Delayed internal fixation of fractures of the neck of the femur in young adults. A prospective, randomised study comparing closed and open reduction.

We have compared the results and complications after closed and open reduction with ternal fixation in young adults with displaced intracapsular fractures (Garden grades III and IV) of the neck of the femur. We also studied the risk factors which influenced nonunion and the development of avascular necrosis (AVN). A total of 102 patients aged between 15 and 50 years was randomised to receive either closed or open reduction. Both groups were compared for age, gender, time to surgery and posterior comminution as well as for union and complications. Using univariate and multivariate analysis the

Double-blind randomized prospective study of the efficacy of antibiotic prophylaxis for open reduction and internal fixation of closed ankle fractures.

Antibioprophylaxis has been proven to be efficient for some orthopaedic procedures. However, its efficacy for clean limited procedures with metallic implants is not clear. One hundred twenty-two closed ankle fracture patients undergoing open reduction and internal fixation were randomized, double-blindly, to receive either cephalothin (1 g i.v. every 6 h x four doses) or a placebo starting before tourniquet application. Mean trauma-surgery delay was 30.2 h, and average tourniquet duration was 65.4 min. Four patients (three of 62 on placebo and one of 60 on cephalothin) developed a superficial

Prospective evaluation of a pragmatic treatment rationale: open reduction and internal fixation of displaced and dislocated condyle and condylar head fractures and closed reduction of non-displaced, non-dislocated fractures Part II: high condylar and condylar head fractures.

This study prospectively evaluated closed reduction (CR) outcomes in non-displaced, non-dislocated high-condylar and condylar-head fractures (Class VI after Spiessl and Schroll) and open reduction and internal fixation (ORIF) of displaced (Class III) or dislocated (Class V) fractures. Thirty-eight patients with 54 fractures (16 (42%) with bilateral fractures, 14 (37%) CR, 24 (63%) ORIF) were enrolled in a 1 year follow-up that 18 patients with 33 fractures completed. Condylar translation in Class VI fractures recovered to 11 mm for vertical opening, 8mm for protrusion and 10 mm for mediotrusio

Volar Carpometacarpal Dislocation Associated with Fracture of the Second, Third, and Fourth Metacarpal Bases Treated by Closed Reduction and Percutaneous Fixation: A Case Report.

INTRODUCTION: Volar dislocations of the carpometacarpal (CMC) joints are notably infrequent, attributable to the inherent stability afforded by their osseous morphology and robust volar ligamentous restrictions. When combined with fractures of two or more metacarpal bases, diagnosis and treatment may be delayed. CASE REPORT: A 43-year-old male patient presented with volar dislocation of multiple CMC joints and metacarpal fractures, involving the bases of the second, third, and fourth metacarpals, subsequent to a low-energy fall from a bicycle. The injury was evaluated utilizing plain radiogra

Comparative study of recovery characteristics between remimazolam anesthesia with flumazenil and desflurane anesthesia for closed reduction of nasal bone fracture: a randomized controlled trial.

BACKGROUND: In otorhinolaryngologic surgery, immediate and smooth emergence from general anesthesia is important for patient safety. Therefore, we compared the recovery profile of desflurane to that of remimazolam with flumazenil reversal in patients undergoing nasal bone fracture reduction surgery. METHODS: In this randomized controlled trial, patients were randomly assigned to the Desflurane (n=29) or Remimazolam group (n=29), which received desflurane or remimazolam, respectively, for anesthesia maintenance. Flumazenil (0.2 mg) was administered at the end of surgery in the Remimazolam grou

The effect of early controlled mobilization combined with target focused exercise regime in management of closed reduction K-wire fixation of proximal phalanx fracture.

The phalangeal and metacarpal fractures are common fractures of the upper extremity. The most common complications of proximal phalanx fracture are the extensor lag and stiffness at the proximal interphalangeal joint, which leads to poor outcomes. Poor treatment compliance has also been identified among our patients. The target focused exercise regime with early controlled active mobilization should improve outcomes and treatment compliance among our patients. In this observational study with recruited 13 subjects, subjects age group range from 25 to 55 yrs. Baseline assessment was administrat

Comparison of emergence agitation between succinylcholine and rocuronium-sugammadex in adults following closed reduction of a nasal bone fracture: a prospective randomized controlled trial.

BACKGROUND: Sugammadex allows rapid recovery from rocuronium-induced neuromuscular blockade. Succinylcholine is often used for brief surgeries but is associated with myalgia, headache, histamine release, and increased lactate levels. Thus, we hypothesized that succinylcholine may affect emergence agitation (EA) and compared the effects of succinylcholine and rocuronium-sugammadex on EA in patients undergoing closed reduction of a nasal bone fracture under general anesthesia. METHODS: Forty-two patients were prospectively enrolled and allocated randomly to the succinylcholine group (group SC)

Closed reduction and percutaneous pinning vs open reduction and internal fixation in pediatric lateral condylar humerus fractures displaced by > 4 mm: an observational cross-sectional study.

BACKGROUND: Although open reduction and internal fixation (ORIF) is recommended for lateral condylar humerus fractures (LCHFs) displaced by > 4 mm, several studies have reported the use of closed reduction and percutaneous pinning (CRPP) to treat LCHFs with significant displacement. However, little is known about the clinical differences between these two surgical techniques. This study aimed to compare the therapeutic effects of CRPP and ORIF in treating LCHFs displaced by > 4 mm. METHODS: We retrospectively reviewed pediatric LCHFs displaced by > 4 mm treated with either CRPP or ORIF at our

Outcomes After Surgically Managed Oblique Extra-Articular Proximal Phalanx Fractures: A Comparison of Closed-Reduction Percutaneous Pinning and Open-Reduction Internal Fixation With Lag Screws.

BACKGROUND: Long oblique extra-articular proximal phalanx fractures are common orthopedic injuries. When unstable and without substantial comminution, treatment options include closed-reduction percutaneous pinning (CRPP) and open-reduction internal fixation using lag screws (ORIF-screws). The aims of this study are primarily to compare the functional outcomes and complication rates between these techniques and secondarily to assess potential factors affecting outcomes after surgery. METHODS: All patients with long oblique extra-articular proximal phalanx fractures treated surgically within a
