Imaging of ultrasound specularity at cortical bone interfaces to detect unbalanced remodeling: A preliminary study.

The region near the interface between cortical bone and the medullary cavity-the endosteal surface-is of particular interest for the early detection of osteoporosis. This region is typically the first to exhibit signs of unbalanced remodeling (enlarged pores and increased surface roughness). In this study, we analyze ultrasound reflection at the endosteal surface to introduce a novel potential biomarker of cortical bone health based on specularity. Our hypothesis is that increased pore size and surface roughness enhance ultrasound scattering, thereby reducing specular reflection. We reconstruc

Sites of non-enzymatic crosslinks pentosidine and glucosepane in collagen I of human cortical bone.

Structural alterations of bone extracellular matrix (ECM) may affect bone quality, which, along with bone mass, affects the ability of bone to resist fracture. In particular, an increase in advanced glycation end-product (AGE) crosslinking of type I collagen has been implicated in contributing to increased bone fragility in diabetes and ageing. However, the location of these crosslinks and their relative abundance is unknown, thus hindering our understanding of how AGE crosslinks impact bone quality. We analyzed physiological crosslinks, glucosepane (GN) and pentosidine (PE), in collagen I fro

Design and radiological confirmation of three-column cortical bone trajectory in the lower thoracic vertebrae.

PURPOSE: This study aims to design novel cortical bone trajectories for the lower thoracic vertebrae that traverse the anterior, middle, and posterior columns. We seek to establish novel theoretical guidelines and placement strategies to optimize the trajectory of CBT screws in osteoporotic patients. METHODS: Five different diameters of CBT screws, ranging from 3.5 mm to 5.5 mm, were designed. The tip of the simulated screw was aimed at the anterior half of the vertebral body, with appropriate cephalad and lateral angles. Screw placement was simulated using Mimics software, and key parameters

Comprehensive repair of the alveolar cleft using cortical and cancellous bone layers: A retrospective study.

To retrospectively review the clinical effect of comprehensive treatment of alveolar cleft (CTAC) using the mandible as the bone source. Patients with alveolar clefts who met the inclusion criteria were subjected to a CTAC protocol that included the following: (1) preoperative orthodontic treatment for creating good soft-tissue conditions; (2) 'area-like grafting' with subperiosteal osteogenic chin bone instead of cartilaginous osteogenic iliac bone; (3) simulation of normal bone anatomy via a sandwich-like bone graft consisting of 'cortical bone + cancellous bone + cortical bone'; and (4) str

Regenerative Potential of Hydroxyapatite-Based Ceramic Biomaterial on Mandibular Cortical Bone: An In Vivo Study.

Reconstruction of bone defects and maintaining the continuity of the mandible is still a challenge in the maxillofacial surgery. Nowadays, the biomedical research within bone defect treatment is focussed on the therapy of using innovative biomaterials with specific characteristics consisting of the body's own substances. Hydroxyapatite ceramic scaffolds have fully acceptable phase compositions, microstructures and compressive strengths for their use in regenerative medicine. The innovative hydroxyapatite ceramics used by us were prepared using the tape-casting method, which allows variation in

Comparison of Cortical Bone Trajectory to Pedicle-Based Dynamic Stabilization: An Analysis of 291 Patients.

OBJECTIVE: Pedicle-based dynamic stabilization (DS) has gained popularity outside of America. Although pedicle screw (PS) loosening has always been a concern, it is reportedly innocuous. Cortical bone trajectory (CBT) screw is an emerging option with less invasiveness and similar effectiveness to PS in short-segment lumbar fusion. This study aimed to verify the use of CBT for DS by comparing the outcomes between pedicle- and CBT-based DS. METHODS: Consecutive patients with lumbar spondylosis or low-grade spondylolisthesis who underwent 1- or 2-level DS between L3-5 with a minimum follow-up of

A Chemo-poroelastic Analysis of Mechanically Induced Fluid and Solute Transport in an Osteonal Cortical Bone.

It is well known that transport of nutrients and wastes as solute in bone fluid plays an important role in bone remodeling and damage healing. This work presents a chemo-poroelastic model for fluid and solute transport in the lacunar-canalicular network of an osteonal cortical bone under cyclic axial mechanical loading or vascular pressure. Analytical solutions are obtained for the pore fluid pressure, and fluid and solute velocities. Numerical results for fluid and calcium transport indicate that under a cyclic stress of 20 MPa, the magnitudes of the fluid and calcium velocities increase with

Adaptive changes in micromechanical environments of cancellous and cortical bone in response to in vivo loading and disuse.

The skeleton accommodates changes in mechanical environments by increasing bone mass under increased loads and decreasing bone mass under disuse. However, little is known about the adaptive changes in micromechanical behavior of cancellous and cortical tissues resulting from loading or disuse. To address this issue, in vivo tibial loading and hindlimb unloading experiments were conducted on 16-week-old female C57BL/6J mice. Changes in bone mass and tissue-level strains in the metaphyseal cancellous and midshaft cortical bone of the tibiae, resulting from loading or unloading, were determined u

Interaction of Microcracks and Tissue Compositional Heterogeneity in Determining Fracture Resistance of Human Cortical Bone.

Recent studies demonstrated an association between atypical femoral fracture (AFF) and long-term bisphosphonate (BP) use for osteoporosis treatment. Due to BP treatment, bone undergoes alterations including increased microcrack density and reduced tissue compositional heterogeneity. However, the effect of these changes on the fracture response of bone is not well understood. As a result, the goal of the current study is to evaluate the individual and combined effects of microcracks and tissue compositional heterogeneity on fracture resistance of cortical bone using finite element modeling (FEM

Design and validation of bending test method for characterization of miniature pediatric cortical bone specimens.

Osteogenesis imperfecta is a genetic disorder of bone fragility; however, the effects of this disorder on bone material properties are not well understood. No study has yet measured bone material strength in humans with osteogenesis imperfecta. Small bone specimens are often extracted during routine fracture surgeries in children with osteogenesis imperfecta. These specimens could provide valuable insight into the effects of osteogenesis imperfecta on bone material strength; however, their small size poses a challenge to their mechanical characterization. In this study, a validated miniature t

Long-term results of the use of autogenous cortical bone columellas to replace the stapes at stapedectomy.

The first author has been using an autogenous cortical bone columella to replace the stapes removed during stapedectomy since 1965. The audiograms of 21 of the 271 patients operated on with this method between 1965 and 1989 (i.e. 7.7% of the possible candidates) were available 20-35 (average 26.8) years postoperatively. The operation could be regarded as successful in 20 and unsuccessful in 1 patient in the long run. The audiological data are presented individually as averages of the values obtained at 0.5, 1, 2 and 3 kHz. The averaged data (n = 21): preoperative air conduction, 58.27; bone co

Cortical and trabecular bone microarchitecture and turnover in alcohol-induced chronic pancreatitis: a histomorphometric study.

Alcohol-induced chronic pancreatitis is associated with bone loss, but bone histomorphometric data describing the mechanism of cortical (Ct) and trabecular (Tb) bone loss are scarce. In this case-control study, we investigated 13 black male patients aged 41.2 +/- 8.9 years with alcohol-induced chronic pancreatitis by routine iliac crest cortical and trabecular histomorphometry and by biochemistry relevant to bone, liver function, and iron overload. Patients showed lower values for Ct thickness (P = 0.018), endocortical (Ec) wall thickness (P = 0.0002), Tb bone volume (0.019), Tb thickness (0.0

The influence of lean mass in trabecular and cortical bone in juvenile onset systemic lupus erythematosus.

The aim of this study was to evaluate risk factors for low bone mineral density (BMD) and vertebral fractures, in juvenile systemic lupus (JSLE). Thirty-one consecutive patients with JSLE were compared with 31 gender- and age-matched healthy controls. BMD and body composition from all participants were measured using dual-energy X-ray absorptiometry. Vertebral fractures were defined as a reduction of > or = 20% of the vertebral height for all patients. Lumbar spine and total femur BMD was significantly decreased in patients compared with controls (P = 0.021 and P = 0.023, respectively). A high

Long-term protective effects of zoledronic acid on cancellous and cortical bone in the ovariectomized rat.

UNLABELLED: Current bisphosphonate therapies effectively prevent bone loss in postmenopausal women. We studied the effect of a single intravenous dose of ZOL in ovariectomized rats. Protection from bone loss was dose dependent, lasting for up to 32 weeks, supporting the rationale for an annual intravenous dosing regimen of ZOL for treatment of postmenopausal osteoporosis. INTRODUCTION: Once-yearly dosing with zoledronic acid (ZOL) 5 mg can increase BMD and reduce fracture rate in postmenopausal women with low BMD. The primary objective of this study was to determine the duration of bone prote

Effect of cortical thickness and cancellous bone density on the holding strength of internal fixator screws.

Internal fixators are a new class of implants designed to preserve the periosteal blood supply of the bone. In contrast to conventional plate fixation in which the screws have spherical heads and are loaded mainly by axial pullout forces, screws in internal fixators are "locked" within the plate and therefore subjected to axial as well as bending loads. In this study the ultimate loads of screws of a commercially available internal fixator system were tested in a pullout (n = 72) and cantilever bending mode (n = 72) in metaphyseal and diaphyseal regions of four pairs of human tibiae with diffe

The trapdoor procedure using autogenous cortical and cancellous bone grafts for osteonecrosis of the femoral head.

We have reviewed the results of 30 operations performed on 23 patients with Ficat stage-III or stage-IV osteonecrosis of the femoral head in which autogenous cortical and cancellous bone grafting had been performed through a so-called trapdoor made in the femoral head. At a mean of 56 months (30 to 60) after operation 20 of 24 stage-III hips (83%) had a good or excellent result as determined by the Harris hip-scoring system. Two of six stage-IV hips (33%) had good or excellent results. Eighteen of 21 hips (86%) with a combined necrotic angle of 200 degrees had good or excellent clinical result
