Comprehensive evaluation of chloroquine-resin complexes: functional, biopharmaceutical and antimalarial performance.

Malaria remains a major global health burden, especially in children under the age of five. Pediatric treatment often requires tablet fragmentation for its dose adjustment according to body weight, which intensifies bitterness and may reduce treatment adherence. Complexation of bitter-tasting drugs with ion-exchange resins for taste masking is a promising strategy to improve palatability. Here, we evaluated whether a chloroquine-resin complex (CLQ-R) preserves the pharmacokinetic profile and antimalarial efficacy of chloroquine diphosphate (CLQ) while masking its bitter taste. CLQ-R was prepar

Chloroquine degradation under Fenton-assisted electron beam irradiation in aqueous solution.

The removal of pharmaceuticals in the wastewater has been a tough problem. Herein, the Fenton-assisted electron beam radiation was employed to remove chloroquine (CQ) in aqueous solution. The results showed that electron beam removed 63% CQ at 1.5 kGy with •OH, eaq⁻, and H• contributing to its degradation. Additionally, the presence of CO32-, HCO3-, NO3-, Fe2+, and humic acid decreased the degradation efficiency. Furthermore, the presence of S2O82- improved efficiency due to the generation of SO4•⁻ in addition to •OH. The solely Fenton process removed 57% CQ with 2 mM H2O2 and 0.4 mM Fe(II) af

Clinical Toxicological Follow-Up Analysis of a Suicide Attempt Using Chloroquine.

Chloroquine, a drug approved for the treatment of malaria, is frequently used to commit suicide. We report about a suicide attempt by ingesting a high dose of chloroquine in combination with other drugs. Findings of the emergency toxicology screening of blood and urine and those of the follow-up analyses in blood are discussed. Systematic toxicological analysis approaches revealed the presence of chloroquine, butylscopolamine, cafedrine, diazepam, lorazepam, metoclopramide, nordazepam, norephedrine and 11-nor-9-carboxy-∆9-tetrahydroxycannabinol in blood and/or urine of the subject. Suicide due

Berbamine Reduces Chloroquine-Induced Itch in Mice through Inhibition of MrgprX1.

Chloroquine (CQ) is an antimalaria drug that has been widely used for decades. However, CQ-induced pruritus remains one of the major obstacles in CQ treatment for uncomplicated malaria. Recent studies have revealed that MrgprX1 plays an essential role in CQ-induced itch. To date, a few MrgprX1 antagonists have been discovered, but they are clinically unavailable or lack selectivity. Here, a cell-based high-throughput screening was performed to identify novel antagonists of MrgprX1, and the screening of 2543 compounds revealed two novel MrgprX1 inhibitors, berbamine and closantel. Notably, berb

Chloroquine paradox may cause more damage than help fight COVID-19.

Novel coronavirus disease 2019 (COVID-19) pandemic is the most recent health care crisis without specific prophylactic or therapeutic drugs. Antimalarial drug chloroquine (CHL) and its safer derivative hydroxychloroquine (HCHL) have been proposed to be repurposed to treat SARS coronavirus-2 (SARS-CoV-2), the causative agent of COVID-19. CHL/HCHL have anti-inflammatory activity and are used to treat rheumatoid arthritis, osteoarthritis and lupus. Although, CHL/HCHL have an anti-viral activity against several viruses in cell-cultures, the anti-viral activity in-vivo is questionable. Repurposing

No Plasmodium falciparum Chloroquine Resistance Transporter and Artemisinin Resistance Mutations, Haiti.

We obtained 78 human blood samples from areas in Haiti with high transmission of malaria and found no drug resistance-associated mutations in Plasmodium falciparum chloroquine resistance transporter and Kelch 13 genes. We recommend maintaining chloroquine as the first-line drug for malaria in Haiti. Artemisinin-based therapy can be used as alternative therapy.

Antiplasmodial activity of new 4-aminoquinoline derivatives against chloroquine resistant strain.

Emergence and spread of multidrug resistant strains of Plasmodium falciparum has severely limited the antimalarial chemotherapeutic options. In order to overcome the obstacle, a set of new side-chain modified 4-aminoquinolines were synthesized and screened against chloroquine-sensitive (3D7) and chloroquine-resistant (K1) strains of P. falciparum. The key feature of the designed molecules is the use of methylpiperazine linked α, β(3)- and γ-amino acids to generate novel side chain modified 4-aminoquinoline analogues. Among the evaluated compounds, 20c and 30 were found more potent than CQ agai

[Death following administration of 1,250 mg chloroquine in porphyria cutanea tarda].

The death of a 23-year-old female patient is reported. She was being treated for porphyria cutanea tarda (p.c.t.) and was mistakenly given a single dose of 1250 mg chloroquine (instead of the intended dose of 125 mg). The course differed from the typical course of chloroquine toxification, which involves cardiac arrhythmias as the main complication (reported in adults only after much higher doses); in spite of intensive care, this patient died 13 days after the ingestion of chloroquine, in a coma caused by irreversible, acute decompensation of the liver. Forensic autopsy and histological and t

Chloroquine mitigates long-term effects of in vitro culture in mouse embryos.

BACKGROUND: In vitro culture of preimplantation embryos may increase the risk of long-term effects, such as obesity and metabolic diseases later in life in the offspring. While the long-term consequences of low-protein diets during early development have been reported in the context of DOHaD (Developmental Origins of Health and Disease) theory, the relationship between nutrient supply via autophagy during preimplantation development and these long-term effects remains unclear. In this study, we aimed to determine whether autophagy activity during in vitro culture of mouse embryos contributes t

A chloroquine sensitivity gradient induces tissue regeneration and maintenance phenotypes in planarians.

Failure to properly regenerate and maintain tissues is a root cause of disease and aging. One goal of regenerative biology is to unveil mechanisms by which simpler model organisms successfully regenerate so that we can better understand how to control the cellular and molecular switches in human tissues. Planarians have become a favored model system for these studies because of their amazing capacity to fully and rapidly regenerate following amputation, but also for their response to other stressors such as starvation. Planarians undergo proportional degrowth when starved, a unique process tha

Antipruritic Effect of 2,3-Dehydrosilybin in Glial Cells and Chloroquine-Treated Mice.

Pruritus is an inflammatory skin disorder that reduces the patient's quality of life. Meanwhile, 2,3-dehydrosilybin (DHS) belongs to a class of flavonolignans derived from milk thistle seeds, and is known to have anticancer, hepatoprotective, antioxidant, and angiogenic effects. In the current study, the inhibitory effect of DHS on pruritus was investigated in lipopolysaccharide (LPS)-stimulated microglia, IL-31- and IL-6-treated astrocytes, and chloroquine-treated mice. DHS was shown to suppress pruritus-related cytokines IL-31 and IL-6 in LPS-treated microglia. Moreover, DHS inhibited activa

Risk of flare in patients with SLE in remission after hydroxychloroquine or chloroquine withdrawal.

OBJECTIVE: Previous studies have provided evidence that the discontinuation of hydroxychloroquine (HCQ), and chloroquine (CQ), in patients with systemic lupus erythematosus (SLE) is associated with an increased risk of disease flares, with limited information on the level of disease activity at the time of HCQ/CQ discontinuation. Here we aimed to describe the risk of SLE flare after withdrawal of HCQ or CQ in patients with SLE in remission. METHODS: Case-control study (1:2) comparing the evolution of patients with SLE after HCQ/CQ withdrawal for antimalarial retinopathy (cases) with patients

New Iron Twist to Chloroquine─Upgrading Antimalarials with Immunomodulatory and Antimicrobial Features.

Herein, upgraded chloroquine (CQ) derivatives capable of overcoming Plasmodium resistance and, at the same time, suppressing excessive immune response and risk of concurrent bacteremia were developed. Twelve new ferrocene-CQ hybrids tethered with a small azathia heterocycle (1,3-thiazolidin-4-one, 1,3-thiazinan-4-one, or 5-methyl-1,3-thiazolidin-4-one) were synthesized and fully characterized. All hybrids were evaluated for their in vitro antiplasmodial, antimicrobial, and immunomodulatory activities. Additional assays were performed on selected hybrids to gain insights into their mode of acti
