[Vibrio cholerae strains certification].

AIM: Certification of V. cholerae strains stored at State collection of pathogenic microorganisms and cell cultures SCPM - Obolensk. MATERIALS AND METHODS: 50 V. cholerae strains were studied. Real-time PCR with primers to genes ctxA, ctxB, ace, zot, tcpA, toxR, hlyA, rtxC, rfbO1, ompU, ompW was used. RESULTS: Membership of the studied strains in V. cholerae species was confirmed by molecular-biological methods. 46 strains belong to O1 serogroup, 42 of those - E1 Tor toxigenic, having all the virulence genes and 4 non-toxigenic strains. A strain had ace, zot, tcpA, toxR, rtxC, hlyA, ompU gen…

Implications of the introduction of cholera to Haiti.

With more than 250,000 cases and 4,000 deaths in the first 6 months, the cholera epidemic in Haiti has been one of the most explosive and deadly in recent history. It is also one of the best documented, with detailed surveillance information available from the beginning of the epidemic, which allowed its spread to all parts of the country to be traced. Piarroux et al. make good use of this information, along with their own careful field investigations, to trace the epidemic to its beginning and propose an explanation for its origins.

Genomic evolution of Vibrio cholerae.

Vibrio cholera, the causal agent of cholera, also occupies an autochthonous aquatic inhabitant. The current, seventh cholera pandemic is linked to O1 El Tor biotype and O139 serogroups. In the last decades, we have witnessed a shift involving genetically and phenotypically varied pandemic clones in Asia and Africa. Recent comparative genomic studies have identified a large 'mobilome', or composed of mobile genomic islands in V. cholerae. All seventh pandemic isolates have highly related genome sequences, but they can be differentiated by set of these genomic islands. A consequence of the exten…

Bacteriocin Typing of Vibrio cholerae.

Bacteriocins of Vibrio cholerae have been demonstrated against enterobacterial and vibrio indicator organisms by conventional techniques. Abundant bacteriocin production took place on casein hydrolysate-yeast extract, tryptic soy, digest broth, proteose-peptone, and neopeptone agars. Essential factors were a citrate-phosphate buffer concentration of 0.5 to 0.7%, at pH 7.5 to 7.6, and cold shock. Thermal treatment of indicator organisms at 45 C for 12 min increased the percentage of typable strains. The bacteriocins of V. cholerae appeared to be powerful diffusible bactericidal agents. By using…

Cholera in Africa: A Climate Change Crisis.

BACKGROUND: Cholera, an acute diarrheal infection caused by Vibrio cholerae, remains a significant public health concern globally, with 1.4-4.0 million cases and 21,000-143,000 deaths annually. While the disease is endemic in 47 less-developed countries across Africa and Asia, the African continent has been particularly affected, with 19 of 29 countries reporting cases in 2023 being from Africa. AIM: To explore the trend of cholera outbreaks in Africa and analyze how climate change has contributed to the spread of the disease in the continent. METHODS: A review of current cholera outbreaks i…

Allocation of Oral Cholera Vaccines in Africa.

OBJECTIVES: In this study, we examine the allocation of oral cholera vaccines (OCVs) across 25 African countries between 2013 and 2019. METHODS: We constructed a dataset combining cholera outbreaks and requests, decisions, and deliveries of OCVs from the Global Task Force on Cholera Control, alongside additional covariates. Using machine learning algorithms, we assess the relative importance of socio-demographic, governance, and weather variables in predicting cholera outbreaks. We constructed and used an "index of cholera risk" as an instrumental variable to predict the likelihood of suspect…

Enhanced Risk for Epidemic Cholera Transmission, Haiti.

Sporadic cholera outbreaks continue to occur in Haiti. We used a novel space-time analysis to gain insight from limited government surveillance data. We identified concerning patterns of disease spread in areas known to be at high risk for epidemic cholera in and around the capital city of Port-au-Prince.

Implementation of targeted cholera response activities, Cameroon.

OBJECTIVE: To describe the implementation of case-area targeted interventions to reduce cholera transmission using a rapid, localized response in Kribi district, Cameroon. METHODS: We used a cross-sectional design to study the implementation of case-area targeted interventions. We initiated interventions after rapid diagnostic test confirmation of a case of cholera. We targeted households within a 100-250 metre perimeter around the index case (spatial targeting). The interventions package included: health promotion, oral cholera vaccination, antibiotic chemoprophylaxis for nonimmunized direct…

The Cholera Outbreak in Lebanon: October 2022.

In recent years, Lebanon has been struggling with a socioeconomic crisis exacerbated by population displacement after the Syrian crisis, which put a significant burden on its healthcare system. An additional challenge has been the response to a cholera outbreak- a deadly waterborne disease transmitted through the fecal-oral route that usually manifests as severe watery diarrhea and can rapidly progress to death. After reports of a cholera outbreak in Syria were disclosed in September 2022, the Northern Governorate of Lebanon too began reporting cases immediately after, and the first case was c…

Anticipatory decision-making for cholera in Malawi.

Climate change raises an old disease to a new level of public health threat. The causative agent, Vibrio cholerae, native to aquatic ecosystems, is influenced by climate and weather processes. The risk of cholera is elevated in vulnerable populations lacking access to safe water and sanitation infrastructure. Predictive intelligence, employing mathematical algorithms that integrate earth observations and heuristics derived from microbiological, sociological, and weather data, can provide anticipatory decision-making capabilities to reduce the burden of cholera and save human lives. An example …

Diagnosis, Management, and Future Control of Cholera.

Cholera, caused by Vibrio cholerae, persists in developing countries due to inadequate access to safe water, sanitation, and hygiene. There are approximately 4 million cases and 143,000 deaths each year due to cholera. The disease is transmitted fecally-orally via contaminated food or water. Severe dehydrating cholera can progress to hypovolemic shock due to the rapid loss of fluids and electrolytes, which requires a rapid infusion of intravenous (i.v.) fluids. The case fatality rate exceeds 50% without proper clinical management but can be less than 1% with prompt rehydration and antibiotics.…

Contribution of microbial genomics to cholera epidemiology.

In 2022, the burden of cholera-an acute watery diarrheal disease caused by Vibrio cholerae serogroup O1 (or more rarely O139) bacteria, which produce cholera toxin-remains high in many African and Asian countries. In the last few years, microbial genomics has made it possible to define the bacterial populations responsible for cholera more precisely. It has been shown that the current, seventh pandemic is due to a single lineage with a reservoir in the countries of the Bay of Bengal (India and Bangladesh). There have been several transmissions of the causal agent of cholera from this region to…
