A Clinical Conundrum: Prophylactic Metabolic Bariatric Surgery for People Without Obesity?

Revisional metabolic and bariatric surgery (MBS) in patients with a body mass index (BMI) below the obesity threshold (> 30 kg/m2) presents complex clinical and ethical challenges. Indications for reoperation vary widely, from acute complications like gastric band slippage to chronic metabolic disorders such as persistent hypoglycemia after Roux-en-Y gastric bypass (RYGB). While some revisional procedures, like elongation of the common limb for malabsorption-related complications, are widely accepted, other interventions remain controversial. The introduction of a distinct ICD code could help

Rural Perinatal Loss: A Needs Assessment.

The purpose of this research was to ascertain the availability and depth of services of bereavement care for mothers who live rurally. The specific focus is on those who experienced early losses including pregnancy, stillbirth, neonatal, and young children who were born with fetal anomalies or neonatal disease that resulted in death. The convenience (nonprobability) sample originated from a population of mothers who lived in rural east central Minnesota. Participants were interviewed in a 60-minute interval. All data were coded confidential. Common themes, incidence of resources, or lack of be

The human genome sequence: impact on health care.

The recent sequencing of the human genome, resulting from two independent global efforts, is poised to revolutionize all aspects of human health. This landmark achievement has also vindicated two differeint methodologies that can now be used to target other important large genomes. The human genome sequence has revealed several novel/surprising features notably the probable presence of a mere 30-35,000 genes. In depth comparisons have led to classification of protein families and identification of several orthologues and paralogues. Information regarding non-protein coding genes as well as reg

[Genetics of hypertrophic cardiomyopathies: what are the clinical perspectives?].

Considerable progress has been accomplished in the understanding of the genetics of familial hypertrophic cardiomyopathy over the last five years. This is a monogenic autosomal dominant condition with much inter- and intragenic variability. Four morbid genes coding for the contractile proteins have been identified: the heavy beta chain of cardiac myosin (14q11), cardiac troponin T (1q3), alpha tropomyosin (15q2), and myosin binding cardiac C protein (11p13-q-13). In addition, other genetic localisations not yet identified are certain. Moreover, many mutations have been reported in the already

High-Intensity Functional Concurrent Training for Physical Fitness, Body Composition, and Psychological Outcomes in Schoolchildren: Protocol for a Randomized Controlled Trial.

High-intensity functional concurrent training (HIFCT) has emerged as a form of training characterized by constantly varied functional movements adapted to individual fitness levels. Previous studies have reported positive effects on muscular strength, cardiorespiratory fitness, body composition, and psychological well-being; however, evidence regarding HIFCT interventions in school-aged children remains limited. Therefore, the aim of the present study protocol is to evaluate the effects of an 8-week HIFCT programme on muscular strength, cardiorespiratory fitness, body composition, anxiety, str

From technology anxiety to technology symbiosis: a grounded theory study of mid-career foreign language teachers' subjectivity evolution in the AI era.

The integration of generative AI into foreign language education has profoundly challenged teacher professional identity, particularly for mid-career educators navigating the dual burden of experiential wisdom and technological change. This study employed grounded theory to investigate how 10 mid-career foreign language teachers transition from technology anxiety to symbiosis. Through in-depth interviews and systematic coding, we developed a theoretical model comprising four anxiety types (substitution, autonomy, generational, ethical), three adaptive stages (exploration adjustment, strategy i

Empowering youth with disabilities through community-based approaches: a scoping review protocol on livelihood and social inclusion.

INTRODUCTION: Youth with disabilities in low- and middle-income countries (LMICs) face persistent exclusion from education, employment and social participation. Those affected by leprosy often experience additional stigma, discrimination and structural barriers that further restrict empowerment, livelihood opportunities and social inclusion. Community-based approaches, including Community-Based Rehabilitation (CBR) and Community-Based Inclusive Development, aim to promote inclusion by empowering individuals and addressing barriers. However, evidence on how these approaches contribute to empowe

Relationships between alcohol use and dementia: protocol for an observational study in the UK Clinical Practice Research Datalink.

INTRODUCTION: Alcohol consumption is an increasingly recognised modifiable risk factor for dementia, yet whether it has differential impacts on dementia subtypes and its role in disease progression remains unclear. This study aims to: (1) quantify the association between alcohol intake and incidence of dementia subtypes and (2) examine whether individuals who drink heavily and develop dementia referred to hereafter as 'alcohol-related'-have poorer post-diagnosis outcomes compared with other dementia cases. Clarifying these relationships will determine whether alcohol selectively increases risk

Dynamic prediction of cardiovascular risk from linked primary care records in New South Wales, Australia: protocol for a retrospective prognostic modelling study.

INTRODUCTION: Cardiovascular disease (CVD) remains a leading cause of preventable morbidity and mortality. Most existing CVD prediction tools use a baseline survival modelling framework, in which each predictor is represented by a single value recorded at the start of follow-up. Although risk calculators can be re-run using updated values, these models do not explicitly use the longitudinal, irregularly recorded information in primary-care electronic medical records (EMRs). Dynamic modelling approaches have the potential to generate updated CVD risk estimates at each general practitioner (GP)

Examining two pedagogical methods on attitudes toward religiousness within mental health and social justice in an undergraduate course.

Integrating spirituality and religiousness (S/R) into mental health (MH) treatment aligns with our ethics codes and professional guidelines, and it can promote therapeutic outcomes. There is emerging evidence among samples of professional clinicians and graduate students that the Spirituality Competency Training in MH curriculum (SCT-MH; Pearce et al., 2019) may address the dearth of training in this intersection. However, SCT-MH needs to be assessed among undergraduates to support vocational development and those who pursue graduate school or bachelor-level mental health work. In this explora

Chronic kidney disease in preeclamptic patients: not found unless searched for-Is a nephrology evaluation useful after an episode of preeclampsia?

BACKGROUND: Preeclampsia (PE) and chronic kidney disease (CKD) are linked by an only partially known cause-effect relationship. Knowledge on prevalence of CKD in PE patients is needed for evaluating the diagnostic yield of nephrology work-up after PE. METHODS: The study was undertaken in the Centre Hospitalier Le Mans (CHM), setting of tertiary level obstetric service (about 3500 deliveries/year). PE was identified on hospital's discharge codes; after review, the study included 99 patients, 36 of which were also evaluated in Nephrology. A descriptive analysis was performed as appropriate. Log

Content validity of Scoliosis Research Society questionnaire-22 revised (SRS-22r) for adolescents with idiopathic scoliosis: protocol for a qualitative study exploring patient's and practitioner's perspectives.

INTRODUCTION: Content validity is the most important measurement property for any patient-reported outcome measure (PROM). It being the extent that the PROM measures important concepts that are relevant to the population of interest. Adolescent with idiopathic scoliosis (AIS) is the most common spinal deformity in paediatric populations, with the Scoliosis Research Society questionnaire-22 revised (SRS-22r) a commonly used PROMof quality of life. In the absence of existing evidence, a content validity study for SRS-22r is needed to confirm its suitability for AIS. Thus, this study aims to inve

Beyond technical skills: women's expectations regarding the relational dimensions prior to gynecologic examinations.

INTRODUCTION: Gynecologic examinations expose women to particular vulnerability, making relational and ethical aspects essential beyond technical skills. While professional societies outline recommended behaviors, the importance women attribute to these non-technical elements remains insufficiently characterized. This study aimed to assess how women value key relational components of gynecologic examinations. METHODS: In this exploratory cross-sectional study, a questionnaire was developed from gynecologic and ethical guidelines, and distributed via QR codes in outpatient gynecology and gener
