Trichophyton dermatophytosis--a disease easily confused with pemphigus erythematosus.

Trichophytosis is a rare diagnosis in dogs in Ontario, but recently 4 dogs with a scaling and crusting, alopecic, facial dermatitis have been so diagnosed. In all cases, the histopathological findings of severe epidermal and follicular interface dermatitis, accompanied by an acantholytic intraepidermal pustular dermatitis suggested an immune-mediated disease.

The renal axes in spina bifida: issues of confusion and fusion.

The occurrence of horseshoe kidneys in myelodysplasia has been suggested to be overestimated because of the concurrent prevalence of kyphotic spine in the spina bifida population. Pseudohorseshoe kidneys result from the actual medial migration and apposition of the lower renal poles in the deep fossa created by the gibbus deformity. The presence of a lumbosacral kyphosis, however, does not imply there is not a true horseshoe kidney. In our myelodysplasia population of 189 patients, occurrence of true horseshoe kidneys (13 patients) and pseudohorseshoe kidneys (14 patients) was increased. The f

[A postoperative syndrome of confusion and stupor or a central anticholinergic syndrome?].

A case is reported of a 23 year old man who sustained a central anticholinergic syndrome after undergoing upper limb surgery under regional anaesthesia and sedation with 2 mg midazolam and 75 micrograms fentanyl. The disturbances consisted of excitation and inhibition. On the 4th postoperative day, he developed progressive coma, quickly and totally reversed with 2 mg physostigmine. Chronic benzodiazepine therapy seems to be a facilitating factor of central anticholinergic syndrome. The low rate of this postoperative complication seems to proceed from failure to recognize it. Differential diagn

Confusion as the presenting manifestation of vertebral osteomyelitis: a case report.

A 44-year-old patient presented with increasing confusion. He was first diagnosed as having intermittent pressure hydrocephalus but a further evaluation showed CSF pleocytosis and hypoglycorrhachia. Five weeks later, his physical examination was unrevealing. Nuclear imaging techniques were conflicting, with negative gallium- and indium-labelled white blood cells scans but a Tc scan pointing towards a vertebral infection. A well-demarcated lesion in the T9 vertebral body, demonstrated by CT scan, confirmed the diagnosis of vertebral osteomyelitis. Although we were unable to recover the causativ…

Arcanobacterium hemolyticum infection: confused with scarlet fever and diphtheria.

Arcanobacterium hemolyticum infections are a common cause of pharyngitis and rash in the 10- to 30-year-old age group. Despite its prevalence, many emergency and primary care physicians may not be aware of the pathogenic potential of this organism. We present a case that illustrates the distinctive clinical spectrum of A. hemolyticum infections that may be confused with drug allergy, group A streptococcal scarlet fever, diphtheria, and even toxic shock syndrome. Recognition of this syndrome will reduce misdiagnoses and facilitate appropriate treatment.

Atypical insufficiency fractures confused with Looser zones of osteomalacia.

Six women, aged 24-67 years, had osteopenia and insufficiency fractures, which suggested a diagnosis of osteomalacia. The insufficiency fractures occurred at traditional sites for Looser zones, were multiple in number, were symmetrically distributed in three patients, and did not heal promptly. Bone mass was low, as assessed by single-energy photon absorptiometry at the midshaft of the radius. Two postmenopausal women had vertebral compression fractures. Biochemical indices and bone histomorphometric analysis excluded osteomalacia, and in vivo double tetracycline labeling in five patients reve

Familial dysalbuminemic hyperthyroxinemia: a syndrome that can be confused with thyrotoxicosis.

We investigated 15 euthyroid patients from eight families with a recently recognized syndrome, familial dysalbuminemic hyperthyroxinemia (FDH), that could be mistaken for thyrotoxicosis. The syndrome is characterized by elevations in serum thyroxine and the free-thyroxine index (FT4l), which are due to an abnormal serum albumin that preferentially binds thyroxine. This albumin has an abnormal binding site with a much greater affinity for thyroxine (relative to its affinity for triiodothyronine) than that of the hormone-binding site on thyroxine-binding globulin. Results of thyrotropin-releasin

Angiolymphoid hyperplasia with eosinophilia: a disease that may be confused with malignancy.

Twelve new cases of an unusual, benign vasoproliferative and inflammatory disorder of unknown etiology, angiolymphoid hyperplasia with eosinophilia (ALHE), are described and contrasted clinically and pathologically with those appearing in the literature. Only recently recognized in the United States, the disease is of singular importance because the vascular component may be confused histologically with angiosarcoma, thereby resulting in unwarranted aggressive therapeutic measures. ALHE characteristically affects adults and presents in the head and neck region as either solitary or multiple cu

Barton's fractures-reverse Barton's fractures. Confusing eponyms.

Anterior and posterior marginal fracture dislocations of the distal radius are unusual injuries, both of which are called Barton's fracture in the current world literature. Barton's original description delineates only the posterior marginal injury. An attempt is made to clarify the nature of these injuries by a complete review of the literature and a study of the clinical experience at UCLA Hospital from 1972 through 1975. Ten anterior and five posterior marginal fractures were treated during that period of time, constituting 2.3 per cent of all distal forearm fractures. Fractures in males ty

Buffers in intravenous solutions: is the source of bicarbonate a source of confusion?

There is a widespread belief that organic sodium salts included in intravenous solutions serve as bicarbonate precursors, and that this mechanism explains their effects on plasma pH. We aimed to explain why the effect of organic anions, such as citrate, acetate, gluconate, and lactate on the acid-base balance is independent of bicarbonate generation. For this purpose, we mainly focused on regional citrate anticoagulation (RCA). The sodium load provided with these buffers and its contribution to the plasma strong ion difference is a more suitable model for explaining and predicting their alkali

Anterior segment mass in miliary tuberculosis: a confusing scenario.

Tuberculosis (TB) is one of the most prevalent diseases in the Indian subcontinent. Ocular TB presenting as iris mass is rare; furthermore, invasion of anterior segment without choroidal invasion is even more scarce. Presented here is a case report detailing the condition of a patient in late adolescence who presented with diminution of vision and pain in the left eye. Slit lamp biomicroscopy revealed a nodular vascular mass emerging from the iris. Subsequently, the patient underwent a fine needle aspiration biopsy from the anterior chamber, which yielded a positive result for acid-fast bacill

Cytological diagnosis of lymphoepithelial carcinoma: Simple yet confusing.

Lymphoepithelial carcinoma (LEC) of salivary glands are rare tumours and present as gradually progressing painless masses. LECs have good prognosis and accurate differentiation from secondary tumours and nasopharyngeal carcinoma is essential to dictate management. We present a case of an elderly male presenting with a parotid swelling diagnosed as LEC on fine needle aspiration cytology and confirmed on histopathology and immunohistochemistry.

Pernicious anemia presenting with dysphagia and melanoderma: a confusing manifestation.

Vitamin B12 deficiency is widely recognized as a common cause of anemia. However, symptoms such as dysphagia, melanoderma, and pancytopenia, although less frequent, can also be associated with this deficiency. We report the case of a 47-year-old Caucasian man presented with dysphagia to solids associated to high heart rate, dyspnea and melanoderma. He was diagnosed with severe anemia (hemoglobin 4 g/dl) in association with pancytopenia. Further investigation confirmed that the underlying cause was severe vitamin B12 deficiency secondary to pernicious anemia. Subsequent treatment with vitamin B
