Two patients with vertebral arteriovenous fistulae were treated at the Neurosurgical Clinic of the City Hospital of Hannover between 1981 and 1988. Both patients were males, 19 and 29 year old. The fistulae were secondary to cervical gunshot wounds. Both patients complained of a loud cephalic noise; 1 patient had a non pulsating neck mass. A systolic cervical bruit was heard in both cases. One patient had an incomplete mid-cervical Brown-Sequard syndrome. The fistulae involved the second portion of the vertebral artery: 1 fistula was fed, in addition to the vertebral artery, by the deep cervic

Transverse process fractures of the cervical vertebrae have been considered rare and insignificant. In a retrospective study of 216 patients with cervical fractures evaluated by plain films and computed tomography, we found that transverse process fractures were common. Transverse process fractures were present in 24% of patients with cervical fractures and accounted for 13.2% of all cervical fractures. Cervical radiculopathy and brachial plexus palsy were present in 10% of patients with transverse process fractures. In 78% of transverse process fractures, CT scanning showed that the fracture

OBJECTIVE: Hirayama disease is a rare cause of cervical myelopathy predominantly affecting young individuals. The disease is classically characterized by muscle atrophy in the distal upper limbs. While various etiopathogenesis such as dural sac dysplasia, nerve root dysplasia, structural abnormalities of the spinal ligament, and venous dysplasia have been proposed, this study explores the potential role of venous pathology and surgical management on the basis of it.
METHODOLOGY: This is a prospective descriptive case series of nine cases. The diagnosis was made based on the Huashan diagnostic

BACKGROUND: Diaphragm function can be assessed by electromyography of the diaphragm during electrical phrenic nerve stimulation (ES). Whether phrenic nerve conduction time (PNCT) and diaphragm electrical activity can be reliably measured from chest wall electrodes with ES is uncertain.
METHODS: The diaphragm compound muscle action potential (CMAP) was recorded using an oesophageal electrode and lower chest wall electrodes during ES in six normal subjects. Two patients with bilateral diaphragm paralysis were also studied. Stimulations were deliberately given in a manner designed to avoid or in

This CME article describes the current developments in peripheral regional anesthesia for shoulder dislocation, clavicle fractures and (serial) rib fractures. Ultrasound-guided regional anesthesia techniques are increasingly replacing analgesic sedation or general anesthesia for shoulder reduction, combining pain relief and muscle relaxation with greater safety. In clavicle fractures, the midclavicular block provides targeted analgesia without motor impairment as an alternative to established combined block of the brachial plexus and cervical plexus. For serial rib fractures, fascial plane blo…

Schwannomas are the rare benign tumors of the neck. Patients developing from the brachial plexus and cervical truncus and extending to the posterior mediastinum have been reported, but for the first time the following case report describes the extension of a vagal origin schwannoma from the anterior and middle mediastinum and its extraction with thoracotomy without sternotomy.

Offering invasive prenatal cytogenetic testing in cases of isolated choroid plexus cysts is controversial. To give a contribution to this discussion we recorded prospectively the course of 41 fetuses with cysts of the choroid plexus diagnosed in 4,326 pregnancies sonographically scanned in our center between January 1994 and August 1995. The fetuses were all in the 13th to 24th week of gestation, with an average of 19.3 weeks. Only 1 of these fetuses (with large bilateral choroid plexus cysts) had further sonographically visible malformations (renal and cardiac anomalies, malposition of the ha

We present a woman who developed left arm sympathetically maintained pain (SMP, or "shoulder-hand syndrome") as a result of brachial plexus injury. After confirmatory diagnosis with both stellate local anesthetic block and intravenous phentolamine infusion, the patient had a cervical epidural catheter placed and a local anesthetic infusion started. After numerous unilateral blocks were obtained, a cervical epidurogram demonstrated a probable cervical midline epidural septum. Catheter placement was adjusted, and a successful chemical sympathectomy was performed for 6 days. This resulted in sign

The transmanubrial musculoskeletal sparing approach (TMA) is commonly used for resecting apical lung tumours with vascular involvement. Non-neoplastic conditions which might require surgical exploration of the thoracic outlet include the 'cervical rib', a clinical condition consisting of an additional rib forming above the first rib and growing from the base of the neck just above the clavicle. Type 1 cervical rib-when a complete cervical rib articulates with the first rib or manubrium of the sternum-is the most challenging scenario where the subclavian artery can be damaged by continuous comp

Thoracic outlet syndrome (TOS) refers to an abnormal compression of the subclavian vessels and the brachial plexus at the base of the neck or thoracic outlet. The authors described a novel case of a 66-year-old male with arterial TOS and unilateral upper rib dysplasia following cervical spine disease. Owing to the comorbidity of cervical spondylotic amyotrophy and a history of sloping shoulder due to rib dysplasia, TOS occurred, which subsequently led to subclavian arterial thrombosis and eventually resulted in the gangrene of the hand. Thrombectomy and surgical resection of the first and seco

A technique for recording somatosensory evoked potentials using stimulation of musculocutaneous sensory nerve fibers proximal to the wrist has been used in 10 normal subjects and in 8 patients with traumatic lesions of proximal parts of the brachial plexus. The technique gave satisfactory evoked potentials in all normal subjects and provided useful information in patients with avulsion of the 5th and 6th cervical roots. The results were similar to those obtained by radial nerve stimulation. The results in 3 patients with upper trunk injuries and in 1 patient with avulsion of the 5th cervical r

Thoracic outlet syndrome (TOS) is defined as a set of symptoms caused by the compression of the brachial plexus and subclavian vessels in the thoracic outlet region. Anomalies in musculoskeletal structures may be responsible for TOS, including prolonged transverse process of the seventh cervical vertebra, cervical rib, and first anomalous rib and clavicle fractures. The authors describe a case of a young woman with pain in the left forearm, accompanied by intermittent claudication, weigh loss, myalgias and ischemic lesions in the fingers, with no pulses and no measurable blood pressure in the

A total of 11 patients with combined traumatic injuries of the brachial plexus and spinal cord were reviewed retrospectively. Brachial plexus paralysis in such dual injuries tends to be diagnosed and treated late and the prognosis is usually poor. The associated injuries, which were all on the same side as the plexus lesion, were to the head (nine cases), shoulder girdle (five), thorax (nine) and upper limb (seven). These other injuries were responsible for the delayed diagnosis of brachial plexus paralysis and the poor prognosis was probably because of the delay in starting treatment and the

Intracranial trajectories of sympathetic nerve fibers originating in the superior cervical ganglion (SCG) in the rat were investigated by means of anterograde labeling following the injection of wheat germ agglutinin-horseradish peroxidase conjugate (WGA-HRP) into the unilateral SCG. The trajectory of the sympathetic fiber innervating the pineal gland and its continuing structures was found advancing along the abducent nerve, through the cavernous plexus, then along the trochlear nerve. Labeled sympathetic fibers showed two patterns of distribution in the blood vessels on the basal surface of

Thoracic Outlet Syndrome (NTOS) encloses the whole set of clinical manifestations associated with the compression of the trunks of the brachial plexus and/or of the subclavian vessels as they cross the thoracic outlet (cervico-thoraco-brachial junction). It rarely affects children. We here report the case of a young girl treated for neurologic NTOS caused by supernumerary cervical rib.

Introduction: MR neurography (MRN) of the brachial plexus has emerged in recent years as a safe and accurate modality for the identification of brachial plexopathies in pediatric and adult populations. While clinical differentiation of brachial plexopathy from cervical spine-related radiculopathy or nerve injury has long relied upon nonspecific physical exam and electrodiagnostic testing modalities, MRN now permits detailed interrogation of peripheral nerve anatomy and pathology, as well as assessment of surrounding soft tissues and musculature, thereby facilitating accurate diagnosis. The rea

A case is described in which the 2.7-kg fetus of a nonobese primigravid patient, delivered by midlevel forceps manipulation sustained neurapraxias of both the marginal mandibular branch of the seventh cranial nerve and the upper trunks of the ipsilateral brachial plexus. The pregnancy was uneventful, but labor was complicated by an occipitoposterior presentation and a prolonged second stage. Examination of the neonate revealed bruising and skin markings consistent with injury by obstetric forceps and the anatomic location of these marks suggested that cervical and mandibular compression from t

BACKGROUND: Transfer of the supinator motor branches to the posterior interosseous nerve (SPIN) was first described as a reliable method of restoration of digit extension in cases of paralysis when there is retained function in the 5th and 6th cervical nerve roots with loss of function in the 8th cervical nerve root.
METHODS: The authors performed a retrospective review of all SPIN transfers that were performed in their unit over a 6-year period, which included 16 limbs of 14 patients. The median age was 49 years (range, 22 to 74 years). The average follow-up period was 15 months (range, 6 to
