Stretch for the treatment and prevention of contractures.

BACKGROUND: Contractures are a common complication of neurological and musculoskeletal conditions, and are characterised by a reduction in joint mobility. Stretch is widely used for the treatment and prevention of contractures. However, it is not clear whether stretch is effective. OBJECTIVES: To determine the effects of stretch on contractures in people with, or at risk of, contractures. SEARCH STRATEGY: Electronic searches were conducted on CENTRAL, DARE, HTA (The Cochrane Library); MEDLINE; CINAHL; EMBASE; SCI-EXPANDED; and PEDro (April 2009). SELECTION CRITERIA: Randomised controlled tr…

Capsular contractures: a systematic review.

BACKGROUND: The aim of this article was to review the current literature on capsular contractures, focusing in particular on the epidemiology, risk factors, cause, and treatment modalities, to provide the plastic surgeon with an up-to-date review of the current available evidence. METHODS: A literature search was undertaken of the MEDLINE, EMBASE, and Cochrane Central Register of Controlled Trials databases. The search strategy was conducted using three groups of key words, with the first relating to the organ involved (breast), the second relating to the surgical procedure performed and rela…

Management of axillary burn contractures.

This article will review the basic principles and techniques of managing axillary burn contractures in both industrialized and developing nations. Surgeons specializing in hand and upper extremity surgery should be adept in treating axillary soft tissue deficits secondary to burn contractures. The focus of this article will be to provide suggestions for performing skin grafts and latissimus dorsi musculocutaneous flaps, as well as illustrate guidelines for postoperative rehabilitation.

Arthroscopic mobilization for contracture of the wrist.

PURPOSE: We evaluated the clinical efficacy of arthroscopic mobilization of the wrist for release of persistent traumatic contracture. METHODS: We retrospectively reviewed 11 consecutive patients who had undergone arthroscopic mobilization after trauma. Patients included 9 men and 2 women who ranged in age from 16 to 65 years (mean, 40 years). Injuries causing contracture included 8 fractures of the distal radius, 1 Galeazzi fracture, 1 perilunate dislocation, and 1 carpal bone contusion. A single radiocarpal septum extended from the proximal fibrocartilage of the scapholunate ligament to the…

Factors in the pathogenesis of Dupuytren's contracture.

This article reviews the basic science elements involved in the pathogenesis of Dupuytren's disease. Dupuytren's contracture is effected by a multitude of complex processes at the cellular level and the exact relative contribution of each is unknown. Various investigators have proposed different theories and documented individual findings regarding the pathophysiology of Dupuytren's contracture. The current report attempts to summarize many of these findings together in a schema of pathogenesis.

The salute splint for axillary contractures.

A new splint for use in the treatment of burn contractures of the axilla is presented. The splint is easy to deploy, comfortable for patients and achieves the same results as other splints.

The role of the fibroblast in Dupuytren's contracture.

Ultrastructural, immunohistochemical, and biochemical studies to date show that the fibroblast in Dupuytren's contracture is identical to palmar fascia fibroblasts in patients unaffected by Dupuytren's contracture, and to all other fibroblasts. The major difference relating to fibroblasts is that in Dupuytren's contracture there are more of them, and they are clustered around narrowed microvessels. It is probable that these two phenomena are linked because recent studies indicate a greater potential for ischemia-induced oxygen free radical generation in Dupuytren's contracture, and because oxy…

Upper extremity burn contractures.

Upper extremity burn contractures are a major challenge to the reconstructive surgeon. Despite increasing sophistication in the overall management of acute thermal injuries, contractures still occur and are the most common cause of skin contracture in the hand. Reconstructive options for axillary, antecubital, wrist, metacarpophalangeal joint, and interdigital web contractures are discussed, with special emphasis on the techniques and advantages of local flap reconstruction.

[Arthrolysis of the elbow in posttraumatic contracture].

Intra- and periarticular fractures about the elbow joint are treated with open reduction and internal fixation. This allows early functional after-treatment. Nevertheless, the range of motion remains more or less unsatisfactory. In these cases open arthrolysis provides a considerable improvement in joint function. We therefore recommend this operation when the hardware is removed about 9 months after the accident. The reasons for post-traumatic contracture of the elbow could be intrinsic such as interposed fragments, intra-articular adhesions, incongruity of the articular surfaces--or extrinsi…

[Prevention of decubitus, edema, contractures].

This paper presents a historical reminder of the development of medical care, a contemporary consideration of the division of care, as well as the definition of the term-quality medical care, and a review of the preventive measures for bed-sore, edema and contracture. In regard to the fact that care and therapy are a tightly bound process, they must also be applied as such. The definition of the term-quality medical care is an open question. The prevention of bed-sore, edema and contracture is the element of quality care.

An approach to research on congenital contractures.

We have described a study evaluating over 1,500 patients said to have arthrogryposis. It is clear that arthrogryposis is a sign and not a diagnosis. The approach was one of collecting information and sorting out specific groups of patients. We employed a flexible protocol that allowed us to go back and collect additional information from families. Our study was pared down to include 350 patients, who were studied in detail. A great deal of heterogeneity exists within what has been called arthrogryposis. We have developed an approach to patients with congenital contractures, which is to divide …

The role of the myofibroblast in Dupuytren's contracture.

A case report of Dupuytren's contracture illustrates the functional and light and electron microscopic characteristics of the myofibroblast. The myofibroblast is now associated with an increasing number of diseases, and is believed to be a morphologic and functional variant of the fibroblast, which shows changes typical of smooth-muscle cells. Its presence may be suspected by light microscopy, but its ultrastructure is characteristic. Its distinctive electron microscopic features are a fibrillar system in the cytoplasm similar to that of a smooth muscle, nuclear deformations indicative of cont…
