Acute cerebellar ischaemic stroke secondary to arterial thoracic outlet syndrome.

Arterial thoracic outlet syndrome comprises a collection of symptoms due to compression of the neurovascular structures of the thoracic outlet. Cervical ribs are rare congenital abnormalities that are a cause of thoracic outlet syndrome, leading to upper limb complications depending on the compressed structure. Management tends to be surgical in the form of rib resection. We report a case of arterial thoracic outlet syndrome secondary to a cervical rib in a 45-year-old male who presented with left-sided cerebellar stroke caused by subclavian artery thrombosis. Medical management in the form of…

Combined endovascular and open surgical approach for the management of subclavian artery occlusion due to thoracic outlet syndrome.

A 32-year-old male with arterial thoracic outlet syndrome (TOS) underwent endovascular treatment for the chronic total occlusive lesion from the subclavian to the brachial artery after resection of the first rib and cervical rib. A combined endovascular and surgical treatment represents an attractive alternative to the traditional surgical approach for the treatment of complicated arterial TOS.

Subclavian artery aneurysm due to costoclavicular compression.

Proximal involvement of the innervation of the arm is a well recognised cause of hand symptoms; a proximal vascular abnormality is rarely responsible. This case report describes such a rare cause, namely, a post-stenotic aneurysm of the subclavian artery due to an abnormal first rib but in the absence of a cervical rib or scalene bands.

Dissimilar metals in a rib spreader a surgical electrical hazard.

A case is presented and electrochemical theory is discussed in a situation in which a battery was produced between two dissimilar metals in a Finochietto rib spreader. The problem developed in the thorax of a patient undergoing transthoracic cervical sympathectomy and resection of the first rib. Caution is advised in the use of dissimilar metals in patients undergoing surgery.

Supraclavicular resection of the first rib for thoracic outlet syndrome.

The experience with supraclavicular transcervical first rib resection (or cervical rib resection) and total scalenotomy reported herein shows this to be a safe and effective treatment for patients with the thoracic outlet syndrome. We recommend this method as a satisfactory anatomic approach for the correction of this condition. Its advantages are as follows: anatomic structures may be visualized fully by both the surgeon and the assistant; complex or recurrent thoracic outlet problems may be dealt with directly; additional procedures (such as vascular graft, neurolysis, neck exploration, symp…

Unusual etiology of upper extremity ischemia in a scleroderma patient: thoracic outlet syndrome with arterial embolization.

Although the causes of digital ischemia and necrosis are diverse in women, the most common etiology is connective tissue disease. We describe a patient with scleroderma who presented with arm ischemia secondary to arterial embolization from thoracic outlet syndrome. Her sublavian artery was compressed by the anterior scalene muscle and a cervical rib, leading to a stenosis with poststenotic dilation of the artery. Within the aneurysmal formation was a thrombus, which was probably the source of the distal embolization. The patient underwent surgical resection of the cervical and first rib. The …

Isolated bifid rib: clinical and radiological findings in children.

BACKGROUND: Bifid rib is usually asymptomatic but sometimes occurs associated with other pathological conditions. The aim of this study was to investigate clinical and radiological characteristics in children with bifid rib. METHODS: Nine children with the diagnosis of bifid rib between 2005 and 2010 were reviewed. Chest radiography and computed tomography (CT) were performed in all patients, and 3-D images were additionally reconstructed in six patients. RESULTS: Five girls and four boys with a mean age at presentation of 4.2 years had various types of unilateral bifid rib. Seven patients c

Segmental vertebral artery agenesis with deep cervical reconstitution in a cervicothoracic variant cluster.

PURPOSE: To report a complex cervicothoracic vascular-osseous phenotype detected on CTA and define its surgical and radiological relevance. METHODS: Neck CTA of a 76-year-old woman was reviewed on multiplanar reconstructions for aortic arch branching, supraaortic courses, vertebral artery segments, collateral channels, cervical ribs, styloid processes, and pharyngeal carotid relationships. RESULTS: CTA showed a common origin of the brachiocephalic trunk and left common carotid artery (colloquially termed bovine aortic arch), bilateral rudimentary cervical ribs, high-riding pretracheal brachi…

[Two post-traumatic "Gilliatt-Sumner hands" from decompensation of thoracic outlet syndrome].

The clinical picture of hand atrophy related to a cervical rib has been well described in 1970 by Gilliatt and Sumner. These authors reported a series of nine patients whose motor status was stabilized following decompressive surgery of the brachial plexus. We report two young patients decompensating a predisposed thoracic outlet (rudimentary cervical rib), following a scapular or cervical trauma. After several years of neck and arm painful complaints, the two patients progressively developed hand atrophy. One patient had been operated both at cervical (double discectomy and disc prosthesis) a…

[Thoracic outlet syndrome due to first rib anomaly: a case report].

Compared to cervical rib, first rib anomaly is less familiar to us as a cause of thoracic outlet syndrome. We report a case of thoracic outlet syndrome due to first rib anomaly. A 50-year-old man was admitted to our hospital because of right arm pain when his arm was raised up. Chest X-ray showed an abnormal right first rib which made a J-shaped curve and was attached to the second rib. A palpable mass was noted in the supraclavicular region. Angiography revealed complete occlusion of the right subclavian artery in the Allen test position. During conservative therapy for two months, the sympto…

[Acroparesthesias in plexus lesions: clinical aspects and treatment (author's transl)].

The author delimits the so-called brachial dysparesthesias from the "true acroparesthesias", the actual carpal tunnel syndrome. Dysparesthesias may be caused by cervical ribs, the scalenus syndrome and mixed forms of various abnormalities. In addition to a cervical rib, e.g. changes in the costoclavicular joint may be present, or an abnormal position of the subclavian artery. Also, lesions of nerves and vessels may emanate from the neighboring organs, as from vertebral metastases, enlarged lymph nodes etc. Generally, the treatment is conservative temporizing. Only severe cases with a confirmed…

Keep your eyes on the ribs: the spectrum of normal variants and diseases that involve the ribs.

A variety of normal variants or pathologic conditions of the ribs may be overlooked at chest radiography if the ribs are not evaluated carefully. Rib lesions may simulate pulmonary disease as well. Normal rib variants include cervical, intrathoracic, and pelvic ribs; forked rib; fusion and bridging; and pseudarthrosis of the first rib. Trauma-related lesions are common and usually occur in isolation but can alert the radiologist to other injuries. Metastases may appear as vague areas of increased opacity overlying the lung if seen en face and typically have a smooth interface with the lung on

[Treatment of the thoracic outlet vascular syndrome].

INTRODUCTION: The title "Thoracic Outlet Syndrome" (TOS) was introduced by Peet in 1956 [1]. In 1958 Charles Rob defined TOS as a "set of symptoms that may exist due to compression on the brachial plexus and on subclavian vessels in the region of the thoracic outlet" [2]. Compression due to cervical rib was first described by Galenus and Veaslius in the 2nd century A.D. The first unsuccessful resection of the cervical rib in patients with TOS was performed by Coote in 1861 [4]. In 1905 Murphy first made a successful resection of the cervical rib in patients with TOS and subclavian artery aneur…

Clinical predictors of surgical outcome in patients with thoracic outlet syndrome operated on via transaxillary approach.

OBJECTIVES: Although the transaxillary route appears to be the optimal approach in patients with thoracic outlet syndrome (TOS), the effect … duration (P=0.023), the subgroups of ulnar nerve conduction velocity (P=0.033) and the presence of cervical rib (P=0.003) showed a significant correlation with surgical outcome. Multivariate analysis revealed that the shorter duration of symptoms (P=0.017) and the presence of a cervical rib (P=0.026) had a significantly unfavorable effect on surgical outcome. …

The cervico-thoracic outlet compression syndrome. Analysis of surgical treatment.

Surgical treatment of the thoracic outlet compression syndrome is being presently reconsidered. Until these last few years, there was the ch … tial scalenectomy, resection of all fibrous bands pressing on nervous trunks, or the resection of a cervical rib. Should the costo-clavicular space appear anatomically too narrow, the first rib, already partially freed by the cervical approach, will be resected through the axillary route.

Pseudotumor of the supraclavicular fossa.

Minor musculoskeletal variants of the upper thoracic spine or of the ribs can cause effacement of the supraclavicular fossa, simulating a mass ("pseudotumor"). These variants may occur singly or in combination, and include unilateral prominence or asymmetry of the first rib, unilateral prominence or asymmetry of a cervical rib, and mild upper thoracic scoliosis causing prominence of the first rib on one side. The detection of variants by physical examination or more easily by review of the chest x-ray film may in selected instances obviate the need for costly diagnostic procedures and unnecess…
