An 80-year-old man was evaluated for an epibulbar tumor on a phthisical eye. The initial biopsy diagnosis of the epibulbar tumor was poorly differentiated neoplasm. Exenteration of the phthisical eye and orbital contents showed an extensive pleomorphic adenocarcinoma of the nonpigmented epithelium of the ciliary body with extraocular extension. There was evidence of hyaluronic acid secretion and immunohistochemical staining was strong for vimentin, focal for epithelial membrane antigen and S-100 protein, and weak for neuron-specific-enolase. Long-term changes in the vascular network of rabbit ciliary processes induced by Nd:YAG laser cyclophotocoagulation were examined both from a morphological viewpoint and with respect to three-dimensional organization (in vascular casts). Relative changes in intraocular pressure (IOP), monitored within eyes irradiated across one-quarter, one-half, three-quarters, or all of the ciliary body circumference, are discussed in relation to morphological changes. The clinical implications of these findings are considered.

There are three forms of lens dislocation in the posterior segment: intravitreal, prebasal, and intrabasal. The location of the dislocated lens depends on gravity and the condition of the vitreous body. As a rule the dislocated lens lies at the bottom of the vitreous cavity; it may be completely embedded in the vitreous substance or completely outside the vitreous body, or partially within the vitreous base. The topographic relations between dislocated lenses and the vitreous body cannot be determined by ophthalmoscopic examination, but biomicroscopy generally permits an accurate assessment. B

Experiments were undertaken to determine if serotonin (5-HT) radioligand binding sites were present in a membrane fraction of iris + ciliary body from adult, albino rabbits. The total binding of 3H-5-HT, 3H-spiroperidol (SPI) and 3H-ketanserin (KET), all at 2 nM, was determined in the absence and in the presence of ketanserin, mianserin, methysergide or 5-methoxytryptamine (5-MT), all at 1 microM. In a retrospective study, 50 (8.2%) of 610 patients with malignant melanoma of the ciliary body or choroid had extrascleral extension. Of 46 patients for whom follow-up data were available, 24 (52%) survived five years, 20 (42%) survived ten years, and 17 (37%) survived 15 years. Factors that seemed to affect survival significantly included intraocular tumor size and extraocular tumor size and location. Our observations did not support the role of primary exenteration. A large survey was conducted to identify patients with choroidal and ciliary body melanomas in whom preoperative evaluation for the presence of metastases was performed. Twenty-nine (2.47%) of 1,214 patients were found to have metastases prior to surgical manipulation of the globe. Liver function enzymes combined with liver-spleen scanning is the most effective means for screening for the presence of metastases in these patients and should be performed in all cases in which surgical manipulation of the globe is anticipated.

Fluorescein gonioangiography was performed in 4 eyes of 4 patients with ocular sarcoidosis and in an eye of a patient with presumptive tuberculous anterior uveitis. Small discrete whitish lesions in the ciliary body band near the iris root showed hyperfluorescent shortly after fluorescein injection. These hyperfluorescence dots seemed to be pathognomonic changes in cases with granulomatous uveitis.

PURPOSE: To report a novel approach for removal of intraocular foreign body in the ciliary sulcus.
OBSERVATIONS: A 72-year-old male presented with an intraocular foreign body embedded in the ciliary body, localized with ultrasound biomicroscopy. An intraoperative endoscopic camera was then used to directly visualize the foreign body in the ciliary body and the object was successfully removed with intraocular forceps.
