Radioimmunoguided surgery for recurrent colorectal cancer manifested by isolated CEA elevation.

BACKGROUND: Carcinoembryonic antigen (CEA) is a sensitive marker for detecting recurrent colorectal carcinoma. An asymptomatic rise of CEA can precede by several months the detection of recurrent cancer by standard imaging modalities. Yet, surgeons are hesitant to operate solely on the basis of an observed increase in CEA. We investigated the ability of radioimmunoguided surgery to enhance the surgeon's capability of detecting intraabdominal disease in these patients. METHODS: Nineteen patients who underwent radioimmunoguided surgery for suspected tumor recurrence based solely on elevated CEA

Can flexible sigmoidoscopy predict need for colorectal surgery in ovarian carcinoma?

INTRODUCTION: It is difficult to predict which patients with ovarian cancer will require bowel surgery. We propose that sigmoidoscopy performed by an experienced colorectal surgeon could predict the need for bowel resection as part of optimum cytoreduction by assessment of rigidity and encasement of the rectosigmoid colon or mucosal involvement. Laparotomy may then be performed electively in collaboration with a colorectal surgeon after administration of bowel preparation. METHODS: In a prospective study 30 patients undergoing surgery for a high suspicion of ovarian malignancy and with at lea

β-Lactam vs Non-β-Lactam Prophylaxis in Elective Colorectal Surgery.

IMPORTANCE: Previous studies found reduced surgical site infections (SSIs) with β-lactam surgical infection prophylaxis compared with non-β-lactam alternatives; however, variations in timing and dosing have been reported. Whether guideline-concordant preoperative antibiotic dosing or timing are associated with reported SSI differences between regimens is unclear. OBJECTIVE: To evaluate the incidence of SSIs between patients receiving β-lactam and non-β-lactam alternative prophylaxis, while controlling guideline-concordant dosing and timing. DESIGN, SETTING, AND PARTICIPANTS: This multicenter

Intrathecal Morphine Versus Transversus Abdominis Plane Block in Robotic Colorectal Surgery.

INTRODUCTION: Intrathecal morphine (ITM) and transversus abdominis plane (TAP) block are both used for analgesia after minimally invasive colorectal surgery, but direct comparisons in robotic surgery are limited. We compared them for postoperative opioid consumption and pain after robotic colorectal resection. METHODS: We conducted a two-hospital retrospective cohort study of patients undergoing robotic colorectal resection between January 2022 and December 2023 who received either ITM or TAP block. Propensity score matching (1:1) balanced age, sex, body mass index, American Society of Anesth

Indocyanine Green Fluorescence Imaging for Colorectal Surgery: A Health Technology Assessment.

BACKGROUND: Both malignant and benign conditions may require colorectal surgery. Anastomotic leak is a serious potential complication, and assessing tissue perfusion at the planned site of anastomosis is critical to try to prevent leaks. The approaches used by surgeons to assess anastomotic integrity and tissue perfusion involve visual assessment of the planned resection area. Indocyanine green fluorescence imaging (ICGFI) is a technology that involves the use of a fluorescent dye and a near-infrared imaging system to allow surgeons to visualize tissue perfusion intraoperatively in real time.

Sustained Outcomes After Enhanced Recovery Pathway Implementation in Pediatric Colorectal Surgery.

INTRODUCTION: Previous studies have demonstrated improvements in length of stay and opioid utilization after implementing Enhanced Recovery After Surgery Pathways (ERPs) for pediatric colorectal surgery. However, the long-term impact of ERPs on postoperative outcomes following the implementation of specific ERP components has not been examined. METHODS: In this single-institution retrospective and longitudinal cohort review of pediatric patients undergoing colorectal surgery before (Pre-ERP - 4/2014-8/2015) and after an ERP implementation (ERP - 9/2015-1/2024). Poisson regression was performe

Advanced Protocols for Preoperative Colon Preparation: Enhancing Outcomes in Colorectal Surgery.

Introduction: Since the very beginnings of colorectal surgery, it has been dominated by the effort to reduce postoperative infectious complications and to find a way to prevent the risks associated with the dehiscence of intestinal anastomoses. Despite the technical, technological, and scientific advances achieved in medicine over the last decades, the mortality and morbidity figures in elective gastrointestinal surgery remain at a low, but relatively constant level. They continue to represent a problem that leads to an extension of treatment time, postoperative recovery, le

Prophylactic Ureteral Catheterization for Preventing Ureteral Injury in Colorectal Cancer Surgery.

Background/Objective: Iatrogenic ureteral injury is a rare but serious complication of colorectal cancer surgery. Although prophylactic ureteral catheterization (PUC) is used to facilitate intraoperative ureter identification and reduce the risk of ureteral injury, its efficacy is debated. We aimed to evaluate the clinical utility and outcomes of PUC in colorectal cancer surgery. Methods: This retrospective study included 42 patients who underwent PUC before colorectal cancer surgery at the Tohoku University Hospital between February 2010 and September 2024. Preoperative ureteral stents were i

Cytoreductive surgery without intra-peritoneal chemotherapy for metachronous colorectal peritoneal metastases.

BACKGROUND: Cytoreductive surgery and chemotherapy reportedly improve the prognosis of patients with metachronous peritoneal metastases. However, the types of peritoneal metastases indicated for cytoreductive surgery remains unclear. Therefore, we aimed to clarify the category of cases for which cytoreductive surgery would be effective and report the prognosis associated with cytoreductive surgery for metachronous peritoneal metastases. METHODS: This study included 52 consecutive patients who underwent cytoreductive surgery for metachronous peritoneal metastases caused by colorectal cancer be

Deep Learning-Based Real-Time Ureter Identification in Laparoscopic Colorectal Surgery.

BACKGROUND: Iatrogenic ureteral injury is a serious complication of abdominopelvic surgery. Identifying the ureters intraoperatively is essential to avoid iatrogenic ureteral injury. We developed a model that may minimize this complication. IMPACT OF INNOVATION: We applied a deep learning-based semantic segmentation algorithm to the ureter recognition task and developed a deep learning model called UreterNet. This study aimed to verify whether the ureters could be identified in videos of laparoscopic colorectal surgery. TECHNOLOGY, MATERIALS, AND METHODS: Semantic segmentation of the ureter

Role of robotic natural orifice specimen extraction surgery in colorectal neoplasms.

Natural orifice specimen extraction surgery (NOSES) is especially suitable for colorectal surgery. Until now, most of the reports published were about laparoscopic NOSES, the reports about robotic NOSES are extremely rare. This study aims to explore the safety and feasibility of robotic NOSES for colorectal neoplasms. All patients underwent robotic NOSES from March 2016 to October 2019 in our hospital were enrolled for retrospective analysis. Clinicopathological data including patient characteristics, perioperative information and pathological information were collected and analyzed. According

Laparoscopic intraarterial catheterization with selective ICG fluorescence imaging in colorectal surgery.

The quality of mesorectal resection is crucial for resection in rectal cancer, which should be performed by laparoscopy for better outcome. The use of indocyanine green (ICG) fluorescence is now routinely used in some centers to evaluate bowel perfusion. Previous studies have demonstrated in animal models that selective intra-arterial ICG staining can be used to define and visualize resection margins in rectal cancer. In this animal study, we investigate if laparoscopic intra-arterial catheterization is feasible and the staining of resection margins when performing total mesorectal excision wi
