What is it to discipline a child: What should it be? A reanalysis of time-out from the perspective of child mental health, attachment, and trauma.

Parental discipline strategies are a necessary and critical aspect of positive child development. Their qualities confer risk versus protection for the development of mental health problems. Time-out from positive reinforcement is now one of the most common and well-researched discipline procedures across the world, with overwhelming evidence to support its efficacy and acceptability. It has also recently attracted considerable criticism from writers evoking child well-being considerations based on attachment theory. The main concern is that the removal of a child to time-out exposes the child

Impact of child maltreatment on the costs of health service use and productivity loss: Findings from the Australian child maltreatment study.

BACKGROUND: Child maltreatment (CM) poses a substantial health burden, but its impact on healthcare costs and productivity loss in Australia remains unclear. OBJECTIVE: To estimate the costs of health service use and productivity loss associated with CM in Australia. PARTICIPANTS: Cross-sectional data (N = 8503) from the Australian Child Maltreatment Study were analyzed to identify people with CM, their health service use, and employment status. METHODS: Hurdle models estimated in two parts, comprising logistic regression and generalized linear models, were used to estimate differences in h

Social ecology of child soldiers: child, family, and community determinants of mental health, psychosocial well-being, and reintegration in Nepal.

This study employed a social ecology framework to evaluate psychosocial well-being in a cross-sectional sample of 142 former child soldiers in Nepal. Outcome measures included the Depression Self Rating Scale (DSRS), Child Posttraumatic Stress Disorder Symptom Scale (CPSS), and locally developed measures of functional impairment and reintegration. Hierarchical linear modeling was used to examine the contribution of factors at multiple levels. At the child level, traumatic exposures, especially torture, predicted poor outcomes, while education improved outcomes. At the family level, conflict-re

Longitudinal maternal depression and child mental health from pregnancy to child age 7: a study of three life course frameworks.

PURPOSE: Maternal depression is associated with mental health difficulties in children, but it is unclear how the timing or duration of depressive symptoms impact child outcomes. This study tested three life course frameworks in the longitudinal association between maternal depression and child mental health: sensitive periods, accumulation, and chain of risk. METHODS: 841 mother-child dyads in Pakistan were followed from pregnancy through child age 7. Maternal depressive symptoms were assessed during the third trimester of pregnancy, infancy (3 and 6 months postpartum), early childhood (chil

Can coders abstract child maltreatment variables from child welfare administrative data and case narratives for public health surveillance in Canada?

BACKGROUND: Public health surveillance is essential to inform programs that aim to eradicate child maltreatment (CM) and to provide services to children and families. However, collection of CM data imposes a burden on child welfare workers (CWWs). This study assesses the feasibility of hiring coders to abstract the required information from administrative records and case narratives. METHODS: Based on a convenience sample of child welfare data from Manitoba, Canada, two coders abstracted information on 181 alleged CM cases. The coders completed a short web-based questionnaire for each case to

Possible paths to increase detection of child sexual abuse in child and adolescent psychiatry: a meta-synthesis of survivors' and health professionals' experiences of addressing child sexual abuse.

BACKGROUND: Efforts are directed both towards prevention and early detection of Child sexual abuse (CSA). Yet, only about 50% of CSA survivors disclose before adulthood, and health professionals rarely are the first disclosure recipients. Increasing the detection rate of CSA within the context of Child and Adolescent Psychiatry (CAP) therefore represents a significant secondary prevention strategy. However, facilitating CSA disclosure when the survivor is reluctant to tell is a highly complex and emotionally demanding clinical task. We therefore argue that efforts to increase detection rates o

Healthy Lifestyle Matters: A Network Analysis of Urban Chinese Pre-School Children's Adiposity, Sleep Health, Mental Health, Child Functioning and Health Behaviours.

BACKGROUND: In the Chinese context, a greater emphasis is placed on academic skills rather than healthy lifestyles among pre-school children. To promote well-being as a whole, a comprehensive interpretation of multivariant relationships between health, functioning and well-being is necessary for children in this age group. METHODS: The current study adopted a network analysis among 422 urban Chinese pre-school children (57.8 ± 9.7 months, 54.2% boys) to detect the inter-relationships between the variables from nine domains, including demographics, adiposity, physical activity levels, executiv

Climate change and child health in Europe: a scoping review of emerging impacts.

UNLABELLED: Children are extremely vulnerable to climate-related environmental impacts and air pollution due to both biological and behavioral factors. Despite the emerging evidence of the increasing effects of climate change on the world and on children's health, policies to drive change and halt the effects are lacking. Climate change is causing Europe to heat up faster than other continents. Here, we assessed the evidence-based effects of climate change and air pollution on child health in Europe. A scoping review was performed to map the impact of climate-related exposures on the health of…

A Health Equity-Driven Approach to Primary Care for the Child Born Premature.

Every year, 1 in 10 children is born premature in the United States. Although survival rates for children born premature have improved, health and educational disparities remain persistent across their lifetime compared to full-term peers. Recent attention has been given to the promotion of health and development for children born premature through high-risk follow-up programs. However, there remains limited guidance in primary care and, in particular, guidance with an emphasis on health equity-an even more important focus given the often heightened medical and social needs of children born pr

Familial confounding and causal inference in child and adolescent neurodevelopment and mental health.

Recent announcements by the US Government linking paracetamol use during pregnancy to autism in offspring highlight the risks of misinterpreting observational research to inform policy; this is a clear example of the principle that association does not equal causation. Unmeasured familial confounding is a common bias in epidemiological studies, whereby shared genetic or environmental factors within families produce spurious associations between risk factors and outcomes. In this Viewpoint, aimed at clinicians from a range of disciplines working with children and young people with neurodevelopm

The impact of a sleep extension intervention on child health-related quality of life.

OBJECTIVE: To examine the effect of a brief behavioral intervention aimed at optimizing sleep duration in children on their overall health-related quality of life (HRQoL), and within physical and psychosocial health domains. METHODS: Participants were 66 typically developing children (68.2% female), aged 8-11 years, identified as short sleepers (≤9.5 h/night), randomized to a sleep intervention (n = 33) or control (n = 33) group, and who reported on HRQoL. The intervention group was provided with behavioral strategies to increase their time in bed by approximately 1-1.5 h/night; those randomi

Cesarean delivery on child health and development in Japanese nationwide birth cohort.

The long-term effects of cesarean delivery (CD) on child health and development remain controversial. This study aimed to investigate these effects using an outcome-wide approach in a Japanese context, where perinatal mortality rates are among the world's lowest. We analyzed data from 2,114 children in a nationwide Japanese birth cohort, linking the 21st Century Longitudinal Survey of Newborns with the Perinatal Research Network database. We examined associations between CD and various health and developmental outcomes up to 9 years of age, including hospitalizations, obesity, and developmenta…

The intersection of food insecurity and child health: Implications for policy and practice in the Bronx.

BACKGROUND: Food insecurity, a significant stressor for many US families and children, is strongly associated with poverty and increases the risk of adverse physical and mental health outcomes. Communities like the Bronx, disproportionately affected by the COVID-19 pandemic due to job losses and increased reliance on emergency food resources, experience heightened food insecurity and face increased risk of various health conditions, such as asthma. This study aims to investigate the relationship between household food insecurity risk and child health outcomes in the Bronx. METHODS: A cross-se…

The impact of poverty on child malnutrition and health in Lebanon: the need for multisectoral interventions.

BACKGROUND/OBJECTIVES: Poverty and child malnutrition are deeply interconnected. Socioeconomic disparities limit access to adequate nutrition, healthcare, and education, and can lead to long-term developmental and economic consequences. This study examines the relationship between poverty and nutritional status, micronutrient deficiencies, and recent illnesses in children in Lebanon. METHODS: Data was collected as part of a national stratified cross-sectional household-based survey conducted in 2023. A total of 1,594 children aged 0-59 months were included in the analysis. Child health and psychosocial wellness in the context of maternal role overload and depression.

OBJECTIVE: The objective of this study was to examine the variability of child health and psychosocial outcomes in the context of maternal role overload and maternal depression. BACKGROUND: Role overload, characterized as time-based role stress, is commonly linked to depression in mothers. However, it remains uncertain whether maternal role overload poses a risk for negative child health and wellness. Although maternal depression is a known risk factor for poor child health and psychosocial outcomes, the outcomes themselves often reveal variability. METHOD: This study aimed to investigate th…

Voter Support for Policies Associated With Child Health as National Campaign Priorities.

IMPORTANCE: Policies that are associated with child health are rarely included in platforms of candidates for national political office. Candidates may underrecognize voter support for such priorities or perceive that such policy issues are not sufficiently divisive to appeal to partisan voters. Key policy questions associated with child health may be considered by the next Congress, including the consistency of Medicaid coverage across states and restoring the recently lapsed refundable child tax credit. OBJECTIVE: To examine voter support for candidates regarding policies that are associate…

Policy solutions to eliminate racial and ethnic child health disparities in the USA.

Societal systems act individually and in combination to create and perpetuate structural racism through both policies and practices at the local, state, and federal levels, which, in turn, generate racial and ethnic health disparities. Both current and historical policy approaches across multiple sectors-including housing, employment, health insurance, immigration, and criminal legal-have the potential to affect child health equity. Such policies must be considered with a focus on structural racism to understand which have the potential to eliminate or at least attenuate disparities. Policy ef…
