Cognitive behavior therapy for anxiety disorders: 40 years of progress.

Cognitive-behavior therapies (CBT) have been evaluated in randomized controlled studies (RCT) and anxiety disorders since 1966 and for each disorder there are at least two CBT methods that are considered evidence based (empirically supported) today. Numerous meta-analyses have evaluated the efficacy of these methods against various control conditions. However, none has looked at whether modern CBT studies lead to better treatment effects than were obtained 10-40 years ago. The aim of this paper is to present a meta-analysis focusing on the mean extent of change achieved by the CBT treatments a

Enduring effects for cognitive behavior therapy in the treatment of depression and anxiety.

Recent studies suggest that cognitive and behavioral interventions have enduring effects that reduce risk for subsequent symptom return following treatment termination. These enduring effects have been most clearly demonstrated with respect to depression and the anxiety disorders. It remains unclear whether these effects are a consequence of the amelioration of the causal processes that generate risk or the introduction of compensatory strategies that offset them and whether these effects reflect the mobilization of cognitive or other mechanisms. No such enduring effects have been observed for

[Cognitive behavior therapy of agoraphobia, retrospective study of 345 cases].

Epidemiological data of 533 (42% male) agoraphobic patients referred to an anxiety disorder unit are reported. Comorbidity with mood disorders was high (48%). Therapeutic response was studied in 345 patients who received cognitive-behaviour therapy (CBT): 131 patients dropped out and 214 completed treatment. At post-test 71% of the completers were improved with maintenance of the gains in patients reevaluated at follow-up points ranging from 6 months to 18 years. Medication intake was significantly decreased. Intent to treat analysis of the whole CBT sample showed that 57% of the 345 patients

Comparison of cognitive-behavior therapy and desipramine in the treatment of bulimia nervosa.

A comparison of cognitive-behavior therapy alone, desipramine alone, and cognitive-behavior therapy combined with desipramine was made in the treatment of bulimia nervosa. The study was terminated early with an N of only 7 subjects per condition because of a high drop-out rate and lack of positive response in the desipramine alone group compared to the other two groups. By this time it was also apparent that at posttreatment and at 6 months follow-up no benefit was being realized from combining cognitive-behavior therapy with desipramine.

Behavioral Inhibition and Social Anxiety Disorder as Predictors of Long-Term Outcomes of Cognitive Behavioral Therapy for Youth Anxiety Disorders.

The temperamental trait behavioral inhibition (BI) is related to the development and maintenance of anxiety, particularly much so to social anxiety disorder. We investigated if BI and social anxiety disorder predicted cognitive behavioral therapy (CBT) outcomes for youth anxiety. Youth (N = 179; Mage = 11.6 years) were assessed 4 years following a randomized controlled CBT effectiveness trial. BI was measured by the parent-reported Behavioral Inhibition Questionnaire at baseline. The outcomes were diagnostic recovery, youth- and parent-reported anxiety symptoms, and clinical severity at post-t…

Effectiveness of cognitive behavioral therapy for postpartum depression: a systematic review and meta‑analysis.

BACKGROUND: Postpartum depression (PPD) affects 17.22% of women worldwide and represents a major public‑health concern. Cognitive-behavioral therapy (CBT) is a recommended intervention for PPD. However, evidence regarding its overall efficacy, additional clinical benefits, and real‑world performance across heterogeneous delivery formats remains insufficient. This meta‑analysis quantifies the efficacy of CBT compared with usual care, assesses the certainty of evidence, and descriptively explores intervention‑mode characteristics across included trials. METHODS: We systematically searched PubMe

Trauma-Informed Cognitive Behavioral Therapy for Insomnia in Women Veterans: An Intervention Development Study.

This paper describes the methodology and outcomes of the development of a trauma-informed protocol for cognitive behavioral therapy for insomnia (CBT-I). Methods included (a) convening an expert panel of U.S. Department of Veterans Affairs providers (n = 4) to identify trauma-related symptoms that may interfere with standard CBT-I delivery and to assess trauma-informed adaptations to an existing CBT-I protocol, (b) presenting adapted protocol materials to veteran engagement groups for feedback, and (c) sequentially delivering the trauma-informed CBT-I protocol to women veterans (n = 5) with it…

Digital Cognitive Behavioral Therapy for Older Adults With Symptoms of Depression: Feasibility Cohort Study.

BACKGROUND: Depressive symptoms are common among older adults and can significantly impact their quality of life. However, many older adults face barriers to accessing psychological treatment. Internet-based cognitive behavioral therapy (iCBT) is a promising alternative to face-to-face treatments, but its feasibility among older adults has been less extensively studied than in adult populations. OBJECTIVE: This study evaluated the feasibility of guided iCBT for adults aged 55 years and older with mild to moderate depressive symptoms recruited from the general population. METHODS: This study …

Internet-Delivered Cognitive Behavioral Therapy for Social Anxiety Disorder: Nationwide Retrospective Cohort Observational Study.

BACKGROUND: Internet-delivered cognitive behavioral therapies (iCBTs) can address the accessibility and affordability limitations of conventional therapy. Although iCBTs for social anxiety disorder (SAD) are efficacious, their effectiveness in routine care remains less well established. Helsinki University Hospital (HUS) provides a novel nationwide iCBT program for SAD. OBJECTIVE: We aimed to assess whether this 7-session, flexible time-scheduled, therapist-supported HUS-iCBT for SAD, delivered by a specialized clinic, (1) is effective in routine care and (2) can enhance adherence, compared t

Internet-Based Cognitive Behavioral Therapy Interventions for Caregivers of Patients With Cancer: Scoping Review.

BACKGROUND: Cancer imposes significant physical and emotional distress not only on patients, but also on their caregivers. In recent years, there has been a growing focus on the mental and physical well-being of caregivers. Among various psychological interventions, cognitive behavioral therapy (CBT) is widely recognized as one of the most effective approaches. However, traditional CBT is often limited by time and geographical constraints, resulting in delayed or inefficient support for caregivers. Internet-based cognitive behavioral therapy (ICBT) presents a valuable alternative for alleviati…

Nationwide Implementation of Unguided Cognitive Behavioral Therapy for Adolescent Depression: Observational Study of SPARX.

BACKGROUND: Internet-based cognitive behavioral therapy (iCBT) interventions are effective in clinical trials; however, iCBT implementation data are seldom reported. OBJECTIVE: The objective of this study is to evaluate uptake, adherence, and changes in symptoms of depression for 12- to 19-year-olds using an unguided pure self-help iCBT intervention (SPARX; Smart, Positive, Active, Realistic, X-factor thoughts) during the first 7 years of it being publicly available without referral in Aotearoa New Zealand. METHODS: SPARX is a 7-module, self-help intervention designed for adolescents with mi…

Assessment of Quality and Reliability of YouTube Videos on Cognitive-Behavioral Therapy including Hypnosis.

Hypnosis combined with cognitive behavioral therapy (CBT-hypnosis) is a type of psychological treatment that focuses on how people think and behave in various mental and medical illnesses. It treats behavioral and emotional issues by tapping into the subconscious mind. Patients who are hypnotized are more open to new ideas and less prone to decline hard ones. The result is that it is simpler to adopt the healthy cognitive patterns and habits that CBT tries to promote. YouTube is a great resource for health-related education that has the power to greatly impact the choices and actions of medica…

Neurocognitive performance in obsessive-compulsive disorder before and after treatment with cognitive behavioral therapy.

BACKGROUND: Cross-sectional studies have reported neurocognitive performance deficits in obsessive-compulsive disorder (OCD), particularly on tasks assessing response inhibition and proactive control over stimulus-driven behaviors (task control). However, it is not clear whether these deficits represent trait-like markers of OCD or are state-dependent. METHODS: This study examined performance on two neurocognitive tasks in OCD patients (N = 26) before and after cognitive behavioral therapy (CBT) and matched healthy controls (HCs, N = 19). Tasks included the stop-signal task (assessing respons…

Lifetime Potentially Traumatic Events and Cognitive-Behavioral Therapy Outcomes in Pediatric Obsessive-Compulsive Disorder.

OBJECTIVE: This study evaluated impacts of traumatic stress, a social determinant of health, on cognitive-behavioral therapy (CBT) outcomes in pediatric obsessive-compulsive disorder (OCD). Closing this literature gap is crucial for optimizing individual treatment approaches in pediatric OCD and limiting misinterpretation of OCD symptoms in the context of a trauma history. METHOD: An evidence-based CBT group program was completed by 98 OCD-affected youths (8-19 years old). Lifetime potentially traumatic events (PTEs) were assessed at baseline by type and cumulative load with the Traumatic Eve

Screening system for assessing suitability of cognitive behavioral therapy for chronic low back pain.

An objective method to evaluate patient suitability for cognitive behavioral therapy (CBT) for chronic low back pain (LBP) is currently lacking. Inappropriate application can result in prolonged hospital visits and increased medical costs. Therefore, identifying an objective biomarker for evaluating suitability is crucial. This study focused on electroencephalogram (EEG) complexity as a potential biomarker for evaluating CBT suitability for chronic LBP, assessing its discriminative ability and identifying factors that impede treatment. Complexity was analyzed as multiscale fuzzy sample entropy…

The provision of cognitive behavioral therapy in Japan: an analysis using insurance claims data.

BACKGROUND: Cognitive behavioral therapy (CBT) is a first-line treatment for many mental health conditions. In Japan, patients can receive health insurance coverage for CBT treatment of major depressive disorder (MDD), social anxiety disorder (SAD), panic disorder (PD), obsessive compulsive disorder (OCD), post-traumatic stress disorder (PTSD), and bulimia nervosa; however, utilization of CBT remains insufficient. This study aimed to examine the status of CBT provision for these disorders using Japanese health insurance claims data. METHODS: Data for April 2015-March 2022 from the Detroit Emp…

Metacognition in Children and Adolescents With Obsessive-Compulsive Disorder Treated With Cognitive Behavioral Therapy.

According to the metacognitive theory, maladaptive metacognition is associated with the development and maintenance of emotional disorders. This study is the first to explore maladaptive metacognition in a sample of children and adolescents (7-17 years) with obsessive-compulsive disorder (OCD) in the context of cognitive behavioral therapy (CBT). A total of 114 children and adolescents were included in the study. Of these 56 were OCD patients who were assessed on metacognition (Metacognitions Questionnaire-Child Version) and OCD symptom severity (Children's Yale-Brown Obsessive Compulsive Scal…
