Inhibition of root dentin demineralization by ion releasing cements.

OBJECTIVES: To compare the ability of resin-modified glass ionomer (RMGI) and bioactive cements to prevent root dentin demineralization. MATERIALS AND METHODS: Fifty molars were prepared at the cementum-enamel junction (n = 10) and restored with three bioactive cements (Activa Bioactive Cement, ACT; Ceramir Crown and Bridge, CER; and Theracem, THE), a self-adhesive resin cement (Rely X Unicem 2, UNI), and a RMGI cement (Rely X Luting Plus, LUT). Specimens were cycled for 30 days between a demineralization solution (pH = 4) composed of 0.1 M lactic acid and 3 mM Ca3 (PO4 )2 for 4 hours and a r

Complications following 250 cemented modular hip hemiarthroplasties.

BACKGROUND AND AIMS: Hemiarthroplasty is a common method of treating displaced femoral neck fractures, especially among elderly non-active patients with frailty syndrome. Complications arising from the use of a modern, modular hemiendoprosthesis via a posterior approach have been poorly reported in this population. The aim of this study was to evaluate complications and mortality associated with the use of Lubinus sp II cemented, unipolar hemiarthroplasty prosthesis and posterior approach during a 9-year follow-up. MATERIAL AND METHODS: All patients (244) who received hemiarthroplasty due to

Ten-year follow-up of a cemented tapered stem.

INTRODUCTION: This stem was cleared by the FDA in 2002 and has been implanted in cementless and cemented versions. Despite its long history, there are no long-term clinical  results available for the cemented version of this implant. The aim of this study was to provide such data. It was hypothesized that this implant delivers clinical success comparable to other tapered cemented stems. MATERIALS AND METHODS: A total of 113 hip replacements were performed in 106 patients between October 2007 and December 2009 using the cemented version of this stem. The mean age of the patients at operation w

Fluorination of zirconia surfaces impairs bonding to resin cement.

The aim of this study was to evaluate the fluorination of a zirconia surface as an alternative treatment for bonding to resin cement; the null hypothesis was that fluorination would not improve adhesion values. Four different surface treatments were performed on the surface of yttria-stabilized tetragonal zirconia polycrystal discs (N = 80): Ctrl (control), no treatment; SB, sandblasting with 50-μm alumina particles; SBS, sandblasting with 30-μm silica-modified alumina particles and application of silane; and SF, fluorination with sulfur hexafluoride-containing plasma. The surface roughness (R

Application of cement on strategic vertebrae in the treatment of the osteoporotic spine.

BACKGROUND CONTEXT: The application of pedicle screws with cement to strengthen the fixation of the osteoporotic spine has increasingly gained popularity. However, the technique has also led to an increase in cement-related complications. PURPOSE: The aim of the present study was to compare the clinical and radiological results of the patients with degenerative spinal pathologies who were treated with pedicle screws and cement injections on all segments versus those who were treated with cement injections only on the strategic vertebrae selected. STUDY DESIGN: A retrospective clinical study.

Early complications following cemented modular hip hemiarthroplasty.

INTRODUCTION: Hemiarthroplasty is the recommended treatment for displaced, intracapsular, femoral neck fractures. This study aimed to evaluate the early complications following insertion of the JRI Furlong cemented hemiarthroplasty, a contemporary, modular, double tapered, polished prosthesis. METHOD: A series of 459 consecutive patients (May 2006 - June 2009) treated with a JRI hemiarthroplasty with a minimum of one-year (1-4years) follow-up were evaluated. Data collected retrospectively from clinical records and hospital databases included patient demographics, mortality, deep infection, di

Ossiculoplasty with hydroxyapatite bone cement: our reconstruction philosophy.

The main objective of this study is to analyze results obtained with hydroxyapatite bone cement (HABC) ossiculoplasties. This is a retrospective study of a case series. This study was conducted in an academic hospital and tertiary referral center. A total of 127 ossiculoplasties using HABC were evaluated. Ears were divided into three groups according to procedure: group 1 involved reinforcement of the incudostapedial joint with cement and reconstruction of an incus long process defect with cement. Group 2 involved partial ossicular reconstruction between the stapes and malleus handle with HABC

Antibiotic-loaded bone cement and periprosthetic joint infection.

Antibiotic-loaded bone cement (ALBC) is commonly used for antibiotic delivery during total joint arthroplasty (TJA) for prevention or treatment of periprosthetic joint infection (PJI). ALBC is commonly used in two-stage exchange arthroplasty with static and dynamic spacers, beads, rods, and other custom spacers. The use of commercially available or hand-made ALBC for primary and revision TJA to prevent infection has also been studied. Commonly used antibiotics include gentamicin, tobramycin, and vancomycin powder, and these antibiotics can be used alone or in combination, depending on the orga

Load transfer after cemented total shoulder arthroplasty.

BACKGROUND: Glenoid loosening is the primary reason for failure after a total shoulder arthroplasty (TSA), but the failure mechanism is not yet known. This study determined how the load transfer and stress distribution are affected by the introduction of a glenoid implant. METHODS: We developed a finite-element model of a scapula with and without a virtually implanted modern glenoid prosthesis design. Two load magnitudes were considered: normal and high. Loading locations were simulated at the center and at 4 eccentric positions on the glenoid. A metal-backed implant was also simulated to und

Localized multiple cemental excrescences: a rare presentation of hypercementosis.

Hypercementosis is an abnormal thickening of cementum which may be generalized or localized. Localized hypercementosis is usually characterized by nodular enlargement of apical third of root. Hypercementosis in the form of multiple cemental spikes distributed throughout the entire length of root is a very rare finding. In this article, we report such a rare manifestation of localized hypercementosis affecting right maxillary third molar in a 24-year-old male. This finding was an accidental observation following a difficult extraction procedure as the spikes were not evident in the orthopantamo

Osteolytic pseudotumor after cemented total knee arthroplasty.

Wear debris leading to local inflammatory reactive changes and osteolysis is a common complication after total knee arthroplasty (TKA). While massive osteolytic lesions may be commonly encountered in the revision setting, the appearance and location of these lesions rarely mimic expansile bone lesions. We report a case of severe osteolysis in a cemented TKA design, which presented as a pseudotumor of the fibular head. The diagnostic work-up and pathologic analysis are included in this report of a pseudotumor secondary to wear debris osteolysis.

Removal of Bone Cement through Right Anterolateral Thoracotomy.

A 55-year-old woman who had a history of percutaneous vertebroplasty was referred to our institution with sudden onset of chest pain. Computed tomography (CT) scan demonstrated a long, linear, highly-attenuated segment in the right side of the heart and fragmented pieces in the right pulmonary artery. The CT scan and echocardiogram revealed no pericardial effusion or hemopericardium. Based on these findings, we performed surgery through right anterolateral thoracotomy without cardiac arrest. As a result, we safely removed the foreign body. This approach may be a feasible and effective procedur

Dust exposure and respiratory health effects in cement production.

Dust can be produced by almost all production processes in Portland cement factory. Dust exposure potentially can affect respiratory function. But evidence for respiratory effect of cement dust exposure has not been conclusive. In this study we assessed effect of cement dust exposure on respiratory function in a cement production factory. A respiratory symptoms questionnaire was completed and pulmonary function tests were carried out on 94 exposed and 54 non exposed workers at a cement factory in the east of Iran. Additionally, respirable dust level was determined by the gravimetric method. X-

Cement spacer as definitive management for postoperative ankle infection.

BACKGROUND: Postoperative infection can be a devastating complication of ankle replacement and arthrodesis surgery. Management consists of eradication of the infection and either, revision of the initial surgery or some form of salvage procedure. There are instances however when the patient is asymptomatic, medically unfit, or the local tissue is too tenuous to warrant performing additional surgery. We conducted a retrospective review of the outcome of the use of an antibiotic impregnated cement spacer as the definitive procedure in this kind of patient. METHODS: There were nine patients with
