxorubicin-vincristine-prednisone) like regimens, high dose cytarabine, and/or consolidation with autologous stem cell transplant (autoSCT) have shown promise in significantly prolonging remissions. Data from phase II studies show that even in patients with chemotherapy refractory MCL, allogeneic stem cell

Only in recent years has there been a specific focus on the treatment of T-cell lymphomas in general and peripheral T-cell lymphomas (PTCLs) in particular. An increasing number of PTCL-specific retrospective analyses have been reported, and the first data from PTCL-restricted prospective clinical trials have appeared more recently. In this context, the role of hemopoietic stem cell transplantation-primarily autologous but also allogeneic-has been investigated. High-dose therapy with autologous stem cell transplantation (HDT/ASCT) proved feasible in both relapsed/refractory and previously untre…

Previous studies identified radiation therapy as a key modifier of basal cell carcinoma (BCC) risk in survivors of hematopoietic cell transplantation (HCT). In the present analysis, risk of BCC was analyzed in relation to age at transplant, attained age, race, total-body irradiation (TBI), and radiation fractionation in 6,306 patients who received HCT at ages 0-65 years after conditioning regimens with (n = 3870) or without (n = 2436) TBI, and who were followed from 100 days to 36.2 years after HCT. While age-specific BCC rates in the unirradiated patient population were higher than those repo…

OBJECTIVES: Aggressive natural killer cell leukaemia (ANKL) is a malignant disorder of mature NK cells with a poor prognosis, for which no effective therapeutic approach has been established. We investigated the role of allogeneic haematopoietic cell transplantion (allo-HCT) in ANKL.
PATIENTS AND METHODS: Three patients with ANKL received allo-HCT and seven did not. Epstein-Barr virus (EBV) viral load (VL) of the whole blood was measured with real-time quantitative polymerase chain reaction.
RESULTS: We transplanted three patients using a myeloablative conditioning regimen with human leucocy

The objectives of study was to investigate the clinical characteristics and risk factors of pure red cell aplasia (PRCA) following ABO-incompatible allogeneic hematopoietic stem cell transplantation (allo-HSCT). The clinical data of 72 patients receiving ABO-incompatible allo-HSCT were collected and retrospectively studied. The clinical parameters including sex, age, granulocyte engraftment time and blood transfusion were analyzed for the exploration of risk factors resulting in development of PRCA. The results indicated that 4 out of 72 patients receiving ABO-incompatible allogeneic hematopoi…

Peripheral T-cell lymphomas (PTCL) are a rare group of heterogeneous lymphoproliferative disorders with their origin in the post-thymic T cells. Most PTCL have a relatively poor outcome, with a 5-year overall survival of less than 30%. Anaplastic large cell lymphoma (ALCL) has a better prognosis compared with other subtypes of PTCL. As with aggressive B-cell non-Hodgkin's lymphoma, high-dose chemotherapy followed by autologous stem cell transplantation should be offered to patients with PTCL in their first relapse. Patients with poor-risk features (alk-negative ALCL, histologic subtypes other …

Immune-mediated hemolysis is a well-recognized occurrence which complicates the period following a bone marrow transplant (BMT). However, although many studies have investigated the hemolytic anemia following ABO-incompatible BMT, data regarding the occurrence of alloantibodies against red blood cell (RBC) antigens other than ABO in patients undergoing hematopoietic stem cell transplantation are limited. In this review, we briefly analyze the most important non-ABO red blood cell (RBC) antigen systems involved in the development of post-BMT alloimmune hemolytic anemia, paying particular attent…

Allogeneic donor leukocytes can be used after nonmyeloablative conditioning to exploit their graft-versus-tumor (GVT) activity in the setting of reduced conditioning-regimen toxicity. This approach may be particularly useful for patients who relapse after autologous stem cell transplantation (SCT). However, GVT activity, toxicity, and ability to establish mixed chimerism may differ in patients who were heavily pretreated prior to SCT compared with patients treated earlier in the course of their disease. We have performed a series of studies of nonmyeloablative allogeneic transplantation and pr…

Human fetal ventral mesencephalic tissue transplantation provided early evidence supporting the efficacy of cell replacement therapy for Parkinson's disease; however, major challenges, including ethical concerns and graft-induced dyskinesia (GID), were raised. Recently, clinical trials using dopaminergic progenitor cells derived from induced pluripotent or human embryonic stem cells have demonstrated favorable safety profiles, graft survival, and the apparent absence of GID. Well-designed clinical trials are required to confirm the safety and establish its definitive clinical efficacy.
